Screening for Food Insecurity, Accessing Healthy Foods, and Resources for Patients
Food Insecurity and access to healthy food is a large health care issue in the United States, Vermont, and Chittenden County. This project aimed to educate health care providers at Colchester Family Practice about food insecurity to increase the amount of patients screened for food insecurity. Distantly Labeling Data for Large Scale Cross-Document Coreference
Cross-document coreference, the problem of resolving entity mentions across
multi-document collections, is crucial to automated knowledge base construction
and data mining tasks. However, the scarcity of large labeled data sets has
hindered supervised machine learning research for this task. In this paper we
develop and demonstrate an approach based on ``distantly-labeling'' a data set
from which we can train a discriminative cross-document coreference model. In
particular we build a dataset of more than a million people mentions extracted
from 3.5 years of New York Times articles, leverage Wikipedia for distant
labeling with a generative model (and measure the reliability of such
labeling); then we train and evaluate a conditional random field coreference
model that has factors on cross-document entities as well as mention-pairs.
This coreference model obtains high accuracy in resolving mentions and entities
that are not present in the training data, indicating applicability to
non-Wikipedia data. Given the large amount of data, our work is also an
exercise demonstrating the scalability of our approach.Comment: 16 pages, submitted to ECML 201
JD and Me: Exploring Hybrid Representation of Pro Se Defendants in Capital Murder Cases
The United States Constitution grants those facing the loss of life and liberty the right to due process and a fair trial under the law. What can be done to ensure criminal defendants facing the death penalty feel as though their desired argument and defense will be presented while still having the appearance of a fair trial? This Article compares a person the law says is qualified to waive counsel and represent themselves and a person qualified to be appointed to represent those facing the death penalty, what is required to waive counsel, the involvement of the trial court and the roles of standby counsel in pro se cases, the unique differences of a death penalty case, and ultimately demonstrate what a hybrid representation could look like when a defendant is pro se but wishes for standby counsel to be involved in the process.

Computational simulation of an unmanned air vehicle impacting water
Computational simulations were performed to study the splashdown of an unmanned air vehicle (UAV) falling nose-first into seawater from various heights. Solutions were generated with a time-accurate finite-volume method based on the unsteady compressible ensemble averaged Navier-Stokes equations for the air and the unsteady incompressible ensemble averaged Navier-Stokes equations for the seawater. The volume of fluid model was used to track the air-water interface and a deforming mesh algorithm was used to move the UAV through the computational domain. Computed pressure histories at four key locations on the UAV forebody were compared with experimentally measured values to validate this study. The computational simulations were shown to have accurately predicted the magnitude and character of the pressure histories, but with some discrepancies in the behavior of the pressure within the UAV inlet aperture. Results are presented for various drop heights, which simulated a range of impact velocities. Modifications were also made to the UAV geometry to examine the effect of deflecting the upper inlet lip downward. Deflection angles of 30 deg and 20 deg were analyzed for a drop height condition of 35 ft with results showing a significant decrease in impact force and pressure within the inlet
Engagement and Disengagement in Art Interventions with Memory Impairment
Introduction. Studies have shown art intervention to be an effective therapy for patients with memory impairments, leading to overall positive behaviors, increased quality of life and decreased caregiver burden. We conducted a preliminary study to evaluate and compare the effect of participation in weekly art appreciation and painting sessions on the behavior of memory impaired residents in an assisted living facility.
Methods. Residents’ behaviors were observed during art appreciation and active painting sessions over a six-week period. Each session consisted of either viewing and discussing artwork or actively painting in the style of the artist discussed. Positive and negative behaviors were recorded and tallied throughout the sessions. Over the course of 12 sessions, the 7 observers made a total of 1957 observations of a variable patient population. The total number of both positive and negative behaviors was compared between activities, over time within sessions, as well as over the six weeks. Each session was percent normalized to the time interval with the highest occurrence of select behaviors.
Results. Upward trends for positive and negative behaviors were noticed in ap- preciation and painting sessions respectively. The negative to positive engagements ratios for each painting session showed an increase in negative behaviors. Disengagement increased as the appreciation sessions progressed and decreased as painting sessions progressed. Overall, positive engagement increased in both appreciation and painting sessions.
Conclusions. Despite several confounding variables encountered in this study, we demonstrated art appreciation and active painting to be a viable non- pharmacological therapeutic approach for individuals with memory impairments.https://scholarworks.uvm.edu/comphp_gallery/1258/thumbnail.jp
Commercial Cargo Derivative Study of the Advanced Hybrid Wing Body Configuration with Over-Wing Engine Nacelles
LM has leveraged our partnership with the Air Force Research Laboratory (AFRL) and NASA on the advanced hybrid wing body (HWB) concept to develop a commercial freighter which addresses the NASA Advanced Air Transport Technology (AATT) Project goals for improved efficiency beyond 2025. The current Air Force Research Laboratory (AFRL) Revolutionary Configurations for Energy Efficiency (RCEE) program established the HWB configuration and technologies needed for military transports to achieve aerodynamic and fuel efficiencies well beyond the commercial industry's most modern designs. This study builds upon that effort to develop a baseline commercial cargo aircraft and two HWB derivative commercial cargo aircraft to quanitify the benefit of the HWB and establish a technology roadmap for further development

