Significance of liver biopsy in clinical practice: recommendations of Hepatology Group of Polish Society of Gastroenterology

Abstract

Biopsja wątroby mimo wielu ograniczeń jest „złotym standardem” w ocenie stanu tego narządu u pacjentów z chorobami miąższowymi i cholestatycznymi wątroby. Postęp w zrozumieniu zjawisk patofizjologicznych i poznanie historii naturalnej chorób wątroby, jak również pojawienie się nowych metod diagnostycznych o charakterze nieinwazyjnym, zmieniły w ostatnim czasie wskazania do wykonywania biopsji wątroby. Bez względu na te zmiany, histopatologiczna ocena wątroby nadal stanowi podstawę wnioskowania diagnostycznego, rokowniczego i decyzyjnego w kwestii wyboru optymalnej terapii. Artykuł jest zbiorem polskich rekomendacji dotyczących znaczenia biopsji wątroby w praktyce lekarskiej. Celem pracy jest podsumowanie zasad bieżącej praktyki w stosowaniu biopsji wątroby u dorosłych chorych z rozlanymi i ogniskowymi chorobami wątroby. Przedstawiono przegląd wiedzy na temat techniki wykonywania i wskazań do biopsji wątroby. Tematem pracy są również powikłania i przeciwwskazania do wykonywania tego zabiegu.Liver biopsy, despite many limitations, is the “gold standard” method of assessing liver status in patients with parenchymal and cholestatic liver diseases. Recent advances in the understanding of the pathophysiology and natural history of liver diseases, as well as the introduction of novel non-invasive methods assessing liver status have instigated changes in indications for liver biopsy. Regardless of these changes, histological liver assessment still remains the basis of diagnosis, prognosis and guides decision-making in patients with liver diseases. In the current manuscript we present Polish recommendations concerning liver biopsy. This position paper was developed by gastroenterologists, infectious diseases specialists and pathologists. Its aim is to summarize the current practice of liver biopsy in adults with disseminated and focal liver diseases. We present an overview of techniques and indications for liver biopsy. Finally we address the complications and contraindications of this invasive procedure
