Opinnäytetyön tarkoitus oli kuvata terveydenhoitajien kokemuksia laajan 4-vuotisterveystarkastuksen yhteydessä käytettävän tiedonsiirtolomakkeen toimivuudesta. Tavoitteena on opinnäytetyön avulla saada uutta tietoa tiedonsiirtolomakkeen toimivuudesta terveydenhoitajien näkökulmasta. Tuloksia voidaan hyödyntää kehitettäessä ja tehostettaessa neuvolan ja päivähoidon välistä tiedonsiirtoa.
Opinnäytetyö toteutui yhteistyössä Katse lapseen -hyvinvointihankkeen kanssa. Hankkeen tarkoitus on Hyve-mallin mukaisesti hyvinvointitiedon kokoaminen laajassa 4-vuotisterveystarkastuksessa. Tiedonsiirtolomake on laaja-alainen, vanhempien, päivähoidon ja terveydenhoitajan käyttämä lomake, jossa arvioidaan neljävuotiaan lapsen kehitystä ja käyttäytymistä. Tiedonsiirtolomakkeen tavoite on tukea lapsen kehitystä ja kasvua ja helpottaa lapsen kanssa työskentelevien tahojen yhteistyötä lapsen parhaaksi. Opinnäytetyön aineisto koostui kahden terveydenhoitajan haastatteluista, haastattelut tehtiin yhdessä Vantaan neuvoloista. Aineisto analysoitiin laadullisella sisällönanalyysillä.
Opinnäytetyöstä saatujen tulosten mukaan tiedonsiirtolomakkeen täyttämiskäytänteet ovat joiltakin osin puutteelliset. Neuvolassa ei ole selvää yhtenäistä kirjallista ohjetta siitä miten tiedonsiirtolomaketta tulisi täyttää. Tuloksien mukaan tiedonsiirtolomake on hyvä yhteistyönväline neuvolan, päivähoidon ja vanhempien välillä. Terveydenhoitajat saavat tiedonsiirtolomakkeen avulla paljon tietoa lapsesta päivähoidolta ja vanhemmilta. Lisäksi tiedonsiirtolomake helpottaa asioiden puheeksi ottamista vanhempien kanssa. Jatkokehittämisehdotuksena on tiedonsiirtolomakkeen sähköistäminen sekä tiedonsiirtolomakkeen yhtenäisten kirjallisten täyttöohjeiden luominen.Data transfer form in a comprehensive 4 year health check as experienced by public health nurses
The purpose of the thesis is to describe how public health nurses experience the functionality of the data transfer form in a comprehensive 4 year health check. The aim of the thesis is to produce new information of the functionality of the data transfer form from the viewpoint of public health nurses. The aim is also to develop and enhance with the collected information the communication between the child health clinics and day care centres.
Data transfer form is a broad-based form filled in by the parents, daycare personnel and public health nurses which assesses a four year old child's development and behavior. The objective of the data transfer form is to support the child's growth and development and to facilitate the cooperation between the daycare personnel and public health nurses for the child's best interests. The thesis is done in cooperation with Katse lapseen welfare project. The project aims at collecting information on welfare following the Hyve-malli in a comprehensive 4 year health check. Qualitative research method was applied in the study. The data was collected by interviewing two public health nurses at a child health clinic in Vantaa.
According to the results of the thesis, the filling guides of the data transfer form are incomplete. There are no clear single written instructions how to fill the data transfer form in a child health clinic. According to the results, the data transfer form however is a good collaboration tool between the child health clinic, the daycare personnel, and the parents. With the data transfer form nurses get a lot of information about the child from the daycare, which helps broaching subjects with the parents, and in addition the parents are also taken into consideration in the data transfer form. The suggestion for further development is to create an electronic data transfer form and to compile coherent written filling-in instructions for the data transfer form
