Insulin resistance in endocrinopathies
Insulinooporność jest stanem zmniejszonego działania insuliny na tkanki
docelowe mimo prawidłowego lub podwyższonego stężenia insuliny
w surowicy krwi. Może mieć charakter pierwotny lub wtórny, przebiegać
w sposób utajony lub ujawniać się poprzez rozmaite zaburzenia,
takie jak: zaburzenia gospodarki węglowodanowej, hipercholesterolemia,
hipertriglicerydemia, otyłość typu androidalnego, nadciśnienie
tętnicze czy hiperurykemia. Obserwujemy ją także w wielu endokrynopatiach
- w tym przypadku zwiększa ryzyko wystąpienia chorób
sercowo-naczyniowych. W prezentowanym artykule autorzy przedstawiają
metody oceny insulinooporności, jak też choroby gruczołów
wydzielania wewnętrznego, w których ten stan występuje.Insulin resistance is a state of decreased activity of insulin in target cells,
despite its normal or elevated concentration in blood serum. This state
can be primary or secondary, can be latent or manifest itself in the form
of various disturbances such as: abnormal carbohydrate metabolism,
hypercholesterolaemia, hypertriglicerydaemia, androidal type of obesity,
hypertension or hyperurykaemia. It can also appear in many endocrine
diseases, increasing the risk of cardiovascular complications. In
this article, authors present methods of insulin resistance’s measurements
and also endocrinopathies where this disturbance develop
Measuring salivary androgens as a useful tool in the diagnosis of polycystic ovary syndrome
Introduction: Polycystic ovary syndrome (PCOS) is one of the commonest endocrinopathies. Clinically it can present as oligo-/amenorrhoea,
hyperandrogenism and/or fertility problems.
Material and methods: The study involved 60 women admitted to the Department of Internal Medicine and Endocrinology at the Medical
University of Warsaw. The initial evaluation, including case history and two-dimensional vaginal ultrasound, was performed by gynaecologists.
All hormonal investigations (fT, free testosterone; bioT, bioavailable testosterone; T, total testosterone; T EQ, free testosterone by
equilibrium dialysis; A, androstenedione; A EQ, free androstenedione by equilibrium dialysis; salA, salivary androstenedione; salT, salivary
testosterone) were performed. Anthropometrical data, excess facial and body hair, acne, and menstrual cycle frequency were also assessed.
Results: Increased levels of T, fT, T EQ and A were noted in 20.0%, 89.8%, 100% and 28.3% of women, respectively. A very high correlation
was found between salivary androstenedione and free androstenedione estimated by EQ in plasma (p < 0.05, r = 0.67), and total
androstenedione in plasma (p < 0.05, r = 0.71). Correlation between salT and T was r = 0.31, p < 0.05 and salT and T EQ was r = 0.26,
p = 0.04. Correlation between salA/salT and T, A in plasma (respective values r = 0.39 and r = 0.28, p < 0.01) and between salA/salT and
A EQ, T EQ (respectively r = 0.34 and r = 0.48, p < 0.01) was evident.
Conclusions: SalA/salT ratio may be a good indicator of hyperandrogenism in women. We also confirm that measurement of androstenedione
in plasma may be useful in making a diagnosis of PCOS.Wstęp: Zespół policystycznych jajników (PCOS) jest jedną z najczęstszych endokrynopatii. Charakteryzuje się zaburzeniami
miesiączkowania o charakterze rzadkich miesiączek, niepłodnością, hiperandrogenizmem.
Materiał i metody: Do badania włączono 60 kobiet hospitalizowanych w Klinice Chorób Wewnętrznych i Endokrynologii Warszawskiego
Uniwersytetu Medycznego. Przeprowadzono badanie podmiotowe i przedmiotowe, łącznie z badaniem ultrasonograficznym narządu
rodnego. Analizie poddano panel badań hormonalnych (fT, wolny testosteron; bioT, testosteron biodostępny; T, całkowity testosteron;
T EQ, wolny testosteron wyznaczony metodą dializy równowagowej; A, androstendion; A EQ, wolny androstendion wyznaczony metodą
dializy równowagowej; salA, androstendion w ślinie; salT, testosteron w ślinie) wykonanych w godzinach porannych, między 3. a 6. dniem
cyklu. Oceniano parametry antropometryczne, stopień hirsutyzmu oraz charakter cyklów miesiączkowych.
