    Monsoon and cyclone induced wave climate over the near shore waters off Puduchery, south western Bay of Bengal

    Seasonal and annual variations in wave characteristics are studied based on measurements during 2009-2011 using wave rider buoy. Presence of swells generated by the south Indian Ocean cyclones are found over the south western Bay of Bengal during pre-monsoon season. Maximum significant wave height is measured 6 m during Thane cyclone in December, 2011. Long period waves are observed mostly during the summer (SW) monsoon and are negligible in the winter (NE) monsoon period. Short period waves dominate (63) the wave climate during the NE monsoon. Wave spectra during the SW monsoon are multi peaked whereas during the post monsoon season single peaked spectra are found. Single peaked spectra observed during SW monsoon of 2011 coincides with the presence of positive Indian Ocean Dipole. Waves during the pre monsoon and SW monsoon season are influenced by sea breeze. Analysis indicates that directional width is minimum for waves from the NE since they are wind waves and maximum for waves from SE since they are swells. Study indicates that wave climate of the south western Bay of Bengal is in contrast to that in eastern Arabian Sea during the SW monsoon

    Observational evidence of summer shamal swells along the west coast of India

    Wave data collected off Ratnagiri, which is on the west coast of India, in 2010 and 2011 are used to examinethe presence of the summer shamal swells. This study also aims to understand variations in wave characteristics and associated modifications in wind sea propagation at Ratnagiri. Wind data collected using an autonomous weather station (AWS), along with Advanced Scatterometer (ASCAT) and NCEP data, areused to identify the presence of summer shamal winds along the west coast f the Indian subcontinent and on the Arabian Peninsula. NCEP and ASCAT data indicate the presence of summer shamal winds over the Arabian Peninsula and northwesterly winds at Ratnagiri. This study identifies the presence of swells from the northwest that originate from the summer shamal winds in the Persian Gulf and that reach Ratnagiri during 30 of the summer shamal period. AWS data show the presence of northwest winds during May and southwest winds during the strong southwest monsoon period (June-August). Another important factor identified at Ratnagiri that is associated with the summer shamal events is the direction of wind sea waves.During the onset of the southwest monsoon (May), the sea direction is in the direction of swell propagation (northwest); however, during the southwest monsoon (June-August), a major part of the wind sea direction is from the southwest. The average occurrence of summer shamal swells is approximately 22 during the southwest monsoon period. An increase in wave height is observed during June and July at Ratnagiri due to the strong summer shamal event

    Effect of zirconium oxide and cellulose nanoparticles addition on the flexural strength, impact strength and translucency of heat polymerized acrylic resin: an in vitro study.

    Background: Polymethyl methacrylate denture base material is considered the most popular denture base material to date. The advantages of the PMMA       include low cost, biocompatibility, ease of processing, stability in the oral environment, and acceptable aesthetics. To improve the acrylic polymer’s properties for removable acrylic appliances, the significant issues to be addressed are its low mechanical properties such as impact, bending, and fatigue. Aim: This study was aimed to evaluate the effect of incorporating different concentrations of zirconium oxide and cellulose nanoparticles on flexural strength, and impact strength and translucency of heat polymerized acrylic resin. Materials and methods: A total of 180 acrylic specimens were made and divided into two groups, which comprises 90 specimens in each. Group I and Group II were reinforced with ZrO2 and cellulose nanoparticles, respectively. Each group was divided into three subgroups depending on the properties to be evaluated i.e., flexural strength, Impact strength, and Translucency, respectively. Each subgroup was further divided into three based on the concentrations (1.5 wt%, 2.5 wt% and 5.0 wt%) of the nanoparticles. The flexural strength was determined using a universal testing machine. The Izod impact tester was used to evaluate the impact strength. Translucency measured by UV visible spectrophotometer. The obtained data were analysed using one way ANOVA within the group followed by posthoc comparison by TUKEY’S method for the comparison between groups. Results: Acrylic specimens incorporated with 2.5 wt% ZrO2 exhibited more mean flexural strength, and the specimens with 2.5 wt% and 5.0 wt% cellulose nanoparticles showed the highest impact strength and translucency, respectively. One-way ANOVA showed significant differences (p=0.000) between the groups. Conclusion: PMMA incorporated with 2.5 wt% of ZrO2 NPs, 2.5 wt% and 5.0 wt% of cellulose NPs showed superior flexural strength, impact strength, and translucency, respectively.

    Bottom-up or top-down: unit cost estimation of tuberculosis diagnostic tests in India.

