Nazal polipozis üst solunum yolunun kronik, enflamatuar bir hastalığı olup, meatuslardan
nazal kavite içine sarkan benign, ödematöz kitleler olarak tanımlanabilir. Histolojik olarak
solunum epiteli ile sarılı olan ödematöz bir stromada ağırlıklı olarak fibroblastlar ve
enflamatuar hücreler içinde de en büyük oranda eozinofiller bulunmaktadır. Polip histolojisi
tipik olarak kronik, eozinofilik bir enflamasyon gösterir.
Nazal polipozis etiyopatogenezini açıklayan pek çok sayıda kuram ortaya atılmasına rağmen
mekanizması ve bunun çözümü halâ büyük oranda bilinmemektedir. Nazal polipozis tedavisi ve
bu yaygın görülen hastalıkta yinelemelerin önlenmesi pek çok klinisyen ve araştırmacının
ilgisini çeken bir konudur.
Son zamanlarda rinosinüzit patogenezinde gastroözefageal veya özefagonazofarengeal
reflünün rolü olabileceğine dikkat çekilmiştir. Gastrik içeriğin nazofarenkse doğrudan
reflüsünün mukozal ödem ve enflamasyona neden olarak bunun da sinüs ostiumlarında
sekonder obstrüksiyona yol açabileceği ortaya atılmıştır. Gastroözefageal reflu hastalarının
önemli bir kısmının Helikobakter pilori ile enfekte olduğu saptanmıştır.
Helikobakter pilori normalde gastrik mukus tabakasında yerleşir ve kronik gastrit, gastrik ve
duodenal ülserasyon ve gastrik karsinoma ile ilişkilidir.
Helikobakter pilori’nin dental plaklarda, tükrükte, tonsil ve adenoid dokularında kolonize
olduğu gösterilmiştir. Oral kavitenin burun ve paranazal sinüslerle olan komşuluğundan dolayı
H.pilori enfeksiyonunun oral kavite içinden buraya doğrudan geçişini düşündürmektedir. Fakat
bununla beraber, H.pilori’nin sinüzit ve NP patogenezindeki rolü ile ilgili yapılan çalışmalar
nadirdir. Helikobakter pilori enfeksiyonunun nasıl geçiş gösterdiği ve gastrointestinal sistem dışı
hastalıklardaki rolü hakkında pek az bilinen vardır
Biz çalışmamızda PZR ve üre nefes testi kullanarak nazal polipozisde H.pilori bulunma
sıklığını saptamaya çalıştık
Çalışmamız küçük bir hasta grubunda yapılmasına rağmen nazal polip dokusunda H.pilori
varlığı gösterilmiştir. Bu sonuç ise H.pilori’nin nazal polipozisteki muhtemel rolünü göstermesi
bakımından ilgi çekicidir.Nasal polyposis can be defined as a chronic inflammatory disease of the upper airway,
leading to the protrusion of benign edematous polyps from the meatus into the nasal cavities.
Histologically, nasal polyps are covered with respiratory epithelium and filled with an
edematous stroma, a rather striking density of fibroblasts and inflammatory cells of which the
most prevalent cell type is the eosinophyl. Polyp histology typically shows chronic,
eosinophilic inflammation.
Altough there are a series of theories about ethiopathogenesis of nasal polyposis, the
mechanisms and resolution are largely unknown.The treatment of nasal polyposis, and the
prevention of relapses of this common disorder, is a subject of much debate among clinicians
and researchers.
Recently, attention has been directed toward the role of gastroesophageal or
esophagonasopharyngeal reflux in the pathogenesis of rhinosinusitis. It has been suggested
that direct reflux of gastric juice into the nasopharynx may cause mucosal edema and
inflammation, leading to secondary obstruction of sinus ostia, but the exact mechanism remains
unclear. A significant percentage of patients with gastroesophageal reflux disease are actually
infected with H.pylori.
Helicobacter pylori normally inhabits the gastric mucus layer. It is associated with chronic
gastritis, as well as gastric and duodenal ulceration, and has recently been linked to gastric
carcinoma.
Colonization by H.pylori has been found in dental plaque, saliva, tonsils, and adenoids.
Because the oral cavity is located near the nose and sinuses, direct transmission of H.pylori
infection from the oral cavity to these structures might occur. However, the role of H.pylori in
the pathogenesis of sinusitis and nasal polyposis has rarely been discussed. How H.pylori
infection is transmitted remains unclear, and little is known about the role of H.pylori in
extragastrointestinal diseases.
In our study, we used the PCR, urea breaht tests, to investigate the H.pylori status of nasal
polyps, obtained from patients with nasal polyposis.
Although our study involved only a small group of patients, the results suggest the presence
of H.pylori in nasal polyps, nasal mucosa. These results may have interesting implications for a
possible role of H.pylori in nasal polyposis
