    Utjecaj visine vjeđnog rasporka u primarnom položaju pogleda na stabilnost suznog filma

    This study aimed to investigate the effect of palpebral fissure height in primary gaze position in healthy individuals on tear film stability. In this cross-sectional study, 120 subjects (60 male and female each) were enrolled and divided according to age into two groups, i.e., group 1 (aged 18-50 years) and group 2 (aged 51 and older). Palpebral fissure height on both eyes was measured in primary gaze position with a clear plastic ruler held in a central vertical position between the upper and lower lid margin, and the standard tear break-up time (TBUT) test was performed to evaluate tear film stability. Palpebral fissure height was significantly higher in younger than older subjects in all measurements on both eyes (p<0.001), and TBUT was shorter in older than in younger subjects. In all subjects included in the study, palpebral fissure height was not related to TBUT (p=0.589). However, analyzing the two age groups separately, a significant negative correlation was found between the palpebral fissure height and TBUT in both groups of younger (p<0.001) and older (p=0.009) subjects. In conclusion, an enlarged exposed ocular surface due to higher palpebral fissure height in healthy individualā€™s primary gaze position negatively affects tear film stability expressed by TBUT.Cilj ovog istraživanja bio je ispitati utjecaj visine vjeđnog rasporka u primarnom položaju pogleda kod zdravih osoba na stabilnost suznog filma. U ovo presječno istraživanje bilo je uključeno 120 ispitanika (60 muÅ”karaca i 60 žena) i prema životnoj dobi podijeljeno u dvije ispitivane skupine: 1. skupina (u dobi od 18-50 godina) i 2. skupina (u dobi od 51 i viÅ”e godina). Visina vjeđnog rasporka na oba oka mjerena je u primarnom položaju pogleda prozirnim plastičnim ravnalom koje se držalo u srediÅ”njoj okomitoj liniji između ruba gornje i ruba donje vjeđe, a standardni test pucanja suznog filma (tear break-up time, TBUT) primijenjen je za procjenu stabilnosti suznog filma. Visina vjeđnog rasporka bila je u svim mjerenjima na oba oka značajno veća u mlađih nego u starijih ispitanika (p<0,001), a TBUT je bio kraći u starijih nego u mlađih ispitanika. U svih ispitanika uključenih u istraživanje visina vjeđnog rasporka nije bila povezana s TBUT-om (p=0,589). No, analizirajući dvije dobne skupine odvojeno utvrđena je značajna negativna povezanost između visine vjeđnog rasporka i TBUT-a u obje skupine, u mlađih (p<0,001) i starijih (p=0,009) ispitanika. U zaključku, povećana izložena očna povrÅ”ina zbog veće visine vjeđnog rasporka u primarnom položaju pogleda u zdravih osoba negativno utječe na stabilnost suznog filma izraženu TBUT-om

    Lens Replacement Habits in Soft Contact Lens Wearers

    The aim of this paper was to assess soft contact lens replacement habits of patients of Contact Lens Clinic, Zagreb University Hospital Department of Ophthalmology, Zagreb, Croatia. During spring of 2010, a survey was conducted in Contact Lens Laboratory, Zagreb University Hospital Department of Ophthalmology, Zagreb, Croatia. 47 completed surveys were collected and analyzed. The results of the survey have shown that the majority of soft contact lens wearers wear them longer than recommended by the manufacturer. It also showed that they eventually replace the lenses in regular intervals, indicating that they base their lens replacement on constant criteria. The main reasons for the eventual lens replacement were increased blur or discomfort while wearing lenses. Since it is obviously impossible to convince all lens wearers to replace their lenses as recommended by the manufacturer, it is up to the contact lens practitioners to find the way to reduce the risks of lens overwear. It may be achieved by introducing a method that will enable each soft contact lens wearer to choose an optimal lens replacement regimen without risking their eye health

