Opinnäytetyö oli tutkimuksellinen kehittämistyö, jonka tarkoituksena oli kehittää uusi toimintamalli lapsiperheiden varhaiseen tukemiseen Ylä-Savon SOTE kuntayhtymässä. Toimintamallin tarkoituksena on selkiyttää varhaisen tuen perhetyön työtapoja perheiden tukemisessa ja perheiden ohjaamista perhetyöhön. Toimintamallin tavoitteena on lapsiperheiden tuen tarpeen tunnistaminen varhaisessa vaiheessa.
Kehittämistyössä hyödynnettiin laadullisen tutkimuksen aineistonkeruun ja analysoinnin menetelmiä Kehittämistyöhön osallistettiin henkilöstöä ja asiakkaita. Aluksi aineistoa kerättiin kartoittamalla teoriatietoa toimintamallia varten. Toimintamallin viitekehyksen pohjalle valittiin Varhaisen puuttumisen malli (Pirskanen) ja Perheiden hyvinvoinnin osa-alueita arvioiva malli (Myllärniemi). Tämän jälkeen toimintamallia kehitettiin henkilöstön ja asiakkaiden haastattelujen analysointiin perustuen.
Kehittämistyössä kehitettiin toimintamalli, joka sisältää kolme eri tuotosta. Ensimmäisenä kehitettiin lapsiperheiden tuen tarpeen tunnistamisen malli lapsiperheiden kanssa työskenteleville. Malli perustuu dialogiin perheiden kanssa. Toisena kehitettiin varhaisen tuen perhetyön mallinnus, jossa kuvataan perhetyön prosessi. Mallinnus perustuu perheen ja työntekijöiden yhteistyöhön sekä moniammatillisuuteen. Lisäksi kehitettiin varhaisen tuen perhetyöhön ohjaava tuen tarpeen arviointimenetelmä, joka toimii keskustelun välineenä perhetyön tavoitteiden ja toteutuksen arvioinnille. Menetelmä antaa mahdollisuuden myös perhetyön vaikuttavuuden arviointiin.
Toimintamalli päätettiin ottaa käyttöön Ylä-Savon SOTE kuntayhtymässä. Sitä voidaan käyttää muissakin organisaatiossa. Toimintamalli vaatii koulutuksen ja perehdyttämisen sen käyttöönotossa. Toimintamalli on sovellettavissa myös kansainväliseen käyttöön.The thesis was an investigative development work, the purpose of which was to develop a new operating model for supporting families with children in the Ylä-Savo SOTE Joint Municipal Authority. The purpose of this model is to clarify the working methods of early support family work and how families are referred to family work. The aim of the operating model is to identify the families’ need of support as early as possible.
The data collection and analysis methods of qualitative research were used in this development work that was participated by employees and clients. At first data was collected by identifying relevant theory for the operational model. The basis of the model’s frame of reference came from The model of the early intervention (Pirskanen) and The assessment model of family well-being (Myllärniemi). After this the operating model was developed based on the analysis of the interviews with the employees and clients.
The operating model includes three different outputs. The first one was a model for the identification of the families’ needs families. This model would be used by those working with families, and it is based on a dialogue with families. The second one was a model of the early support family work describing the process of family work. The model is based on co-operation between the family and the professionals and on a multi-professional work approach. The third one was an assessment method of instructive family support needs for the early support family work. This method can be used for determining the objectives of family work, and it also provides an opportunity to assess the effectiveness of family work.
The Ylä-Savo SOTE Joint Municipal Authority decided to adopt the model. Moreover, it can also be used in other organizations. The model requires education for and orientation to its introduction. The model can also be applied for international use
