Making in-class skills training more effective: the scope for interactive videos to complement the delivery of practical pedestrian training
Skills and awareness of young pedestrians can be improved with on-street practical pedestrian training, often delivered in schools in the United Kingdom by local authorities with the intention of improving road safety. This training is often supplemented by in-class paper based worksheet activities which are seen to be less effective than practical training in that they focus on knowledge acquisition rather than directly improving the correct application of safe pedestrian skills at the roadside. Previous research indicates that interactive video tools have the potential to develop procedural skills whilst offering an engaging road safety educational experience, which could positively impact on road crossing behaviour.In this paper, the design and development of a hazard-identification interactive road safety training video targeting child road crossing skills is presented. The interactive video was shown to be an engaging training resource for 6-7 year old children. The tool’s scope for improving pedestrians’ roadside skills is considered along with the wider implications for interactive video to aid safety training in other areas
Building the economic evidence case for social prescribing
Understanding the economic impact of social prescribing remains an urgent priority for the National Academy of Social Prescribing (NASP). As yet it is unclear how much data exists within the different systems to enable economic analyses of the impact of social prescribing schemes to be conducted.
The complexity in understanding the economic impact of social prescribing—and indeed all non-clinical community-based approaches to health—is compounded by the multisector nature of social prescribing. Furthermore, a variety of approaches are being used to test similar but different understandings of both cost and value, including social value, cost, benefit and economic value.
There are a growing range of reports and peer-reviewed publications that focus on the impact of social prescribing on health and social care demand, some of which have economic analyses and some which remain as potential data sets for economic analyses. At least one third of all outcomes (if not more) are directly related to the social determinants of health1,2 (SDH) which are not taken into account with economic analysis focused only on health service usage.
This range of outcomes experienced by service users2-4 is driving many researchers to conduct economic analyses that attempt to assign value to outcomes beyond the health sector, for instance using social return on investment (SROI) and proxy values. Other researchers have discussed the evolution in economic analyses at length and suggest additional components to existing methodologies, e.g., multi criteria decision analysis (MCDA) to account for additional complexity of social prescribing5. Further developments are also being trialled such as the Wellbeing-adjusted Life Years (WELLBY) to understand the economic value attached to wellbeing6, as opposed to the Quality Adjusted Life Years (QALY), which reports the economic value of quality of life.
We are entering an era of providing personalised support to people in integrated and multidisciplinary systems with different local population needs. As such, there is a need to evolve the approaches to
determining cost and value of social prescribing, and to reach agreements on methodologies that all sectors are willing to accept as sound approaches. Furthermore, as discussed by McDaid and colleagues in 2019 7, there is a need to move beyond the immediate benefits of social prescribing and to explore the longer-term benefits of sustained engagement in non-clinical activities and provision of support to address issues linked to the SDH. This would enable more data to inform the preventative role and economic impact that social prescribing may have, which is currently an evidence gap.
This rapid scoping review was commissioned by NASP and additional roundtables were supported by the National Centre for Creative Health and UKRI/AHRC’s ‘Mobilising Community Assets to Tackle Health Inequalities’ research programme (led by University College London). It aims to provide an update to the first economic evidence review from NASP and explore economic data and health and social care usage data in more detail.
This rapid scoping review aims to ascertain:
What the current literature indicates in terms of cost or value of social prescribing schemes or parts of the social prescribing scheme.
If there are potential data sets that report the impact of social prescribing on health service usage that could have economic analysis applied to them.
Stakeholder opinions on the methodological approaches for creating the current economic evaluation evidence base for social prescribing and potential future developments that are needed.
How these findings can inform a larger programme of research that is needed to establish the economic impact and value of social prescribing across all relevant sectors in the community.
As this report contains three separate elements to it, each element will be reported with methods and results, and then key themes will be brought together with recommendations
Screening a protein kinase inhibitor library against <i>Plasmodium falciparum</i>
Abstract Background Protein kinases have been shown to be key drug targets, especially in the area of oncology. It is of interest to explore the possibilities of protein kinases as a potential target class in Plasmodium spp., the causative agents of malaria. However, protein kinase biology in malaria is still being investigated. Therefore, rather than assaying against individual protein kinases, a library of 4731 compounds with protein kinase inhibitor-like scaffolds was screened against the causative parasite, Plasmodium falciparum. This approach is more holistic and considers the whole kinome, making it possible to identify compounds that inhibit more than one P. falciparum protein kinase, or indeed other malaria targets. Results As a result of this screen, 9 active compound series were identified; further validation was carried out on 4 of these series, with 3 being progressed into hits to lead chemistry. The detailed evaluation of one of these series is described. Discussion This screening approach proved to be an effective way to identify series for further optimisation against malaria. Compound optimisation was carried out in the absence of knowledge of the molecular target. Some of the series had to be halted for various reasons. Mode of action studies to find the molecular target may be useful when problems prevent further chemical optimisation. Analysis of shock capturing methods for chemical species transport in unsteady compressible flow
This paper presents a chemical species transport model to account for variable composition and gas properties along the flow path in internal combustion engines. The numerical solution to adapt the gas dynamic model to chemical species transport in boundary conditions by means of the Method of Characteristics and in volumes by means of a filling and emptying model is described. The performance for chemical species transport in 1D elementsof shock-capturing methods, such as the two-step Lax Wendroff method and the Sweby s TVD scheme considering several flux limiter definitions, is carried out by means of shocktube tests. Policy-making tool for optimization of transit priority lanes in urban network
Transit improvement is an effective way to relieve traffic congestion and decrease greenhouse gas emissions. Improvement can be in the form of new facilities or giving on-road priority to transit. Although construction of off-road mass transit is not always viable, giving priority to transit can be a low-cost alternative. A framework is introduced for optimization of bus priority at the network level. The framework identifies links on which a bus lane should be located. Allocation of a lane to transit vehicles would increase the utility of transit, although this can be a disadvantage to auto traffic. The approach balances the impact on all stakeholders. Automobile advocates would like to increase traffic road space, and the total travel time of users and total emissions of the network could be reduced by a stronger priority scheme. A bilevel optimization is applied that encompasses an objective function at the upper level and a mode choice, a traffic assignment, and a transit assignment model at the lower level. The proposed optimization helps transport authorities to quantify the outcomes of various strategies of transit priority. A detailed sensitivity analysis is carried out on the relative weight of each factor in the objective function. The proposed framework can also be applied in the context of high-occupancy-vehicle lanes and heavy-vehicle priority lanes
SN 2009kf : a UV bright type IIP supernova discovered with Pan-STARRS 1 and GALEX
We present photometric and spectroscopic observations of a luminous type IIP
Supernova 2009kf discovered by the Pan-STARRS 1 (PS1) survey and detected also
by GALEX. The SN shows a plateau in its optical and bolometric light curves,
lasting approximately 70 days in the rest frame, with absolute magnitude of M_V
= -18.4 mag. The P-Cygni profiles of hydrogen indicate expansion velocities of
9000km/s at 61 days after discovery which is extremely high for a type IIP SN.
