Opinnäytetyömme tarkoituksena on tutkia oikeuspsykiatrista kuntoutusta ja hoitoa Suomessa sekä selvittää hoitoa antavat yksiköt. Opinnäytetyössämme luomme katsauksen oikeuspsykiatrisen hoitotyöhön liittyviin erityispiirteisiin: tahdonvastaiseen psykiatriseen hoitoon, rikosoikeudelliseen oikeuspsykiatriaan sekä mielentilatutkimuksiin. Lisäksi käsittelemme oikeuspsykiatrisen potilaan hoitotyöhön liittyviä asioita, kuten lääkehoitoon liittyviä käytänteitä, potilaiden väkivaltaista käyttäytymistä sekä psykoedukaatiota. Opinnäytetyössämme esitellään oikeuspsykiatrista kuntoutusta antavat yksiköt: Niuvanniemen sairaala, Kellokosken sairaala sekä Vanhan Vaasan sairaala.
Työssämme otamme myös esille psykiatriseen kuntoutukseen liittyviä kokonaisuuksia: psykiatrinen kuntoutus, hoitomenetelmät, hoitajan tehtävät ja merkitys toivon ylläpitäjänä sekä kuntoutuksen kokonaisvaltaisuus, ohjaava työote sekä kuntoutusprosessi. Lopuksi käsitellään mielenterveyskuntoutuksen haasteita, kuten potilaalle sopivia kuntoutusmuotoja ja potilaan osallisuutta omaan kuntoutukseensa. Opinnäytetyömme tutkimuskysymyksinä olivat: Missä annetaan oikeuspsykiatrista hoitoa Suomessa? Mitä oikeus-psykiatrinen kuntoutus on?
Opinnäytetyömme toteutimme narratiivisena eli kertovana kirjallisuuskatsauksena. Keräsimme tietoa eri lähteistä oikeuspsykiatrisesta kuntoutuksesta ja hoitoyksiköistä. Kokosimme aineiston ensiksi tekstiksi teoriaosuuteen ja sen jälkeen kokosimme lähteistä saamamme tulokset kahteen taulukkoon. Taulukoinnin avulla hahmotimme keskeiseksi tulokseksi oikeuspsykiatrisessa kuntoutuksessa potilaiden moniongelmaisuuden useine liitännäissairauksineen ja sosiaalisine ongelmineen. Myös vihan ja käyttäytymisen hallinta ovat keskeisiä piirteitä oikeuspsykiatrisen potilaan hoitotyössä. Potilaan oma osallistuminen hoitoonsa ja toivon ylläpitäminen kuntoutusprosessin aikana nähdään useassa tutkimuksessa sekä lähdekirjallisuudessa tärkeänä. Arviointi kuntoutusprosessin aikana sekä pienten tavoitteiden ja pitkän aikavälin tavoitteiden asettaminen on myös tärkeää. Käyttämissämme lähteissä on tuotu myös esille hoitajiin kohdistuva väkivalta sekä hoitajien kokemuksia väkivallan kohteeksi joutumisesta.
Toivomme, että Mikkelin ammattikorkeakoululle on hyötyä tekemästämme opetusmateriaalista oikeuspsykiatrian kuntoutuksen kokonaisuudesta. Lisäksi pyrimme siihen, että keräämämme tietoa voidaan hyödyntää hoitotyön opiskelijoiden psykiatrisen hoitotyön koulutuksessa sekä muiden asiasta kiinnostuneiden keskuudessa.The aim our thesis is to explore forensic psychiatric rehabilitation and nursing and find the care units in Finland. In the thesis we take a glance at the special features of forensic psychiatric nursing: involuntary psychiatric treatment, forensics in criminal justice system and psychiatric assessments. In addition we discuss the matters dealing with forensic psychiatric nursing such as medical care practices, violent behav-ior and psycho education. In the thesis we also present the care units of forensic psychiatry in Finland: the Niuvanniemi Hospital, Kellokoski Hospital and Vaasa Hospital.
We also discuss the entities considering psychiatric rehabilitation: psychiatric rehabilitation, the nurse’s duties and meaning as the preserver of hope as well as the comprehensiveness of rehabilitation, instructive approach to work and rehabilitation process. In the end we discuss the challenges of rehabilitation in men-tal health care such as the right ways of rehabilitation for the patient them and the patient’s inclusion to his or her rehabilitation. The research problems of our thesis were: Where in Finland forensic psychiatric care is given? What is forensic psychiatric rehabilitation?
We have implemented our thesis as a narrative literature review. We gathered information from different sources about forensic psychiatric rehabilitation and about care units. We first gathered the material as a text in the theory part and after that gathered the results from the sources into two tables.
Through tables we perceived as the main result in forensic psychiatric rehabilitation the patients who have many problems and several accompanying illnesses as well as social problems. Also anger management and behavioral control are key features in the nursing of a forensic psychiatric patient. Patient’s participa-tion and preserving the hope under the rehabilitation process are considered important in several research-es and in the source literature. Evaluation under the rehabilitation process and setting both short and long term goals is also important. The sources we used also discussed the violence towards the nurses and the experiences of nurses of being the target of violence.
We hope the education material we made considering the entity of psychiatric rehabilitation to be useful for the Mikkeli University of Applied Sciences. We also, by the knowledge we gathered, aim to offer valuable material in the education of the nursing students and among others interested in the theme