Wyniki: Podwyższone stężenie androgenów: T, fT, T EQ i A odnotowano u 20,0, 89,8, 100,0 i 28,3% badanych kobiet. Odnotowano wysoką
korelację między salA i A EQ (p < 0,05, r = 0,67) i A (p < 0,05, r = 0,71). Korelacja między salT i T wynosiła r = 0,31, p < 0,05 oraz salT
i T EQ była równa r = 0,26, p = 0,04. Wykazano zależność pomiędzy salA/salT oraz T, A (odpowiednio r = 0,39 i r = 0,28,
Adiponectin, insulin resistance and atherosclerosis
Adiponektyna (ADPN) jest cytokiną wytwarzaną w tkance tłuszczowej.
Wpływa na szereg procesów metabolicznych, szczególnie
na przemianę węglowodanów i kwasów tłuszczowych w wątrobie
i mięśniach, pośrednio oddziałując na wrażliwość tkanek
obwodowych na insulinę. Adiponektyna może w związku z tym
wykazywać działanie przeciwmiażdżycowe, zwiększające insulinowrażliwość,
a także przeciwzapalne. Jako potencjalny biomarker
służący oznaczaniu insulinooporności może być przydatna
w ocenie klinicznej. Zmiany stężenia ADPN wykazują związek nie
tylko z insulinoopornością i ryzykiem rozwoju zaburzeń gospodarki
węglowodanowej, ale także chorób sercowo-naczyniowych.

186–191Adiponectin (ADPN) is adipocytokine secreted by adipose tissue.
Adiponectin modulates many of the metabolic processes, especially
the metabolism of carbohydrates and fatty acids in the liver
cells and muscles, indirectly influences on the insulin resistance.
Thereby it shows the anti-inflammatory, probably anti-atherogenic
activity and improves insulin sensitivity. Adiponectin as a biomarker
of insulin resistance that may have potential diagnostic value.
Changes of the adiponectin’s concentration are related not only to insulin resistance and the risk of the development of the diabetes
of type 2, but also the risk of cardiovascular diseases.

Controversies concerning the pathogenesis of polycystic ovary syndrome
Zespół policystycznych jajników (PCOS) jest jedną z najczęstszych
endokrynopatii, występującą u około 10% kobiet w wieku reprodukcyjnym.
Dotychczas nie ustalono jednoznacznej przyczyny
rozwoju tej jednostki chorobowej. Głównymi objawami PCOS są
zaburzenia miesiączkowania, będące przyczyną niepłodności, oraz
hiperandrogenizm. Objawy kliniczne u pacjentek są wyrażone
w różnym stopniu, co powoduje różnorodność obrazu klinicznego.
U części kobiet współistnieją insulinooporność, cukrzyca typu 2,
otyłość lub zaburzenia gospodarki lipidowej. Definicja PCOS ewoluowała
przez okres niespełna jednego wieku. Jednak mimo posiadania
coraz lepszych narzędzi diagnostycznych są wątpliwości,
czy obowiązująca definicja jest do końca słuszna.

with a heterogeneous presentation whose etiology is still
uncertain. It appears in about 10% women of reproductive age.
The criteria of PCOS take account of the broad spectrum of presenting
features of PCOS, including women with menstrual disturbance,
signs of androgen excess. PCOS is common of its heterogeneity.
The prevalence of obesity, disturbances of lipid profile,
diabetes mellitus type 2, impaired fasting glucose, impaired glucose
tolerance in women with PCOS is higher than in normal population.
Definition of PCOS has been evaluated through the years,
but still although having better diagnostic tools there is doubt if
actual definition is complete.

