    The Maltese version of the DN4 questionnaire : initial validation to assess neuropathic pain in patients with chronic spinal or spinal-radicular pain

    Background: Neuropathic pain is frequently encountered in patients with spinal and spinal-related pain which needs specific treatment. Therefore, the objective of this study was to do an initial linguistic translation and validation of the Maltese DN4 questionnaire to diagnose neuropathic pain in this population. Methods: The study was designed as a single-blinded, observational, prospective collected data and retrospective analysis. The English and French DN4 questionnaires underwent forward and backward translation, literal assessment and adaptation of the semantic equivalence into the Maltese language, followed by assessment of the Maltese DN4 during the initial patient assessment in patients who met the inclusion criteria. Results: The total Maltese DN4 score obtained a Cronbach’s alpha of 0.735 therefore having satisfactory internal consistency. Test-retest using the Intraclass Correlation Coefficient (95% CI) ranged from 0.975 to 0.991 (p=0.000), while inter-rater agreement using Intraclass Correlation Coefficient (95% CI) ranged from 0.986 to 0.995 (p=0.000). Test-retest reliability yielded an intraclass correlation coefficient (95% CI) ranging from 0.975 to 0.991 (p < 0.001), while inter-rater reliability yielded an intraclass correlation coefficient (95% CI) ranging from 0.986 to 0.995 (p < 0.001). Both the English and the Maltese DN4 questionnaires obtained the same sensitivity and specificity values of 0.422 and 0.941 respectively, and a positive likehood ratio of 7.153 and a negative likehood ratio of 0.614, at a cutoff score of 4. Conclusion: The results of this study support the transcultural internal consistency, inter-rater, test-retest reliability, validity of the Maltese DN4 questionnaire to differentiate between neuropathic and nociceptive pain in patients with chronic spinal and spinal-radicular pain. Therefore, this simple tool can be used both in daily clinical practice but also in the clinical research setting to quickly screen for neuropathic pain.peer-reviewe

    Test for Intraclass Correlation Coefficient under Unequal Family Sizes

    Three tests are proposed based on F-distribution, Likelihood Ratio Test (LRT) and large sample Z-test for intraclass correlation coefficient under unequal family sizes based on a single multinormal sample. It has been found that the test based on F-distribution consistently and reliably produces results superior to those of Likelihood Ratio Test (LRT) and large sample Z-test in terms of size for various combinations of intraclass correlation coefficient values. The power of this test based on F-distribution is competitive with the power of the LRT and the power of Z-test is slightly better than the powers of F-test and LRT when k = 15, but the power of Z-test is worse in comparison with the F-test and LRT for k = 30, i.e. for large sample situation, where k = sample size. This test based on F-distribution can be used for both small sample and large sample situations. An example with real data is presented

    Modified Profile Likelihood Approach for Certain Intraclass Correlation Coefficient

    In this paper we consider the problem of constructing confidence intervals and lower bounds forthe intraclass correlation coefficient in an interrater reliability study where the raters are randomly selected from a population of raters.The likelihood function of the interrater reliability is derived and simplified, and the profile likelihood based approach is readily available for computing the confidence intervals of the interrater reliability. Unfortunately, the confidence intervals computed by using the profile likelihood function are in general too narrow to have the desired coverage probabilities. From the point view of practice, a conservative approach, if is at least as precise as any existing method, is preferred sinceit gives the correct results with a probability higher than claimed. Under this rationale, we propose the so-called modified likelihood approach in this paper. Simulation study shows that, the proposed method in general has better performance than currently used methods

    Reliability of environmental sampling culture results using the negative binomial intraclass correlation coefficient.

    The Intraclass Correlation Coefficient (ICC) is commonly used to estimate the similarity between quantitative measures obtained from different sources. Overdispersed data is traditionally transformed so that linear mixed model (LMM) based ICC can be estimated. A common transformation used is the natural logarithm. The reliability of environmental sampling of fecal slurry on freestall pens has been estimated for Mycobacterium avium subsp. paratuberculosis using the natural logarithm transformed culture results. Recently, the negative binomial ICC was defined based on a generalized linear mixed model for negative binomial distributed data. The current study reports on the negative binomial ICC estimate which includes fixed effects using culture results of environmental samples. Simulations using a wide variety of inputs and negative binomial distribution parameters (r; p) showed better performance of the new negative binomial ICC compared to the ICC based on LMM even when negative binomial data was logarithm, and square root transformed. A second comparison that targeted a wider range of ICC values showed that the mean of estimated ICC closely approximated the true ICC

    Estimation and Inference of the Three-Level Intraclass Correlation Coefficient

    Since the early 1900\u27s, the intraclass correlation coefficient (ICC) has been used to quantify the level of agreement among different assessments on the same object. By comparing the level of variability that exists within subjects to the overall error, a measure of the agreement among the different assessments can be calculated. Historically, this has been performed using subject as the only random effect. However, there are many cases where other nested effects, such as site, should be controlled for when calculating the ICC to determine the chance corrected agreement adjusted for other nested factors. We will present a unified framework to estimate both the two-level and three-level ICC for both binomial and multinomial outcomes. In addition, the corresponding standard errors and confidence intervals for both ICC measurements will be displayed. Finally, an example of the effect that controlling for site can have on ICC measures will be presented for subjects nested within genotyping plates comparing genetically determined race to patient reported race. In addition, when determining agreement on a multinomial response, the question of homogeneity of agreement of individual categories within the multinomial response is raised. One such scenario is the GO project at the University of Pennsylvania where subjects ages 8-21 were asked to rate a series of actors\u27 faces as happy, sad, angry, fearful or neutral. Methods exist to quantify overall agreement among the five responses, but only if the ICCs for each item-wise response are homogeneous. We will present a method to determine homogeneity of ICCs of the item-wise responses across a multinomial outcome and provide simulation results that demonstrate strong control of the type I error rate. This method will subsequently be extended to verify the assumptions of homogeneity of ICCs in the multinomial nested-level model to determine if the overall nested-level ICC is sufficient to describe the nested-level agreement

