    Quantitative assessment of systolic left ventricular function with speckle-tracking echocardiography in adult patients with repaired aortic coarctation

    Despite successful aortic coarctation (CoA) repair, systemic hypertension often recurs which may influence left ventricular (LV) function.     Echocardiographic parameters of severe pulmonary regurgitation after surgical repair of tetralogy of Fallot

    Aims: Reliable evaluation of the severity and consequences of pulmonary regurgitation (PR) in patients with repaired tetralogy of Fallot (TOF) is crucial to timely identify the need for pulmonary valve intervention. We aimed to identify the accuracy of echocardiographic parameters to differentiate between moderate and severe PR, using phase contrast cardiac magnetic resonance imaging (CMR) as gold standard. Methods and results: In this cross-sectional study, 45 TOF patients with both echocardiographic and CMR measurements of PR were enrolled. All quantitative and semiquantitative echocardiographic measurements such as pressure half time (PHT), Color flow jet width (CFJW), ratio CFJW/right ventricle outflow tract (RVOT) diameter, PR index and the presence of early termination of the PR jet, end-diastolic antegrade flow and diastolic backflow in main pulmonary artery (MPA), and PA branches correlated significantly with PR fraction on CMR. Qualitative assessment with color flow on echocardiography overestimated PR Multivariate linear regression analysis identified the ratio of CFJW/RVOT diameter and PHT as independent predictors of PR fraction. Accuracy of echo parameters was tested to differentiate between mild-to-moderate and severe PR Combining different echocardiographic parameters increased sensitivity and specificity. The addition of diastolic flow reversal in the PA branches to PHT below 167 milliseconds increased the NPV from 87% to 89% and PPV from 62% to 76%. Conclusions: Comparison with CMR confirms that echocardiographic parameters are reliable in predicting PR severity. Combined measurement of diastolic flow reversal in the pulmonary artery branches and PHT is reliable in the detection of severe PR in the follow-up of TOF patients

    Personal Publication Assistant: Abstract recommendations by a cognitive model

    This paper discusses an analysis of how scientists select relevant publications, and an application that can assist scientists in this information selection task. The application, called the Personal Publication Assistant, is based on the assumption that successful information selection is driven by recognizing familiar terms. To adapt itself to a researcher's interests, the system takes into account what words have been used in a particular researcher's abstracts, and when these words have been used. The user model underlying the Personal Publication Assistant is based on a rational analysis of memory, and takes the form of a model of declarative memory as developed for the cognitive architecture ACT-R. We discuss an experiment testing the assumptions of this model and present a user study that validates the implementation of the Personal Publication Assistant. The user study shows that the Personal Publication Assistant can successfully make an initial selection of relevant papers from a large collection of scientific literature. (C) 2008 Elsevier B. V.     Op 24 mei werd in Leeuwarden een referendum gehouden. Het referendum ging over de herinrichting van het Zaailand, een groot plein in het centrum. Onderdeel van het project was de bouw van een nieuw Fries Museum. De bevolking stemde in meerderheid tegen het plan van de gemeente, maar de benodigde drempel van 30 procent van alle kiesgerechtigden werd niet gehaald. De gemeenteraad besloot daarom dat het plan kon worden uitgevoerd. In dit onderzoeksrapport wordt het Leeuwarder referendum beschreven, wordt geïnventariseerd wat er goed en verkeerd is gegaan, en worden aanbevelingen gedaan met het oog op toekomstige referenda. De belangrijkste conclusie is dat de vraagstelling niet goed was en dat dit de uitslag heeft beïnvloed

We aimed to detect early LV dysfunction using LV global longitudinal strain (GLS) in adults with repaired CoA, and to identify associations with patient and echocardiographic characteristics. In this cross-sectional study, patients with repaired CoA and healthy controls were recruited prospectively. All subjects underwent echocardiography, ECG and blood sampling within 1 day. With speckle-tracking echocardiography, we assessed LV GLS on the apical four-, three- and two-chamber views. We included 150 subjects: 75 patients (57 % male, age 33.4 ± 12.8 years, age at repair 2.5 [IQR: 0.1–11.1] years) and 75 healthy controls of similar sex and age. LV GLS was lower in patients than in controls (−17.1 ± 2.3 vs. −20.2 ± 1.6 %, P < 0.001). Eighty percent of the patients had a normal LV ejection fraction, but GLS was still lower than in controls (P < 0.001). In patients, GLS correlated with systolic and diastolic blood pressure (r = 0.32, P = 0.009; r = 0.31, P = 0.009), QRS duration (r = 0.34, P = 0.005), left atrial dimension (r = 0.27, P = 0.029), LV mass (r = 0.30, P = 0.014) and LV ejection fraction (r = −0.48, P < 0.001). Patients with either associated cardiac lesions, multiple cardiac interventions or aortic valve replacement had lower GLS than patients without. Although the majority of adults with repaired CoA seem to have a normal systolic LV function, LV GLS was decreased. Higher blood pressure, associated cardiac lesions, and larger left atrial dimension are related with lower GLS. Therefore, LV GLS may be used as objective criterion for early detection of ventricular dysfunction.