    SETTING: Of 18 sites that participated in an implementation study of the Xpert® MTB/RIF assay in India, we selected five microscopy centres and two reference laboratories. OBJECTIVE: To obtain unit costs of diagnostic tests for tuberculosis (TB) and drug-resistant TB. DESIGN: Laboratories were purposely selected to capture regional variations and different laboratory types. Both bottom-up and the top-down methods were used to estimate unit costs. RESULTS: At the microscopy centres, mean bottom-up unit costs were respectively US0.83(rangeUS0.83 (range US0.60-US1.10)andUS1.10) and US12.29 (US11.61US11.61-US12.89) for sputum smear microscopy and Xpert. At the reference laboratories, mean unit costs were US1.69forthedecontaminationprocedure,US1.69 for the decontamination procedure, US9.83 for a solid culture, US11.06foraliquidculture,US11.06 for a liquid culture, US29.88 for a drug susceptibility test, and US18.18foralineprobeassay.Topdownmeanunitcostestimateswerehigherforalltests,andforsputumsmearmicroscopyandXperttheseincreasedtorespectivelyUS18.18 for a line-probe assay. Top-down mean unit cost estimates were higher for all tests, and for sputum smear microscopy and Xpert these increased to respectively US1.51 and US$13.58. The difference between bottom-up and top-down estimates was greatest for tests performed at the reference laboratories. CONCLUSION: These unit costs for TB diagnostics can be used to estimate resource requirements and cost-effectiveness in India, taking into account geographical location, laboratory type and capacity utilisation

    CARRS Surveillance study: design and methods to assess burdens from multiple perspectives

    Background Cardio-metabolic diseases (CMDs) are a growing public health problem, but data on incidence, trends, and costs in developing countries is scarce. Comprehensive and standardised surveillance for non-communicable diseases was recommended at the United Nations High-level meeting in 2011. Aims: To develop a model surveillance system for CMDs and risk factors that could be adopted for continued assessment of burdens from multiple perspectives in South-Asian countries. Methods Design: Hybrid model with two cross-sectional serial surveys three years apart to monitor trend, with a three-year prospective follow-up of the first cohort. Sites: Three urban settings (Chennai and New Delhi in India; Karachi in Pakistan), 4000 participants in each site stratified by gender and age. Sampling methodology: Multi-stage cluster random sampling; followed by within-household participant selection through a combination of Health Information National Trends Study (HINTS) and Kish methods. Culturally-appropriate and methodologically-relevant data collection instruments were developed to gather information on CMDs and their risk factors; quality of life, health-care utilisation and costs, along with objective measures of anthropometric, clinical and biochemical parameters. The cohort follow-up is designed as a pilot study to understand the feasibility of estimating incidence of risk factors, disease events, morbidity, and mortality. Results The overall participant response rate in the first cross-sectional survey was 94.1% (Chennai 92.4%, n = 4943; Delhi 95.7%, n = 4425; Karachi 94.3%, n = 4016). 51.8% of the participants were females, 61.6% \u3c 45years, 27.5% 45–60years and 10.9% \u3e60 years. Discussion This surveillance model will generate data on prevalence and trends; help study the complex life-course patterns of CMDs, and provide a platform for developing and testing interventions and tools for prevention and control of CMDs in South-Asia. It will also help understanding the challenges and opportunities in establishing a surveillance system across countries

    Cardiovascular risk profile and management of atrial fibrillation in India: Real world data from RealiseAF survey

    BACKGROUND: Atrial fibrillation (AF) is the most common sustained arrhythmia with high risk for many cardiovascular (CV) complications. Adherence to recommended management guidelines is important to avoid complications. In India, there is little knowledge on how AF is managed in real world. METHODS: This is a cross-sectional study of patients in India enrolled in RealiseAF survey between February 2010 and March 2010 with a diagnosis of AF within the last 12 months. RESULTS: From 15 centers, 301 patients {mean age 59.9 years (14.4); 52.5% males} were recruited. AF was controlled in 50% of patients with 77 (26.7%) in sinus rhythm and 67 (23.3%) with heart rate <80beats/min. Hypertension (50.8%), valvular heart disease (40.7%), heart failure (25.9%), and diabetes (20.4%) were the most common underlying CV diseases. Increased risk for stroke (CHADS2 score≥2) was present in 36.6%. Most of the patients (85%) were symptomatic. AF was paroxysmal, persistent, and permanent in 28.7%, 22.7%, and 34.3% respectively. In 14%, AF was diagnosed as first episode. Forty-six percent of patients had rate control, 35.2% rhythm control, 0.3% both strategies, and 18.4% received no therapy for AF before the visit. At the end of the visit, adoption to rate control strategy increased to 52.3% and patients with no therapy decreased to 7%. CONCLUSION: AF in India is not adequately controlled. Concomitant CV risk factors and risk of stroke are high. The study underscores the need for improved adoption of guideline-directed management for optimal control of AF and reducing the risk of stroke

