    Increasing Effectiveness of the Surgical Airway Response System: Introduction of the Otolaryngology Airway Pager, Quality Improvement Project of the PGY-2 Class

    Introduction: The need for an Otolaryngology airway pager is based on several external and internal factors. The current communication pathway for emergent airways at our institution is well established but often misinterpreted. The protocol is outlined in Figure 1. The priority to reach out to other departments is based on their availability of an attending in house overnight. Furthermore, there is often a delay in contacting the Otolaryngology department. Our service has multiple pagers to accommodate for each of our inpatient teams, which can be confusing for other services. One of the driving events for this project was an incident at JHN where a tracheostomy tube became dislodged. There were several attempts to contact our team through the wrong pager and by the time we were notified the patient had expired. In addition our personal cell phones are commonly used as the primary means of contact for urgent situations. On several occasions the wrong person has been called in the middle of the night or the on-call resident is contacted while they are in the operating room during the day, resulting in a slower response time. From the perspective of the Otolaryngology Department at times we receive multiple pages and answer in the order that the pages were received, not in order of acuity as this is unknown.     Martin Ravallion?s ?One Pager? No. 66 focuses on two key issues: the level of the World Bank?s international poverty line (IPL) and its conversion to other currencies and years. Having written on conversion before (?One Pager? No. 54), I can be brief. The purchasing power parities the Bank uses to convert its IPL into other currencies at best preserve purchasing power equivalence relative to the pattern of international household consumption. Similarly, the consumer price indices the Bank uses to convert the results to other years at best preserve purchasing power equivalence relative to each national household consumption basket. Such conversions are unsuitable within a poverty measurement exercise because the prices of necessaries play a much greater role in the lives of the poor than in general consumption expenditure.     1-PAGER: One Pass Answer Generation and Evidence Retrieval

    We present 1-Pager the first system that answers a question and retrieves evidence using a single Transformer-based model and decoding process. 1-Pager incrementally partitions the retrieval corpus using constrained decoding to select a document and answer string, and we show that this is competitive with comparable retrieve-and-read alternatives according to both retrieval and answer accuracy metrics. 1-Pager also outperforms the equivalent closed-book question answering model, by grounding predictions in an evidence corpus. While 1-Pager is not yet on-par with more expensive systems that read many more documents before generating an answer, we argue that it provides an important step toward attributed generation by folding retrieval into the sequence-to-sequence paradigm that is currently dominant in NLP. We also show that the search paths used to partition the corpus are easy to read and understand, paving a way forward for interpretable neural retrieval.Comment: Accepted at EMNLP 2023 (Findings
