    Acute myocardial infarction in labor: a rare case report

    Myocardial infarction (MI) presenting first time in labor is rare with incidence of 3 case per 1,00,000 and its management during labor is rarely seen in literature. In this case, a 29-year-old gravida 2 para 1 with previous caesarean section 1 year back with history of postpartum eclampsia presented with acute breathlessness and lower abdominal pain. She was diagnosed with acute MI. Patient had emergency caesarean section due to scar tenderness. Post operatively patient was managed in ICU with multi-disciplinary team of critical care, cardiologist and obstetrician.

    Study of hematological profile of systemic lupus erythematosus

    Background: Hematological abnormalities are prevalent in systemic lupus erythematosus (SLE), with approximately 72% of patients experiencing anemia, primarily in the form of autoimmune hemolytic anemia. Other manifestations include leukopenia (32%), lymphopenia (54%), and thrombocytopenia (23%). This study aimed to further investigate these hematological manifestations, which may serve as presentations of SLE and might be overlooked if suspicion levels are low. Methods: A descriptive observational study was conducted over 18 months at a Sir Ganga Ram hospital, a tertiary care centre. One hundred thirteen SLE cases, comprising newly diagnosed patients and previously diagnosed patients’ records, were reviewed, with 13 cases excluded based on exclusion criteria. One hundred patients with hematological abnormalities and fulfilling ≥4 SLICC criteria for SLE diagnosis were included in the study. Results: One hundred cases of SLE with hematological abnormalities (88 women, 12 men) were analyzed. At presentation, 83% (n=84) of patients displayed hematological manifestations. The most prevalent abnormality was anemia, present in 72% of the study group, with a mean hemoglobin level of 10.073 gm/dl. Additionally, leukopenia, lymphopenia, thrombocytopenia, and pancytopenia were observed in 32%, 54%, 23%, and 14% of cases, respectively. Neutropenia was detected in only 5% of cases. Conclusions: Hematological manifestations are the most common presenting signs of SLE in North India. Anemia, with a multifactorial basis, is the most frequent hematological abnormality throughout the disease course. A high index of suspicion is crucial when evaluating cases of anemia in daily clinical practice

    Gallbladder reporting and data system (GB-RADS) for risk stratification of gallbladder wall thickening on ultrasonography:an international expert consensus

    The Gallbladder Reporting and Data System (GB-RADS) ultrasound (US) risk stratification is proposed to improve consistency in US interpretations, reporting, and assessment of risk of malignancy in gallbladder wall thickening in non-acute setting. It was developed based on a systematic review of the literature and the consensus of an international multidisciplinary committee comprising expert radiologists, gastroenterologists, gastrointestinal surgeons, surgical oncologists, medical oncologists, and pathologists using modified Delphi method. For risk stratification, the GB-RADS system recommends six categories (GB-RADS 0–5) of gallbladder wall thickening with gradually increasing risk of malignancy. GB-RADS is based on gallbladder wall features on US including symmetry and extent (focal vs. circumferential) of involvement, layered appearance, intramural features (including intramural cysts and echogenic foci), and interface with the liver. GB-RADS represents the first collaborative effort at risk stratifying the gallbladder wall thickening. This concept is in line with the other US-based risk stratification systems which have been shown to increase the accuracy of detection of malignant lesions and improve management.     Tumours in the nasal cavity and paranasal sinuses were recognized during the time of Hippocrates. Whillis (1948) defined tumours as “an abnormal mass of tissue, the growth of which exceeds and is coordinated with that of the normal tissues and persists in the same excessive manner after cessation of the stimuli which evoked the Change. From the table I it is evident that only 5 cases of malignant and 3 cases of fungal Granuloma, there was bony erosion. In 19 cases of antro chonal polyp and antrum and ethmoid sinus is found to be hazy. From the table I, on radiological study, sinuses are found to be hazy in 86.4% cases of antro choanal polyp and 89.4% cases of ethmoidal polyp. This finding is in accordance with that of miles (1971) who started that usually there is some degree of opacity of sinuses either due to thickening of mucosa or superadded infection. erosion of bone is found in 70% cases of malignancy and the 3 case of fungal granuloma

    The nose of the prominent feature of the face and often catches the attention of the face and often catches the attention on the looker whenever it is deformed or altered in shape. Nose is not only important functionally, but automatically and aesthetically as well as. The observations are made from the study of 130 patients with mass in nose attending E.N.T. Department of V.S.S. Medical College Hospital, Burla from September 2009 to August 2011. Out of total number of 20,215 patients attending E.N.T. OPD 459. 130 cases of mass in nose, attending the ENT OPD, VSS Medical College Hospital, Burla from September 2009 to August 2011 were taken for study, which comprised 0.64% of total patients of that period. Rhinosporidiosis was found        to    the most  common     lesion (34.61%) among them followed by AC polyp (16.92%) & Ethmoidal polyp (13.84%). Hemangioma constitute 7.71% of cases, Sq cell papilloma 4.6°/o inverted papilloma 2.3%, nasopharyngeal angiofibroma 2.2%

    Multi-zinc finger proteins are an emerging class of cofactors in DEAH-RHA RNA helicases across highly divergent eukaryotic lineages. DEAH-RHA helicase•zinc finger cofactor partnerships predate the split of kinetoplastid protozoa, which include several human pathogens, from other eukaryotic lineages 100-400 Ma. Despite a long evolutionary history, the prototypical DEAH-RHA domains remain highly conserved. This short review focuses on a recently identified DEAH-RHA helicase•zinc finger cofactor system in kinetoplastid RNA editing, and its potential functional parallels with analogous systems in embryogenesis control in nematodes and antivirus protection in humans

    Effect of Near Work on Intraocular Pressure in Emmetropes

    Objective. To determine whether accommodation induced by reading alters intraocular pressure (IOP) in healthy, young, emmetropic adults and to document the duration and magnitude of this effect. Design. Cross-sectional study. Participants. Fifteen healthy, emmetropic young adults. Methods. Subjects performed 20 minutes of near work (reading at 33 cm) followed by 20 minutes of far work (reading at 520 cm) while IOP was measured using an iCare tonometer at baseline and every 5 minutes thereafter. Statistical analysis was performed using repeated measures ANOVA. Main Outcome Measures. Intraocular pressure. Results. IOP decreased significantly compared to baseline IOP after 10 minutes of near work (average change of −1.60 ± 2.2 (SD) mm Hg, p<0.05). IOP remained lower than baseline IOP throughout all subsequent near and far work. The difference in IOP at the end of experimentation compared to baseline IOP was −1.87 ± 1.81 mm Hg (p<0.05). The minimum IOP reached during experimentation compared to baseline was on average −3.8 ± 2.2 (SD) mm Hg (range: 0 to −8.0 mm Hg). 13 of 15 subjects (87%) and 9 of 15 subjects (60%) had at least one IOP measurement of at least 2 mm Hg and 4 mm Hg less than their baseline IOPs, respectively. Conclusions. Near work decreases IOP in healthy emmetropes, and this effect is sustained for at least 20 minutes after discontinuing prolonged near work. Providers may need to consider this effect when measuring IOP in clinical practice

    Spontaneous rupture of hepatic metastasis causing hemoperitoneum is a rare entity. Ruptured hepatic metastasis has typical imaging findings on biphasic CT which may help in clinching the diagnosis. We present a case of rupture hepatic metastasis from choriocarcinoma in a young-female patient who was managed by transcatheter hepatic artery embolization. A brief review of the imaging features and therapeutic options for the ruptured hepatic metastases is discussed along with the case
