In this paper, we suggest an open eco system model “Community-based living lab” to support people with dementia for safe and happy living. In this model, instead of establishing a closed and secure area for people with dementia, open and reciprocal community is proposed as one of the optimal strategy for future aging society. The suggested open community is characterized with co-creation among people with dementia, industrial partners, municipalities, local care institutions, and citizens, and living lab where stakeholders conduct diverse experimental and spontaneous initiatives. We first introduce and analyse a case in Omuta city, Japan, and then, extract success factors of the Omuta’s dementia care programs. The community-based living lab extracted from Omuta’s success case, contributes in enhancing Omuta’s success, and conducting sustainable improvement of the fast-chaining social infrastructure for wider aging communities. Finally, we discuss a critical aspect of the model