    Towards a large-scale quantum simulator on diamond surface at room temperature

    Strongly-correlated quantum many-body systems exhibits a variety of exotic phases with long-range quantum correlations, such as spin liquids and supersolids. Despite the rapid increase in computational power of modern computers, the numerical simulation of these complex systems becomes intractable even for a few dozens of particles. Feynman's idea of quantum simulators offers an innovative way to bypass this computational barrier. However, the proposed realizations of such devices either require very low temperatures (ultracold gases in optical lattices, trapped ions, superconducting devices) and considerable technological effort, or are extremely hard to scale in practice (NMR, linear optics). In this work, we propose a new architecture for a scalable quantum simulator that can operate at room temperature. It consists of strongly-interacting nuclear spins attached to the diamond surface by its direct chemical treatment, or by means of a functionalized graphene sheet. The initialization, control and read-out of this quantum simulator can be accomplished with nitrogen-vacancy centers implanted in diamond. The system can be engineered to simulate a wide variety of interesting strongly-correlated models with long-range dipole-dipole interactions. Due to the superior coherence time of nuclear spins and nitrogen-vacancy centers in diamond, our proposal offers new opportunities towards large-scale quantum simulation at room temperatures

    Epidemiology and patterns of care for invasive breast carcinoma at a community hospital in Southern India

    <p>Abstract</p> <p>Background</p> <p>Breast cancer incidence in India is on rise. We report epidemiological, clinical and survival patterns of breast cancer patients from community perspective.</p> <p>Methods</p> <p>All breast cancer patients treated at this hospital from July 2000 to July 2005 were included. All had cytological or histological confirmation of breast cancer. TNM guidelines for staging and Immunohistochemistry to assess the receptor status were used. Either lumpectomy with axillary lymph node dissection or Modified radical mastectomy (MRM) was done for operable breast cancer, followed by 6 cycles of adjuvant chemotherapy with FAC or CMF regimens to patients with pT >1 cm or lymph node positive or estrogen receptor negative and radiotherapy to patients after breast conservation surgery, pT size > 5 cm, 4 or more positive nodes and stage IIIB disease. Patients with positive Estrogen receptor or Progesterone receptor were advised Tamoxifene 20 mg per day for 3 years. Descriptive analysis was performed. Independent T test and Chi-square test were used. Overall survival time was computed by Kaplan – Meier method.</p> <p>Results</p> <p>Of 1488 cancer patients, 122 (8.2%) had breast cancer. Of 122 patients, 96.7% had invasive breast carcinoma and 3.3% had sarcoma. 94% came from the rural and semi urban areas. Premenopausal women were 27%. The median age was 50 years. Stage I-6.8%, II-45.8%, III-22%, IV-6.8%, Bilateral breast cancer – 2.5%. The mean pT size was 3.9 cm. ER and PR were positive in 31.6% and 28.1% respectively. MRM was done in 93.8%, while 6.3% patients underwent breast conservation surgery. The mean of the lymph nodes dissected were 3. CMF and FAC regimens were used in 48.8% and 51.2% of patients respectively. FAC group were younger than the CMF group (43.6 yr vs. 54 yrs, P = 0.000). Toxicities were more in FAC than CMF group, alopecia (100% vs. 26.2%), grade2 or more emesis (31.8% vs. 9.2%), grade2 or more fatigue (40.9% vs.19%), anemia (43.1% vs. 16.6%). Median Survival for the cohort was 50.8 months. ER positive patients had better median survival (P = 0.05).</p> <p>Conclusion</p> <p>MRM was the most frequent surgical option. CMF and FAC showed equivalent survival. FAC chemotherapy was more toxic than CMF. ER positive tumors have superior survival. Overall 3 year survival was 70 percent</p

    Fermentation, Isolation, Structure, and antidiabetic activity of NFAT-133 produced by Streptomyces strain PM0324667

    Type-2 diabetes is mediated by defects in either insulin secretion or insulin action. In an effort to identify extracts that may stimulate glucose uptake, similar to insulin, a high throughput-screening assay for measuring glucose uptake in skeletal muscle cells was established. During the screening studies to discover novel antidiabetic compounds from microbial resources a Streptomyces strain PM0324667 (MTCC 5543, the Strain accession number at Institute of Microbial Technology, Chandigarh, India), an isolate from arid soil was identified which expressed a secondary metabolite that induced glucose uptake in L6 skeletal muscle cells. By employing bioactivity guided fractionation techniques, a tri-substituted simple aromatic compound with anti-diabetic potential was isolated. It was characterized based on MS and 2D NMR spectral data and identified as NFAT-133 which is a known immunosuppressive agent that inhibits NFAT-dependent transcription in vitro. Our investigations revealed the antidiabetic potential of NFAT-133. The compound induced glucose uptake in differentiated L6 myotubes with an EC50 of 6.3 ± 1.8 μM without activating the peroxisome proliferator-activated receptor-γ. Further, NFAT-133 was also efficacious in vivo in diabetic animals and reduced systemic glucose levels. Thus it is a potential lead compound which can be considered for development as a therapeutic for the treatment of type-2 diabetes.
