Introduction: In Brazil, the health service’s environment lives a paradox when it comes to the medicine’s relationship with humanization and empathy. It is possible to realize the growth of a movement in favor of changing this reality, starting with the introduction of the term “humanization” in public policies concerning health. Objectives: analyze and review the history of humanization, highlighting ethical and legislative questions, as well as showing reality and challenges for its implementation. Methods: It is an integrative literature review about the theme “humanization in medicine”. 17 articles were selected, published between 2010 and 2021. Results: It all started in the first years of the 60's, with the attention for women’s health. The turn of the century was considered historical when it was implemented the Humanization Policy of the Ministry of Health. It was realized that it was necessary a greater contact of the student with the patient to develop a higher empathy and sensibility. So, a few strategies with PBL (Problem Based Learning) method were born, and many others such as the use of the seventh art, tutoring and different shapes of anamnesis. Conclusion: the curricular guidelines adjusted to the new reality, promoting the holistic vision of the patient as well as the public policies. The current question is searching for the best way of changing a student 's behaviour effectively. The process of changing traditional teaching to the PBL method associated with the searching of new strategies and to the old, but good, makes today’s academics a protagonist of their own formation. It is necessary to do some constant work in both environments of teaching in favor of metrifying the student’s evolutions towards empathy inside medicine. Introdução: No Brasil, o ambiente do serviço de saúde vive um paradoxo no que tange o relacionamento da medicina com a humanização e a empatia. Pode-se perceber o crescimento de um movimento em busca da modificação desta realidade. Objetivo: Analisar e revisar a história da humanização nos cuidados médicos, destacando as questões éticas e legislativas, bem como evidenciar a realidade e os desafios para a sua implementação. Método: Revisão integrativa da literatura sobre “humanização na medicina”. Foram selecionados 17 artigos, publicados entre 2010 e 2021. Resultados: Observa-se o início das politicas de saúde com aspectos humanísticos a partir dos anos 60 com a atenção à saúde da mulher. A virada do século foi considerada histórica ao se implementar a Política de Humanização do Ministério da Saúde. Percebeu-se que era necessário um maior contato do estudante com o paciente para desenvolver uma maior empatia e sensibilidade e assim nasceram estratégias como o método de Aprendizagem Baseada em Problemas (ABP) e muitas outras como o uso da sétima arte, monitorias e formas diferentes de anamnese. Conclusão: As diretrizes curriculares se ajustaram à nova realidade promovendo a visão holística do paciente bem como as políticas públicas. A questão atual é buscar a melhor forma de mudar o comportamento dos alunos de forma efetiva. A mudança do ensino tradicional para o método ABP associado à busca pelas novas estratégias de ensino mescladas com o que era antigo, mas funcional, faz do acadêmico de hoje um ser protagonista da sua própria formação. Faz-se necessário pesquisas constantes em ambos os ambientes de ensino em prol de se metrificar a evolução dos alunos em relação à empatia dentro da medicina
