The Delphi methodsolicits the opinion of an expert panel through a care-fully designed questionnaire which in this case includedquestions on: epidemiology, diagnosis, treatment ofsymptoms, drug choice, adverse events, follow-up visits,hospital and emergency service admissions. Theresponses were analyzed and discussed in a face to facemeeting followed by consensus building steps.ResultsProfiles of sJIA in Turkey were evaluated based on clini-cal experience of panelist. The percentage of patientsdiagnosed with JIA was found 0,004% in the generalpopulation in Turkey. Estimated number of JIA patientswas reported 342 per year. Twenty of them were diag-nosed sJIA. Estimated sJIA patients were 55 per year.The median age of diagnosis was 5.5 years. Consensuswas achieved on diagnosis of the patients which estab-lished by detail investigation inpatients clinics. Bothinfectious and malignancy must be excluded in thepatients before diagnosis.The all sJIA patients was distributed according to sub-types; monocyclic 23%, polycyclic 30% and persistent/polyarticular 47%. The mortality rate of sJIA patientswas found 1%. The most used drugs were reported asglucocorticoid (100%), methotrexate (58%) and cyclos-porine (10%), respectively. Etoposide and IVIG wereused only selected patients. In the treatment of diseasewas determined to be the most preferred drug anakinraafter failure DMARDs therapies. Also, we determinethat canakinumab and tosilizumab were equally pre-ferred (15-20%). The efficacy of the three drugs wasexpressed as the same (80-90%). The selection criteria ofbiological agents used in the treatment were reported:joint involvement was evident, the presence of MAS,characteristics of the patient, effective availability andeasy accessibility. The total duration of the treatmentwas reported two years. The remission rates have beenreported to according to the course of the disease;monophasic disease recovered as 100% while polycyclicand poliarticular forms’ remission rate were reported80% and 60%. The morbidity of polyarticular JIA hasbeen reported as high such as joint deformities, growthretardation, delayed puberty, and osteoporosis.ConclusionsThis consensus, produced through a modified Delphiprocess, reflects our current recommendations for thediagnosis and management of sJIA in Turkey.
