    Objective Because most children with asthma now use inhaled corticosteroids (ICS), the added benefit of immunotherapy in asthmatic children needs to be examined. We re-assessed the effectiveness of subcutaneous (SCIT) and sublingual immunotherapy (SLIT) in childhood asthma treatment focusing on studies with patient-relevant outcome measures and children using ICS. Methods We used the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach to systematically search and appraise the evidence using predefined critical patient-relevant outcomes (asthma symptoms, asthma control and exacerbations). We searched to retrieve systematic reviews and randomised controlled trials on immunotherapy for asthma in children (1960-2017). We assessed the quality of the body of evidence with GRADE criteria. Results The quality of the evidence for SCIT was very low due to a large risk of bias and indirectness (dated studies in children not using ICS). No effect of SCIT was found for asthma symptoms; no studies reported on asthma control. For asthma exacerbations, studies favoured SCIT. We have little confidence in this effect estimate, due to the very low quality of evidence. For SLIT, quality of the evidence was very low due to a large risk of bias, indirectness and imprecision. The outcome 'asthma symptoms' could not be calculated due to lack of standardisation and large clinical heterogeneity. Other predefined outcomes were not reported. Conclusion The beneficial effects of immunotherapy in childhood asthma found in earlier reviews are no longer considered applicable, because of indirectness (studies performed in children not being treated according to current asthma guidelines with ICS). As the effect of real time text-messages in children with asthma is unknown, the primary aim of this study is to determine the effect of a Real Time Medication Monitoring system (RTMM) with text-messages on adherence to inhaled corticosteroids (ICS). The secondary objective is to study the effects of RTMM on asthma control, quality of life and cost-effectiveness of treatment. Methods: A multicenter, randomized controlled trial involving 220 children (4--11 years) using ICS for asthma. All children receive an RTMM-device for one year, which registers time and date of ICS doses. Children in the intervention group also receive tailored text-messages, sent only when a dose is at risk of omission. Primary outcome measure is the proportion of ICS dosages taken within the individually predefined time-interval. Secondary outcome measures include asthma control (monthly Asthma Control Tests), asthma exacerbations, healthcare use (collected from hospital records, patient reports and pharmacy record data), and disease-specific quality of life (PAQLQ questionnaire). Parental and children's acceptance of RTMM is evaluated with online focus groups and patient questionnaires. An economic evaluation is performed adopting a societal perspective, including relevant healthcare costs and parental productivity loss. Furthermore, a decision-analytic model is developed in which different levels of adherence are associated with clinical and financial outcomes. Also, sensitivity analyses are carried out on different price levels for RTMM. Discussion: If RTMM with tailored text-message reminders proves to be effective, this technique can be used in daily practice, which would support children with suboptimal adherence in their asthma (self)management and in achieving better asthma control and better quality of life.     Nonadherence in children who use long-term medication is a serious problem and assessing adherence is an important step to provide solutions to this problem. Medication adherence can be measured by several methods, including (a) self-report questionnaires or structured interviews, (b) therapeutic drug monitoring (TDM), (c) electronic devices, and (d) pick-up/refill rates. The objective of this narrative review is to provide an overview of the literature about methods for the measurement of medication adherence in chronically ill children and adolescents. Therefore, we conducted a literature search by using multiple databases. Four methods of monitoring medication adherence are presented for the most described chronic diseases: asthma, HIV/AIDS, epilepsy, diabetes mellitus and ADHD. First, 10 commonly used self-report questionnaires and structured interviews are described, including the main characteristics, (dis)advantages and their validation studies. Second, the use of TDM in pediatric trials for medication adherence measurement is discussed. New sampling methods (e.g. dried blood spot) and sampling matrices (e.g. hair, saliva and urine) have shown their benefits for TDM in children. Third, electronic devices to measure medication adherence in children are presented, being developed for several drug administration routes. Fourth, the analyses, advantages and disadvantages of pharmacy data are discussed. The usage of this data requires specific calculations and interpretations to assess adherence. As presented in this review, every adherence method has specific (dis)advantages. When deciding which adherence method is applicable, validity and generalizability should be taken into account. Combining multiple methods seems to offer the best solution in the daily clinical practice

    Experiences of Adherence Assessment in Asthma

    Background and Aims: Poor adherence to inhaled corticosteroids is understood to be one of the largest contributors to problematic severe asthma in children (Bracken et al., 2009). Researchers have sought to understand and target nonadherence and assessment of adherence is seen as crucial in this process. Recent research has championed electronic monitoring tools as the “gold standard” for accurately measuring adherence and these devices have been extensively evaluated (Burgess, Sly, Devadason, 2011). Only a small amount of literature has considered how one experiences the process of adherence assessment through electronic tools. One such device, the smart-inhaler has been introduced in the paediatric asthma team at the Royal Brompton Hospital. The proposed study aims to explore young people’s experiences of having their adherence to inhaled corticosteroids assessed through a smart-inhaler. It will also explore the experiences of their caregivers and healthcare professionals. Method: Semi-structured interviews were conducted with eight young people with asthma, aged 11-15, who had been given a smart-inhaler as part of their care at the Royal Brompton Hospital, and eight of their caregivers. A focus group with seven healthcare professionals who used the smart-inhalers in their practice was also carried out. Interviews were analysed using a critical realist thematic analysis. Results: Three themes were identified: “they were trying to help me get better”, “it’s clearly just to check up” and “who is responsible?”. They highlight the variety of perspectives and experiences participants had regarding the smartinhaler. More specifically the themes highlighted the importance of participants’ priorities in influencing their experiences, the impact of the smart-inhaler on the healthcare relationship and on the transferring of responsibility for asthma to young people. Conclusions: The findings suggest that it is important for healthcare professionals to engage in a shared decision-making process with their patients when introducing healthcare interventions such as the smart-inhaler