Insulinopodobny czynnik 3 - nowy hormon związany z zespołem policystycznych jajników?
Introduction: The aim of this study was to find a correlation between insulin-like factor 3 (INSL3) and androgens: androstenedione (A),
free testosterone (fT), and total testosterone (T), in two groups of polycystic ovary syndrome (PCOS) women: those with a body mass
index (BMI) lower than 25 kg/m2 and those with a BMI higher than 25 kg/m2. The association between INSL3 and other serum parameters:
luteinising hormone (LH), follicle-stimulating hormone (FSH), dehydroepiandrosterone sulphate (DHEA-S), sex hormone binding globulin
(SHBG) and glucose and insulin were also investigated.
Material and methods: The study group comprised 37 PCOS women aged 27 ± 4 years. The control group consisted of 34 healthy, premenopausal
women (aged 24.2 ± 1.2) with regular menses and no signs of hyperandrogenism. There were 27 PCOS women of normal
weight (BMI < 25 kg/m2), and ten overweight individuals (BMI ≥ 25–30 kg/m2). Correlations between level of INSL3 and LH, FSH, T, fT,
A, DHEA-S, SHBG, metabolic tests, height, weight, and WHR (waist-to-hip ratio) were also investigated.
Results: PCOS women showed non-significantly higher levels of INSL3 compared to the healthy controls (64.6 ± 27.7 and 62.7 ± 20.0 ng/mL,
respectively). However, we identified very strong correlations between INSL3 and androstenedione (r = 0.48, p = 0.0115), and free
(r = 0.44, p = 0.0108) and total testosterone (r = 0.46, p = 0.0057) in the PCOS subgroup with a BMI of < 25 kg/m2. There was no statistically
significant correlation between INSL3 and LH in any subject of the PCOS group, nor between INSL3 and FSH, DHEA-S, glucose,
basal insulin concentration or HOMA-IR.
Conclusions: We found a positive correlation between INSL3 and androgens in PCOS women, especially those with a BMI of < 25 kg/m2.

(A), wolnego testosteronu (fT) oraz całkowitego testosteronu (T) u kobiet z zespołem policystycznych jajników (PCOS), z uwzględnieniem
wskaźnika masy ciała (BMI). Przedmiotem badania był także związek między INSL3 a hormonem luteotropowym (LH), folikulotropowym
(FSH), siarczanem dehydroepiandrosteronu (DHEA-S), białkiem wiążącym hormony płciowe (SHBG) oraz stężeniami glukozy i insuliny.
Materiał i metody: Badaną grupę stanowiło 37 kobiet z PCOS, średnia wieku 27 ± 4 lat. Grupę kontrolną stanowiły 34 zdrowe kobiety
(średnia wieku 24,2 ± 1,2 roku) z regularnymi miesiączkami, bez objawów hiperandrogenizacji. Kobiety z PCOS podzielono na grupy
pod względem masy ciała; grupę z prawidłową masą ciała tworzyło 27 kobiet, natomiast z nadwagą i otyłością - 10 badanych. Analizie
poddano także zależności między stężeniem INSL3 i stężeniami LH, FSH, T, fT, A, DHEA-S, SHBG, glukozy i insuliny oraz wskaźniki
antropometryczne (wzrost, masa ciała, wskaźnik talia-biodra).
Wyniki: U kobiet z PCOS wykazano wyższe stężenie INSL3 w porównaniu z grupą kontrolną (64,6 ± 27,7 i 62,7 ± 20,0 ng/mL, odpowiednio).
Wykazano silną zależność między stężeniem INSL3 a stężeniem androstendionu (r = 0,48; p = 0,0115), wolnego (r = 0,44; p = 0,0108)
i całkowitego testosteronu (r = 0,46; p = 0,0057) w grupie kobiet z PCOS o prawidłowej masie ciała. Nie wykazano zależności między
stężeniem INSL3 a stężeniami LH, FSH, DHEA-S, stężeniem glukozy, insuliny oraz wskaźnikiem insulinooporności HOMA.