    Collagen bundle morphometry in skin & scar tissue: a novel distance mapping method provides superior measurements compared to Fourier analysis

    Histopathological evaluations of fibrotic processes require the characterization of collagen morphology in terms of geometrical features such as bundle orientation thickness and spacing. However, there are currently no reliable and valid techniques of measuring bundle thickness and spacing. Hence, two objective methods quantifying the collagen bundle thickness and spacing were tested for their reliability and validity: Fourier first-order maximum analysis and Distance Mapping, with the latter constituting a newly developed morphometric technique. Histological slides were constructed and imaged from 50 scar and 50 healthy human skin biopsies and subsequently analyzed by two observers to determine the interobserver reliability via the intraclass correlation coefficient. An intraclass correlation coefficient larger than 0.7 is considered as representing good reliability. The interobserver reliability for the Fourier first-order maximum and for the Distance Mapping algorithms, respectively, showed an intraclass correlation coefficient above 0.72 and 0.89. Additionally, we performed an assessment of validity in the form of responsiveness, in particular, demonstrating medium to excellent results via a calculation of the effect size, highlighting that both methods are sensitive enough to measure a treatment effect in clinical practice. In summary, two reliable and valid measurement methods were demonstrated for collagen bundle morphometry for the first time. Due to its superior reliability and more useful measures (bundle thickness and bundle spacing), Distance Mapping emerges as the preferred and more practical method. Nevertheless, in the future, both methods can be used for reliable and valid collagen morphometry of skin and scars, whereas further applications evaluating the quantitative microscopy of other fibrotic processes are anticipated

    Intra-session and inter-day reliability of the Myon 320 electromyography system during sub-maximal contractions

    Electromyography systems are widely used within the field of scientific and clinical practices. The reliability of these systems are paramount when conducting research. The reliability of Myon 320 Surface Electromyography System is yet to be determined. This study aims to determine the intra-session and inter-day reliability of the Myon 320 Surface Electromyography System. Muscle activity from fifteen participants was measured at the anterior deltoid muscle during a bilateral front raise exercise, the vastus lateralis muscle during a squat exercise and the extensor carpi radialis brevis (ECRB) muscle during an isometric handgrip task. Intra-session and inter-day reliability was calculated by intraclass correlation coefficient, standard error of measurement and coefficient of variation (CV). The normalized root mean squared (RMS) surface electromyographic signals produced good intra-session and inter-day testing intraclass correlation coefficient values (range: 0.63-0.97) together with low standard error of measurement (range: 1.49-2.32) and CV (range: 95% Confidence Interval = 0.36-12.71) measures for the dynamic-and-isometric contractions. The findings indicate that the Myon 320 Surface Electromyography System produces good to fair reliability when examining intra-session and inter-day reliability. Findings of the study provide evidence of the reliability of electromyography between trials which is essential during clinical testing.</p

    Confidence Interval Estimation for Intraclass Correlation Coefficient Under Unequal Family Sizes

    Confidence intervals (based on the χ2 -distribution and (Z) standard normal distribution) for the intraclass correlation coefficient under unequal family sizes based on a single multinormal sample have been proposed. It has been found that the confidence interval based on the χ2 -distribution consistently and reliably produces better results in terms of shorter average interval length than the confidence interval based on the standard normal distribution: especially for larger sample sizes for various intraclass correlation coefficient values. The coverage probability of the interval based on the χ2 -distribution is competitive with the coverage probability of the interval based on the standard normal distribution. An example with real data is presented

    Twenty-four-hour repeatability of diurnal intraocular pressure patterns in glaucomatous and ocular hypertensive individuals

    OBJECTIVE: To verify the 24-hour repeatability of diurnal intraocular pressure patterns in glaucomatous and ocular hypertensive individuals. METHODS: A prospective analysis of 88 eyes from 88 ocular hypertensive or open-angle glaucoma patients was conducted on diurnal tension curves obtained by the same examiner on two consecutive days. The intraclass correlation coefficient test was used for statistical analysis. RESULTS: Eighty-eight eyes from 88 patients were analyzed. Fifty-seven patients (64.8%) were female. The mean age of all participants was 68.7 (SD 10.8, range 51-79) years. The intraclass correlation coefficient values for measurements at 8 AM, 11 AM, 2 PM, and 4 PM were 0.80, 0.82, 0.83, and 0.86, respectively (all intraclass correlation coefficient values, p<0.001). CONCLUSION: Diurnal intraocular pressure data collected on a single day characterize the diurnal intraocular pressure variability over 24 hours in primary open-angle glaucoma and ocular hypertensive patients
