In der Endoprothetik und Implantologie ist die dauerhafte und sichere Implantatveranke-rung ein entscheidenderer Faktor für eine erfolgreiche Behandlung. Das verwendete Material muss einer dauerhaften Belastung standhalten, ohne dass es zur Verformung oder zum Mate-rialbruch kommt. Titan und seine Legierungen sind aufgrund ihrer guten Biokompatibilität, biomechanischen Eigenschaften sowie exzellenten Korrosionsbeständigkeit weit verbreitet. Ein zu großer Unterschied des Elastizitätsmoduls zwischen Knochen und Implantatmaterial wie z. B. bei soliden Titanimplantaten kann jedoch zur frühzeitigen Lockerung führen. 
Poröse Titanimplantate bieten sowohl eine adäquate Materialstabilität als auch aufgrund der Mikrostruktur ein dem Knochen angepasstes Elastizitätsmodul. Zudem fördert die raue Oberfläche der offenporigen Struktur das Einwachsverhalten.
In der vorliegenden Arbeit wurde histomorphologisch und histomorphometrisch das knö-cherne Einwachsverhalten eines porösen zylinderförmigen Titanimplantats (Ti-6Al-4V), das in einem neuartigen Sinterungsprozess hergestellt wurde, in einem Tierversuch untersucht. Als Kontrollgruppe wurden herkömmliche solide Titantestzylinder (Ti-6Al-4V) mit aufge-rauter Oberfläche eingesetzt.
Folgende Hypothesen wurden aufgestellt: 
•	Die neuen offenporigen Titanimplantate werden zumindest eine gleich gute knöcher-ne Integration erfahren wie gleichdimensionierte Titanzylinder mit grobgestrahlter Oberfläche einer Kontrollgruppe.
•	Die neuen offenporigen Titanimplantate werden in den Kraftfluss integriert und das Knochengewebe wird in die Poren der Implantate einwachsen.
Bei 15 Kaninchen wurde pro Seite je ein poröses oder ein sandgestrahltes Implantat in die lateralen Femurkondyle eingesetzt. Die Tiere wurden entweder nach 4 oder 12 Wochen eu-thanasiert. Anschließend wurden Schliffpäparate der Knochen mit einliegendem Implantat hergestellt und eine histologische Färbung mit Methylenblau und Alizarinrot durchgeführt. Nach der Digitalisierung der Schnitte mittels eines vollautomatisierten Mikroskops erfolgten die deskriptive histomorphologische Untersuchung sowie die histomorphometrische Mes-sung.
In der histomorphologischen Untersuchung zeigte sich sowohl für das poröse Implantat als auch für die Kontrollgruppe eine erfolgreiche Osseointegration. Nach 4 Wochen waren die Implantate periimplantär vollständig vom Osteoid oder mineralisierten Geflechtknochen sowie weniger vom Lamellenknochen umgeben. Nach 12 Wochen war ein deutliches Remo-deling nachweisbar. Es war mehr mineralisierter Lamellenkonchen, aber weniger Geflecht-knochen und kaum Osteoid mehr vorhanden. Zwischen den Knochenbälkchen lag zuneh-mend reifes Fettgewebe vor. Am direkten Kontakt zum Implantat entstand kein interponie-rendes Bindegewebe, das die Stabilität des Implantats hätte verringern können. Die periim-plantäre und intraimplantäre Knochenneubildung waren vergleichbar, was für die funktionel-le Beanspruchung des Knochens in den Poren spricht. Histomorphometrisch unterschied sich die neugebildete Knochenmasse periimplantär nicht signifikant zwischen den beiden Implantattypen. Entsprechend den geometrischen Gegebenheiten der porösen Implantate mit einer größeren Implantatoberfläche aufgrund der offenporigen Struktur war die direkte Implantat-Kochen-Kontaktfläche signifikant größer als bei der sandgestrahlten Kontroll-gruppe.
Die histomorphologischen sowie histomorphometrischen Ergebnisse bestätigen die Hypothese, dass das neuartige offenporige Titanimplantat eine mindestens gleich gute Osseointegration im Vergleich zu dem sandgestrahlten Titanimplantat aufweisen würde. Der größere Knochen-Implantat-Kontakt sowie der Nachweis von neugebildetem, funktionellem Knochen in den Poren sprechen für eine überlegene Verankerungskraft der porösen Titanimplantate. Der Nachweis hierfür muss jedoch in einem biomechanischen Ausstoßversuch erbracht werden.In endoprosthetics and implantology, permanent and secure implant anchoring is a decisive factor for successful treatment. Titanium and its alloys are widely used due to their good biocompatibility, biomechanical properties and excellent corrosion resistance. A distinct difference in the modulus of elasticity between bone and implant material, such as solid titanium implants, can lead to early loosening.
Porous titanium implants offer both adequate material stability and, due to the microstruc-ture, a modulus of elasticity that is adapted to the bone. In addition, the rough surface of the open-pored structure promotes the ingrowth behavior.
The following hypotheses were made:
• The novel open-pore titanium implants will show at least a similar osseous integration as solid titanium cylinders of the same size in a control group.
• The new open-pore titanium implants are functionally integrated into the bone and the bone tissue grows into the pores of the implants.
In 15 rabbits, one porous or one sand-blasted implant was inserted into the lateral femoral condyle on each side. The animals were euthanized after either 4 or 12 weeks. Subsequently, thin section preparations of the bones with inserted implants were produced and histological staining with methylene blue and alizarin red was carried out. After the sections were digit-ized using a fully automated microscope, the descriptive histomorphological examination and the histomorphometric measurement were carried out.
The histomorphological examination showed a successful osseointegration both for the po-rous implant and for the control group. After 4 weeks, the implants were completely sur-rounded by osteoid or mineralized woven bone and less by lamellar bone. Remodeling was evident after 12 weeks. There was more mineralized lamellar bone, but less woven bone and hardly any osteoid. Increasingly mature fatty tissue was present between the trabeculae. There was no interposing soft tissue in direct contact with the implant, which could have reduced the stability of the implant. The bone around the implant and the intraporous bone formation were similar, which speaks for a functional stress on the bone in the pores. Histo-morphometrically, the newly formed bone mass around the implant did not differ significant-ly between the two types of implants. In accordance with the geometrical conditions of the porous implants with a larger implant surface due to the open-pored structure, the direct implant-bone contact area was significantly larger than in the sand-blasted control group.
The histomorphological and histomorphometric results confirm the hypothesis that the new open-pore titanium implant would have at least a similar osseointegration as the sand-blasted titanium implant. Furthermore, the higher bone-to-implant contact and the evidence of newly formed, functional bone in the pores speak even for a superior anchoring force of the porous titanium implants.