Wnioski: Wykazano związek między stężeniem INSL3 a androgenemią u kobiet z PCOS, szczególnie silnie wyrażoną u kobiet z prawidłową

Discussion on the methods of testosterone’ concentration measurement
Testosteron to podstawowy męski hormon płciowy, oznaczanie jego stężenia ma istotne znaczenie dla oceny stopnia androgenizacji w wielu jednostkach chorobowych, zarówno u mężczyzn, jak i u kobiet. Stężenie tego hormonu, oznaczane wieloma powszechnie dostępnymi metodami nie zawsze koreluje z fenotypem klinicznym. W pracy przedstawiono dostępne metody, służące do oceny stężenia testosteronu całkowitego oraz jego frakcji (testosteronu biodostępnego i wolnego), opisując ich podstawowe cechy: wady i zalety.Testosterone is the main male sex hormone which determination is useful for assessment of androgen status in many diseases in men and women as well. It seems that serum levels of testosterone, when assayed by commonly used methods, do not correlate with clinical parameters. Ślina jako materiał diagnostyczny
Od około 10 lat ślina stanowi ważny element w diagnostyce laboratoryjnej. Naukowcy
wciąż poszukują coraz nowszych sposobów jej wykorzystania w diagnostyce medycznej
i monitorowaniu efektów leczenia. Autorzy artykułu przedstawiają jedynie
wybrane możliwości zastosowania śliny w różnych dziedzinach medycyny, takich jak
choroby infekcyjne, endokrynologia, kardiologia, zatrucia, choroby autoimmunologiczne.
Korzyści, jakie można odnieść, wykorzystując ślinę w diagnostyce medycznej są
wielorakie. Stosowanie śliny jako materiału diagnostycznego jest nieinwazyjne oraz
bezbolesne, a jednocześnie wygodne w użyciu zarówno u małych dzieci, jak i osób
w podeszłym wieku oraz aktywnych zawodowo. Ślina może być pobierana w warunkach
domowych, bez angażowania personelu pielęgniarskiego. Transport takiego materiału
nie wymaga specjalnej procedury, a związki oznaczane w ślinie zwykle charakteryzują
się dużą trwałością
Association between a nucleotide polymorphism in the calpain 10 gene and carbohydrate metabolism disturbances in patients with polycystic ovary syndrome
Polycystic ovary syndrome (PCO S) is a heterogeneous disease entity affecting a significant percentage of women of childbearing age. Despite of the prevalence of this endocrinopathy, the pathogenesis of PCO S is still not fully understood. In recent years, the significant role of insulin in the pathogenesis of PCO S has been emphasized. The coexistence of genetic syndromes of insulin resistance and hyperandrogenism, frequent occurrence of glucose tolerance disorders in women with PCO S and efficacy of the treatment, leading to increased sensitivity to insulin in the treatment of symptoms of PCO S, constitute indirect evidence for the participation of insulin in the pathogenesis of PCO S. Insulin resistance may be present in both lean and obese women with PCO S, and thus it is not directly related to body mass. As a result of years of intensive scientific work aimed at identifying the gene, it has been established that in the study population, a relationship between several polymorphisms (SNPs, single nucleotide polymorphisms) of the calpain 10 gene, a representative of a large family of cytoplasmic proteases, and development of carbohydrate metabolism disorders, including diabetes, exists. The risk of developing type 2 diabetes is not associated with a variant of a single polymorphism of the gene, but rather results from the haplotypes created by alleles of three SNPs, which have been numbered 19, 43, 63. This paper is a summary of recent reports on the presence of the relationship between a single nucleotide polymorphism of the calpain gene and the development of glucose metabolism disturbances in patients with polycystic ovary syndrome. The importance of the calpain 10 gene in the pathogenesis of type 2 diabetes seems vary in different populations and ethnic groups and is likely to be the subject of numerous further studies in the coming years
