Etälääkäripalvelut ovat yleistymässä, koska kunnat ostavat yhä enemmän palveluja yksityisiltä tuottajilta. Etälääkäripalvelussa korostuu sujuvan dokumentoinnin merkitys, jotta potilasturvallisuus pystytään takaamaan. Kiinnostus opinnäytetyön tekemiseen lähti toisen opiskelijan etälääkäripalvelukokemuksesta.
Teimme opinnäytetyömme etälääkäripalvelua tuottavalle yritykselle. Tutkimushankkeen tarkoituksena oli selvittää miten sairaanhoitajat toteuttavat dokumentointia potilastietoihin sekä millä tavalla potilasturvallisuus toteutuu dokumentoinnissa.
Olemme valinneet opinnäytetyöhön mielestämme luotettavia ja uutta tietoa sisältäviä lähteitä. Opinnäytetyön tietoperusta pohjautuu yhteistyökumppanin edustajan antamaan aineistoon, jota tukeaksemme olemme hyödyntäneet kirjallisuutta, alan internet-sivuja, lehtiartikkeleita sekä väitöskirjoja. Opinnäytetyössämme olemme tutkineet ikääntyneiden palvelukodeissa työskentelevien sairaanhoitajien kokemuksia etälääkäripalvelun dokumentoinnista Webropol-kyselyn avulla. Kyselyssä otimme huomioon yhteistyökumppanimme toiveet sekä teimme sen tietoperustaan pohjautuen. Opinnäytetyö on toteutettu käyttämällä triangulaatio-menetelmää, eli sairaanhoitajien kokemuksia käsitellään sekä määrällisesti että laadullisesti. Määrällisiä vastauksia olemme analysoineet frekvenssi- ja prosenttijakaumin sekä laadullisia vastauksia deduktiivisella sisällönanalyysillä.
Tutkimuksesta selvisi, että hoitajat ovat suurimmaksi osaksi käyneet potilastietojärjestelmään kirjaamisen koulutuksessa, josta he ovat saaneet ohjeita toteuttaakseen dokumentointia. Sairaanhoitajat toivat esille, että hoitajien ja lääkärien käyttämät eri tietojärjestelmät vaikeuttavat dokumentointia ja potilasturvallisuutta. Tästä huolimatta sairaanhoitajat kokivat dokumentoinnin sujuvan potilasturvallisesti.
Kehitysehdotuksena olisi mahdollistaa lääkäreille ja hoitajille yhteisen potilastietojärjestelmän käyttäminen tai tarjota lääkäreille etäkäyttömahdollisuus potilastietojärjestelmään. Yhteistyökumppanimme voi myöhemmin kehittää käytäntöjään tutkimuksesta selvinneiden ongelmakohtien osalta. Jatkotutkimusehdotuksena olisi laajentaa tutkimukseen osallistuvien henkilöiden määrää tai toteuttaa tutkimus laadullisena.Remote doctor services are becoming more common, because the municipalities are buying more and more services from private providers. On the remote doctor service it is important that the documentation is smooth and then patient safety can be guaranteed. Interest in for thesis work arise another student’s remote doctor service experience.
We made our thesis work to the company which produces remote doctor service. Purpose of the research project was to find out how registered general nurses implemented the patient record documentation and how the patient safety was realized on documentation.
The source materials for the thesis work that we have selected are reliable and contain new information. Thesis work is based on knowledge base of provided data from partner representative and we have utilized to support this obtained information by using literature, internet pages of the industry, journal articles and dissertations. In our thesis work we have examined experiences of remote doctor service documentation via Webropol inquiry from registered general nurses who working in older people’s service homes. The inquiry took into account the wishes of our partner and we did it using to the knowledge base. This thesis work has been implemented using triangulation method in other words considering nurses’ experiences considering with both quantitatively and qualitatively. Quantitative answers, we analyzed the frequency and percentage terms as well as qualitative answers with deductive content analysis.
The research showed that most part of nurses have been patient record system documentation training, which they have received guidance on the documentation. Registered general nurses made the point that the various patient record systems used by the nurses and the doctors make it difficult for documentation and patient safety. Nevertheless, the nurses felt that the documentation of the patient can be carried out safely.
Development proposal could be allows that doctors and nurses can use common patient record systems or to provide doctors remote access to patient record system. Our partner can later develop their practices which related to the problem areas what we found during the research. For further research proposals should be to expand the number of people participating in the research or carry out research as a qualitative
