ResumeBaggrund: Et antal hjernesygdomme, herunder epilepsi og Alzheimers sygdom er blevet associeret med unormale ændringer i antallet af synapser i hjernen. Kvantificeringen af synapser i den menneskelige hjerne afhænger på nuværende tidpunkt af hjernevæv fra kirurgisk resektion eller obduktion, hvilket begrænser muligheden for at opnå tidlig diagnosticering og behandlingsmæssig overvågning. Derfor er en ikke-invasiv metode til at kvantificere densiteten af synapser i den levende hjerne ønsket. Synaptiske vesikel proteiner er blevet foreslået at være primære markører for densiteten af synapser på grund af deres begrænsede lokalisering i synaptiske vesikler og allestedsværende fordeling i hjernen. PET radioliganden [11C]UCB-J ([(R)-1-((3-(11C-methyl-11C)pyridin-4-yl)methyl)-4-(3,4,5-trifluorophenyl) pyrrolidin-2-one]), som er målrettet mod det synaptiske vesikel protein 2A (SV2A) er for nylig blevet udviklet. SV2A er lokaliseret i pre-synaptiske vesiklers membran, og er allestedsværende fordelt i hjernen.Formål: Formålet med dette studie var at karakterisere ligand-bindings egenskaber for UCB-J, og de antiepileptiske medikamenter Brivaracetam og Levetiracetam til det pre-synaptiske protein SV2A i rotte, gris og human cortex. Yderligere var formålet at finde og sammenligne niveauet af SV2A og synaptophysin (SYN) proteiner i fem forskellige områder af rottehjernen.Metode: Bindings-karakteristika (Kd, Bmax, and Ki) for UCB-J, Brivaracetam and Levetiracetam blev bestemt ved in vitro autoradiografi mætnings og displacerings forsøg på hjernesnit fra rotte, gris og menneske. Fordelingen af SV2A og SYN blev undersøgt ved hjælp af western blotting i rotte hippocampus, striatum, hjernestamme, cerebellum, og frontal cortex.Resultat: Kd værdien af UCB-J blev fundet til henholdsvis 6.8 ± 1.5 nM i rotte, 3.2 ± 0.3 nM i gris og 2.3 nM i human cerebral cortex, og Bmax værdierne blev fundet til henholdsvis 1881 ± 207 fmol/mg vævsækvivalent (”tissue equivalent”, TE) i rotte cortex, 1003 ± 35 fmol/mg TE i gris cortex og 998 fmol/mg TE i human cerebral cortex. Ki værdien af det anti-epileptiske medikament Levetiracetam blev detekteret ved displacerings forsøg i de tre racer og blev fundet til at være henholdsvis 14.3 ± 9.0 μM i rotte, 11.1 ± 0.7 μM i gris og 3.3 μM i human cerebral cortex. Ki for UCB-J og Brivaracetam blev bestemt til henholdsvis 32.9 ± 5.4 nM og 5.0 ± 3.0 μM i rotte cortex. Western blot analyse viste et betydeligt højere niveau af SV2A og SYN i cerebellum.Konklusion: Autoradiografi-resultaterne indikerer at bindings karakteristika for UCB-J og Levetiracetam er sammenlignelige i alle tre arter, og at UCB-Js affinitet af er mere end 100 gange mere potent end Brivaracetams og Levetiracetams. Yderligere var Kd værdierne i god overensstemmelse med resultaterne fra tidligere studier, hvor Kd værdien af UCB-J er blevet bestemt ved henholdsvis PET og flydende scintillering ("liquid scintillation"). AbstractBackground: A number of brain disorders, including epilepsy and Alzheimer's disease, have been associated with abnormal changes in the number of synapses in the brain. Presently, quantification of synapses in the human brain depends on examination of brain tissue from surgical resection or autopsy, which limits the possibility of obtaining early diagnosis and therapeutic monitoring. Thus, a non-invasive method for quantifying synaptic density in the living brain is desired. Synaptic vesicle proteins have been suggested to be prime markers of synaptic density due to their restricted localisation in synaptic vesicles and ubiquitous distribution in the brain. Recently, the PET radioligand [11C]UCB-J ([(R)-1-((3-(11C-methyl-11C)pyridin-4-yl)methyl)-4-(3,4,5-trifluorophenyl) pyrrolidin-2-one]) which targets the synaptic vesicle protein 2A (SV2A) has been developed. SV2A is located in the membrane of presynaptic vesicles and is ubiquitously distributed in the brain. Aim: The aim of the present study was to characterise ligand-binding properties of UCB-J, and the antiepileptic drugs Brivaracetam and Levetiracetam, to the presynaptic protein SV2A in rat, pig, and human cortex. Furthermore, to detect and compare the level of SV2A and synaptophysin (SYN) protein, in five different regions of the rat brain.Method: Binding characteristics (Kd, Bmax, and Ki) of UCB-J, Brivaracetam and Levetiracetam was determined by in vitro autoradiography saturation and displacement experiments in brain sections from rat, pig, and human. The distribution of SV2A and SYN was detected by western blotting in rat hippocampus, striatum, brain stem, cerebellum, and frontal cortex. Result: The Kd value of UCB-J was found to be 6.8 ± 1.5 nM in rat, 3.2 ± 0.3 nM in pig, and 2.3 nM in human cerebral cortex, respectively, and Bmax values were found to be 1881 ± 207 fmol/mg tissue equivalent (TE) in rat cortex, 1003 ± 35 fmol/mg TE in pig cortex, and 998 fmol/mg TE in human cortex, respectively. The Ki value of the antiepileptic drug Levetiracetam was detected by displacement experiments in the three species and was found to be 14.3 ± 9.0 μM in rat, 11.1 ± 0.7 μM in pig, and 3.3 μM in human cerebral cortex, respectively. The Ki of UCB-J and Brivaracetam was determined to be 32.9 ± 5.4 nM, and 5.0 ± 3.0 μM in rat cortex, respectively. Western blot analysis showed a substantially higher level of SV2A and SYN in the cerebellum. Conclusion: Autoradiography results indicate that the binding characteristics of UCB-J and Levetiracetam are similar in all three species, and that the affinity of UCB-J is more than 100-fold more potent than that of Brivaracetam and Levetiracetam. Furthermore, the Kd values were in good consistency with results from previous studies, where the Kd value of UCB-J has been detected by PET and liquid scintillation, respectively. A substantially higher level of SV2A and SYN was detected in the cerebellum compared to the four other brain regions, but further studies need to be carried out in order to confirm this finding. <br/
