Opinnäytetyön tarkoituksena oli tuottaa ohjausrunko vastasairastuneen tyypin 1 diabetespotilaan alkuohjauksesta Espoon diabeteskeskuksessa ja tavoitteena oli yhtenäistää potilasohjausta. Opinnäytetyö on sidottu Ohjaus hoitotyössä -hankkeeseen, jonka tarkoituksena on tuottaa opinnäytetöitä potilasohjauksesta sisätautien hoitotyön alueelta ja tavoitteena on kehittää potilasohjausta.
Opinnäytetyön aihe saatiin työelämän edustajalta Espoon diabeteskeskuksesta. Tutkimuskysymyksen laatimisessa käytettiin teoriatietoa vastasairastuneen diabetespotilaan hoidosta ja ohjauksesta. Teoriatiedon avulla laadittiin tutkimuskysymys, mikä on tyypin 1 diabeetikon alkuhoidon ja alkuohjauksen sisältö.
Tutkimus toteutettiin kvalitatiivisena eli laadullisena tutkimuksena ja aineistonkeruumenetelmänä käytettiin puolistrukturoitua teemahaastattelua. Haastattelu toteutettiin ryhmähaastatteluna, johon osallistui kolme diabeteskeskuksen hoitajaa. Haastattelu nauhoitettiin ja aineisto analysoitiin teoriaohjaavalla sisällönanalyysillä.
Ohjausrungon laatimisessa käytettiin teoriaohjaavan sisällönanalyysin tuloksia. Valmis ohjausrunko lähetettiin yhteyshenkilölle diabeteskeskukseen, diabeteskeskuksen hoitajat kävivät valmiin ohjausrungon läpi ja antoivat kehitysehdotuksia. Kehitysehdotukset toteutettiin ja ohjausrunko lähetettiin yhteyshenkilölle. Diabeteskeskuksen hoitajat ovat ottaneet ohjausrungon käyttöön helmikuussa 2015.
Ohjausrungon käyttökokemuksia ja toimivuutta ei tämän opinnäytetyön puitteissa ole tutkittu. Kehittämisehdotuksena nousi esiin mahdollisuus tutkia jatkossa ohjausrungon käyttökokemuksia ja toimivuutta Espoon diabeteskeskuksessa.

The purpose of this thesis was to create structured guidance guidelines for the nurses at Espoo Diabetes Center to counsel patients who are newly diagnosed with type 1 diabetes. The goal was to standardize patient education. The thesis was conducted within the Guidance In Nursing –project the purpose of which is to produce theses about patient education in the field of medical nursing and to develop patient education.
The theme for the thesis was suggested by the nurses in Espoo Diabetes Center. The study question was created based on theoretical data about the care and guidance of patients who are newly diagnosed with type 1 diabetes. The study question was: What are the contents of the initial treatment and patient education with patients who are newly diagnosed with type 1 diabetes?
The study was carried out as a qualitative research and the method for collecting the data was a semi-structured interview. The interview was executed as a group interview for three nurses from Espoo Diabetes Center. The interview was recorded and afterwards analyzed with theory driven content analysis.
The structured guidance guidelines were made based on the findings from the theory driven content analysis. The finalized guidelines were sent to our contact person in Espoo Diabetes Center. The nurses in the Diabetes Center studied the guidelines and presented some ideas for improvement. We implemented the ideas and sent the finished guidelines to the Diabetes Center. The nurses in the Diabetes Center have taken the guidelines to daily use in February 2015.
User experiences and functionality of the structured guidance guidelines have not been studied in this thesis. As a development proposal we suggest that the user experiences and functionality of the guidelines should be examined in Espoo Diabetes Center.
