In dieser retrospektiven Studie wurden 108 Röntgenbilder bestehend aus je einem Fernröntgenseitenbild (FRS), einem Orthopantomogramm (OPG) und einem digitalen Volumentomogramm (DVT) von jedem der 36 gesunden Patienten im Alter zwischen 18 und 51 untersucht. Hierbei wurden signifikante Korrelationen zwischen konventionellen kephalometrischen Parametern, gemessen in zweidimensionalen Röntgenaufnahmen (FRS und OPG), und den Knochendimensionen von drei verschiedenen intra-oralen Spenderregionen (Kinnregion, Retromolarregion und Crista zygomatico-alveolaris), gemessen in DVT-Aufnahmen, nachgewiesen. Vertikale Wachstumsmuster korrelieren positiv mit dem vertikalen Knochenangebot in der Kinnregion und negativ mit dem sagittalen und horizontalen Knochenangebot in der Kinnregion. Eine distale Bisslage nach ANB korreliert positiv mit dem retromolaren Knochenvolumen. Sowohl eine distale Bisslage als auch ein verkleinerter Kieferwinkel korrelieren mit einem vergrößerten Abstand zwischen retromolarer Knochenoberfläche und Nervus alveolaris inferior, was möglicherweise ein Hinweis dafür sein könnte, dass Patienten mit einem solchen Wachstumsmuster zu bevorzugende Kandidaten für eine retromolare Knochenentnahme sein könnten. Hierbei gilt es jedoch zu beachten, dass die Wahrscheinlichkeit für einen Verlauf des Nervus alveolaris inferior in der lateralen Hälfte der Mandibula bei einem solchen Wachstumsmuster erhöht ist. Es wurden keine Korrelationen zwischen zweidimensionalen kephalometrischen Parametern und den Knochendimensionen der crista zygomatico-alveolaris nachgewiesen. Das Knochenvolumen der crista zygomatico-alveolaris korreliert positiv mit dem retromolaren Knochenvolumen. Bei der Untersuchung geschlechtsspezifischer Unterschiede der Knochenvolumina konnten keine statistisch signifikanten Korrelationen nachgewiesen werden. Der interforaminale Abstand hingegen ist bei der untersuchten männlichen Population signifikant größer. Die gemessenen Korrelationen zwischen zweidimensionalen Parametern und Knochendimensionen könnten als Anhaltspunkte bei individuellen Erstberatungen und Untersuchungen von Patienten fungieren. Dem Behandler könnten sie außerdem als Hinweise für eine gezielte und volumenreduzierte Anwendung dreidimensionaler Bildgebung dienen. Dies kann zu einer reduzierten Strahlenbelastung und zur Befundung kleinerer Bildvolumina beitragen.In this retrospective study 108 radiographic images consisting of one lateral cephalometric radiograph (ceph), one panoramic radiograph (OPG) and one cone-beam-computed-tomography (CBCT) image per one of the 36 healthy patients between the ages of 18 and 51 have been analyzed. Significant correlations between conventional cephalometric values measured in the two-dimensional x-ray images (ceph and OPG) and bone dimensions of three different intra-oral harvest sites (chin, retromolar region, zygomatic alveolar crest) measured in three-dimensional CBCT images have been found. Patterns of vertical growth show positive correlations with vertical bone dimensions in the chin harvest site and negative correlations with sagittal and horizontal bone dimensions in the chin harvest site. Cephalometric parameters describing distal occlusion patterns via ANB correlate positively with the bone volume of the retromolar region. Distal occlusion patterns and decreased mandibular angles positively correlate with increased distances between the occlusal surface of the retromolar bone region and the inferior alveolar nerve, possibly suggesting that such patients are rather favorable candidates for retromolar bone harvesting. Though care should be taken due to the increased chance of the inferior alveolar nerve being positioned in the lateral half of the mandible in patients exhibiting such growth patterns. No correlations between two-dimensional cephalometric parameters and the bone dimensions of the zygomatic alveolar crest have been found. The bone volume of the zygomatic alveolar crest positively correlates with the bone volume of the retromolar region. While there are no correlations between the bone volumes of the three harvest sites and the gender of the patients, the interforaminal distance is significantly greater in the male group. The measured correlations between two-dimensional Parameters and bone dimensions could function as reference points during individual initial consultations and examinations. Medical practitioners could use this knowledge for more precise radiographic examinations with a reduced field of view and reduced radiation exposure in CBCT imaging.2021-03-3
