The intra-oral andextraosseous distractor was fixed to both basal and osteotomizedbone. After 5 to 10 days of latency the distractor was activatedabout 1mm per day. At the wanted alveolar high the distractionwas stopped. At the end of the phase of consolidation, the distrac-tors were removed and intra-oral implants were inserted. Three3 months later healing screws were inserted and patients werereferred to the dental practitioner for the prosthetic rehabilitation.Results: The average of the distraction was 14mm. 3 patients hadfull oral rehabilitation. The others rehabilitations are in process.Conclusions: The vertical alveolar distraction in mandibularreconstructed using fibula free-flap must be proposed if feasible.P.378 Hydroxyapatite ceramics for restoration ofalveolar boneG. Berzina-Cimdina3. 1Riga StradinsUniversity Institute of Stomatology, 2Institute of Anatomy andAnthropology, 3Riga Technical University, Riga, LatviaObjectives: To evaluate early reactogenicity of porous syntheticHA ceramic implants in experiment and late outcomes in size,shape and density of previous insufficient alveolar bone afterextractions and/or dental implant placement looking for biocom-patible materials more stable to resorbtion as autologous bonegrafts.Methods: HA synthesis by solution–precipitate method, followedby calcination at 800ºC 1 h, milling to grain sizes 20 micrometerpowder, sintering of pressed samples at temperature 1100ºC 0.5 h,grinding to granules 0.2−0.7mm. Porosity of material was35−40%, Ca/P ratio 1.66. After approval by Latvian centralethical committee for experimental animal research HA ceramicshalf-cylinders diameter 2mm were inserted in edentulous part ofright upper jaw of 6 New Zealand male rabbits. After two weeksblocks of soft tissue and bone with implant and from left controlside were taken out. Sections done with EXACT Grunding systemand from paraffin block were stained with hematoxylin/eosin,using biotin and streptavidine immunohistochemical methods forgrowth factors. HA granules were used on 112 patients to fillsockets and bone loss in one stage with SEMADOS implantinsertion and in 82 cases as first stage operation to improve sizeand shape of alveolar bone valid for implant insertion. Total254 Semados (BEGO, Bremen) implants were inserted. Singlecrown metal-ceramic restoration in 49 cases, as fixed bridge in 54patients. Evaluation of out-comes included clinical examination,X-ray with radiodensitometry. Biopsies of alveolar soft tissueat second stage surgery were taken in 24 cases. Slices werestained with hematoxylin–eosin and for immunnohistochemistryof collagen types.Results: In experiment samples distinction between implant andcontrol side was in expression of transforming growth factor(TGFbeta) which was in moderate up to marked number of bonecells while in control side was no expression. In clinical partno complications were in wound healing. Osseointegration failedof 4 implants in upper jaw. Implants were taken out and holeswere filled with HA granules, after 6 months bone/HA compositewas sufficient for implant stability and successful loading afteranother 6 months. Patients were satisfied with functional resultsin all cases, but not with esthetic results in 8 cases caused byalveolar bone deficiency in anterior maxilla. Radiodensitometryinvestigation obtained increase in optical density of alveolarstructure significantly in group more as three years after loading.Histological and immunohistochemical data of gingival samples6 months after HA implantation in part of inflammatory response,vascularity and types of collagen were close to normal.Conclusions: Implantation of HA ceramics into rabbit upper jawresulted in increase of TGFbeta expression which may explainosseoinductive action of HA. Granules of synthetic porous HAceramic have a good biocompatibility even in infected woundsafter teeth extractions and provide long term stability of newlyformed bone/HA composite in size and shape.P.379 Immediate load of nanostructured Ti implantsD. Nanostructuring of titanium bySPD processing has made the material with significantly su-perior mechanical performance when compared to conventionalCP Grade 4 titanium. Cytocompatibility studies with fibroblastmice cells L929 indicated that the nanostructured Ti surface hasa significantly higher cell colonization, suggesting more rapidosseointegration. The design of a reduced 2.4 mm diameterimplant has the strength equivalent to the conventional of 3.5mm diameter implant.Methods: NANO-Ti implants have been successfully designed,and fabricated. Clinical cases of 250 patients, all receivingimmediate load implants, were analyzed during a 3 year followup period. All NANO-Ti implants were implanted as immediateload implants, the usual success criteria were evaluated.Results: Primary retention of all implants was very good; on theday of surgery all patient received a complete provisional bridge.Healing of the operative wound passed without complications,with subsequent attachment of a definitive metalloceramic bridgecompleting treatment. Overal success rate of NANO-Ti implantsis depending on the implant site. Predominantly the mandiblewas the indication for NANO-Ti implants.Conclusion: NANO-Ti implants are a good choice in cases oftransversal alveolar bone deficiencies.P.380 Immediate maxilla prosthesis for maxillectomypatientsT. Department of Oral & Maxillofacial Surgery,School of Medicine Keio University, Shinjyuku-ku, JapanObjectives: Our treatment strategy for surgical therapy of ma-lignant maxilla tumor are: 1. We select the minimum invasivesurgical method, and reduce of the total surgery time. 2 Weconduct early oral taking and recovery the mastication and theswallowing function. 3. We do effort to reduce the post operativecomplications and the total hospital time, and conduct early socialrehabilitation.We perform immediate reconstruction using maxilla prosthesisfor in some cases of after maxillectomy patients. We presentour methods and evaluate it comparing of such as the free flapreconstruction.Methods: We performed the immediate reconstruction usingprosthesis to 5 maxillectomy patients (3 male and 2 female,average age was 64-year-old). Patients disease: 3 patients hadSquamous cell carcinoma, one patient had Adenosquamous carci-noma, one patients had Verrucous carcinoma. Operative methods:4 patients had sub total maxillectomy, one patients had partialmaxillectomy.Results: There are no post operative bleeding and infection.Tracheotomy were performed 2 patients, but both of them wereremoved tracheotomy kanule 4-day post operatively. Oral takinghad been started 5-day post operatively. 2 patients had limitationof mouth opening. One patient had cheek deformity. No patientshad speech disability. All patients could have early social reha-bilitation.Conclusions: The treatment strategy of reconstruction of aftermaxillectomy, it should be concerned the radical cure of tumor,