SN 2009kf is also remarkably bright in the near-ultraviolet (NUV) and shows a
slow evolution 10-20 days after optical discovery. The NUV and optical
luminosity at these epochs can be modelled with a black-body with a hot
effective temperature (T ~16,000 K) and a large radius (R ~1x10^{15} cm). The
bright bolometric and NUV luminosity, the lightcurve peak and plateau duration,
the high velocities and temperatures suggest that 2009kf is a type IIP SN
powered by a larger than normal explosion energy. Recently discovered high-z
SNe (0.7 < z < 2.3) have been assumed to be IIn SNe, with the bright UV
luminosities due to the interaction of SN ejecta with a dense circumstellar
medium (CSM). UV bright SNe similar to SN 2009kf could also account for these
high-z events, and its absolute magnitude M_NUV = -21.5 +/- 0.5 mag suggests
such SNe could be discovered out to z ~2.5 in the PS1 survey.Comment: Accepted for publication in APJ
Sect and House in Syria: History, Architecture, and Bayt Amongst the Druze in Jaramana
This paper explores the connections between the architecture and materiality of houses and the social idiom of bayt (house, family). The ethnographic exploration is located in the Druze village of Jaramana, on the outskirts of the Syrian capital Damascus. It traces the histories, genealogies, and politics of two families, bayt Abud-Haddad and bayt Ouward, through their houses. By exploring the two families and the architecture of their houses, this paper provides a detailed ethnographic account of historical change in modern Syria, internal diversity, and stratification within the intimate social fabric of the Druze neighbourhood at a time of war, and contributes a relational approach to the anthropological understanding of houses
Revision for adverse local tissue reaction following metal-on-polyethylene total hip arthroplasty is associated with a high risk of early major complications
AimsFretting and corrosion at the modular head/neck junction, known as trunnionosis, in total hip arthroplasty (THA) is a cause of adverse reaction to metal debris (ARMD). We describe the outcome of revision of metal-on-polyethylene (MoP) THA for ARMD due to trunnionosis with emphasis on the risk of major complications.Patients and MethodsA total of 36 patients with a MoP THA who underwent revision for ARMD due to trunnionosis were identified. Three were excluded as their revision had been to another metal head. The remaining 33 were revised to a ceramic head with a titanium sleeve. We describe the presentation, revision findings, and risk of complications in these patients.ResultsThe patients presented with pain, swelling, stiffness, or instability and an inflammatory mass was confirmed radiologically. Macroscopic material deposition on the trunnion was seen in all patients, associated with ARMD. Following revision, six (18.2%) dislocated, requiring further revision in four. Three (9.1%) developed a deep infection and six (18.2%) had significant persistent pain without an obvious cause. One developed a femoral artery thrombosis after excision of an iliofemoral pseudotumor, requiring a thrombectomy.ConclusionThe risk of serious complications following revision MoP THA for ARMD associated with trunnionosis is high. In the presence of extensive tissue damage, a constrained liner or dual mobility construct is recommended in these patients. Cite this article: Bone Joint J 2018;100-B:720–4.</jats:sec
The Murchison Widefield Array: Design Overview
The Murchison Widefield Array (MWA) is a dipole-based aperture array
synthesis telescope designed to operate in the 80-300 MHz frequency range. It
is capable of a wide range of science investigations, but is initially focused
on three key science projects. These are detection and characterization of
3-dimensional brightness temperature fluctuations in the 21cm line of neutral
hydrogen during the Epoch of Reionization (EoR) at redshifts from 6 to 10,
solar imaging and remote sensing of the inner heliosphere via propagation
effects on signals from distant background sources,and high-sensitivity
exploration of the variable radio sky. The array design features 8192
dual-polarization broad-band active dipoles, arranged into 512 tiles comprising
16 dipoles each. The tiles are quasi-randomly distributed over an aperture
1.5km in diameter, with a small number of outliers extending to 3km. All
tile-tile baselines are correlated in custom FPGA-based hardware, yielding a
Nyquist-sampled instantaneous monochromatic uv coverage and unprecedented point
spread function (PSF) quality. The correlated data are calibrated in real time
using novel position-dependent self-calibration algorithms. The array is
located in the Murchison region of outback Western Australia. This region is
characterized by extremely low population density and a superbly radio-quiet