A behavioral intervention to promote use of multimodal pain medication for hospitalized patients: A randomized controlled trial.
BACKGROUND: The use of nonsteroidal anti-inflammatory drugs (NSAIDs) can reduce pain and has become a core strategy to decrease opioid use, but there is a lack of data to describe encouraging use when admitting patients using electronic health record systems. OBJECTIVE: Assess an electronic health record system to increase ordering of NSAIDs for hospitalized adults. DESIGNS, SETTINGS AND PARTICIPANTS: We performed a cluster randomized controlled trial of clinicians admitting adult patients to a health system over a 9-month period. Clinicians were randomized to use a standard admission order set. INTERVENTION: Clinicians in the intervention arm were required to actively order or decline NSAIDs; the control arm was shown the same order but without a required response. MAIN OUTCOME AND MEASURES: The primary outcome was NSAIDs ordered and administered by the first full hospital day. Secondary outcomes included pain scores and opioid prescribing. RESULTS: A total of 20,085 hospitalizations were included. Among these hospitalizations, patients had a mean age of 58 years, and a Charlson comorbidity score of 2.97, while 50% and 56% were female and White, respectively. Overall, 52% were admitted by a clinician randomized to the intervention arm. NSAIDs were ordered in 2267 (22%) interventions and 2093 (22%) control admissions (p = .10). Similarly, there were no statistical differences in NSAID administration, pain scores, or opioid prescribing. Average pain scores (0-5 scale) were 3.36 in the control group and 3.39 in the intervention group (p = .46). There were no differences in clinical harms. CONCLUSIONS AND RELEVANCE: Requiring an active decision to order an NSAID at admission had no demonstrable impact on NSAID ordering. Multicomponent interventions, perhaps with stronger decision support, may be necessary to encourage NSAID ordering
Well-differentiated Papillary Mesothelioma With Invasive Foci
Well-differentiated papillary mesotheliomas (WDPMs) are usually encountered as incidental findings in the peritoneal cavity in women. Most WDPMs are benign, and the histologic features that indicate a more aggressive course are controversial. We report 20 cases of WDPM, which contained invasive foci. Thirteen cases arose in the peritoneal cavity, 1 in a hernia sac, 3 in the pleural cavity, and 3 in hydroceles. The female:male ratio was 16:4, and age range was 7 to 74 years. Tumor was multifocal in 15 cases. Some tumors showed back-to-back papillae, a pattern mimicking invasion but discernible on pan-keratin stain as compressive crowding. True invasive patterns ranged from simple bland-appearing glands invading the stalks of the papillae to solid foci of invasive tumor of higher cytologic grade than the original WDPM. All 5 tested cases were negative for p16 deletion by fluorescence in situ hybridization, but 2/3 had abnormal karyotypes. Recurrences were seen in 8 patients, and in 4 multiple recurrences were documented. Of 16 patients with follow-up, 14 are alive from periods of 6 months to 6 years (average 3.5 y), and 2 have known recurrent disease. One patient died of disseminated tumor at 8 years but without histologic confirmation of the nature of the tumor. We conclude that WDPM with invasive foci in the papillae appear to be prone to multifocality and recurrence, but that they rarely give rise to life-threatening disease. We suggest that these lesions be called WDPM with invasive foci to alert clinicians to the possibility of recurrence
Optimizing Retrieval of Biospecimens Using the Curated Cancer Clinical Outcomes Database (C3OD)

Translational research and personalized medicine require well-documented and up-to-date information, but the infrastructure used to support biorepositories and biobanks can easily be out of sync with the host institution. To assist researchers and provide them with accurate pathological, epidemiological, and bio-molecular data, the Biospecimen Repository Core Facility (BRCF) at the University of Kansas Medical Center (KUMC) merges data from medical records, the tumor registry, and pathology reports using the Curated Cancer Clinical Outcomes Database (C3OD).