    Bilateral Amaurosis Caused by Salmonella enteritidis Infection

    The aim of this paper was to show the potential of Salmonella enteritidis infection to eventually result in visual impairment. A case of salmonellosis in a 6-year-old boy, caused by intake of a cake made from eggs infected with Salmonella enteritidis, is presented. Prolonged duration of the disease was followed by complete remission of neurologic complications and persistent amaurosis with bilateral optic nerve atrophy. A severe form of Salmonella enterocolitis with neurologic involvement can lead to optic nerve lesion with consequential loss of vision

    Tear Film Status in Glaucoma Patients

    The purpose of this study was to determine the prevalence of ocular surface disease symptoms and the state of the tear film among patients with glaucoma who are receiving topical intraocular pressure lowering monotherapy. 62 responders were divided in 2 groups: 32 glaucoma patients and 30 healthy individuals. Tear film break-up time (TBUT), Schirmer 1 testing and ocular surface disease questionnaire were performed in both groups. 50% glaucoma patients and 25% control group participants had dry eye symptoms. Advanced disorder in TBUT was found in seven 21.9% glaucoma patients and was not found in the control group. Basal tear secretion measured by Schirmer 1 testing was found reduced in 87% of glaucoma patients and in 16.7% control group patients. Glaucoma patients on topical intraocular pressure lowering monotherapy showed a significantly higher prevalence of ocular surface disease symptoms, significantly reduced basal tear secretion and destabilisation of tear film in comparison with healthy volunteers

    Epidemiology of Monosymptomatic Optic Neuritis in Rijeka County, Croatia: Meteorological Aspects

    In order to analyze the meteorological factors possibly influencing the monosymptomatic optic neuritis in Rijeka County, northwestern part of Croatia, retrospective analysis of all cases diagnosed in Rijeka County in period from 1977 to 2001, was done. Meteorological data on monthly and yearly temperature, humidity, insolation in Rijeka County were collected from Croatian Meteorological and Hydrological Service. In the observed period there were 173 cases of monosymptomatic optic neuritis, female/male ratio was 1.66, and overall incidence was 2.18/100,000 inhabitants. There was no correlation of incidence with average yearly and monthly temperature, humidity, insolation, or month of birth. This is the first study on meteorological factors and optic neuritis in Croatia

    Optotipi su znakovi različite veličine, sustavno poredani na tabli za ispitivanje vidne oÅ”trine. Ljudsko oko posjeduje prostornu rezoluciju od 1ā€™ (Snellen). Sve su table izrađene tako da se optotipi osnovnog reda vide iz daljine ispitivanja pod kutom od 5ā€™, a njihov detalj pod kutom od 1ā€™. Nedostatak klasičnih tabli je nepravilna promjena veličine optotipa susjednih redova. To je 1976. rijeÅ”eno logMAR (Bailey-Lovie) tablom. Na njoj postoji stupnjevita promjena veličine optotipa s konstantnim omjerom promjene 1:1,2589. Ta je tabla postala standard za ispitivanje oÅ”trine vida. Preporučena je od sljedećih tijela: Svjetski oftalmoloÅ”ki zbor (International Council of Ophthalmology ā€“ ICO) 1984. i 2002.; Svjetska zdravstvena organizacija (WHO) 2003.; Međunarodna organizacija za standardizaciju (International Organisation for Standardisation ā€“ ISO) 1996. i Hrvatski zavod za norme (HZN) homologirao je ISO standard kao nHRN EN ISO 8596:1996. U Å”kolskoj medicini, medicini rada i oftalmologiji treba prijeći na upotrebu logMAR-tabli iz normativnih, medicinskih i praktičnih razloga.Optotypes are signs of different sizes, systematically placed on the visual acuity chart. The human eye has a spatial resolution of 1ā€™ of arc (Snellen). Optotypes in the basic row on all charts subtend 5ā€™ of arc and their detail 1ā€™ of arc, from the testing distance. The disadvantage of all classical charts is the irregular progresssion of optotype sizes in adjacent rows. This was solved by logMAR (Bailey-Lovie) chart in 1976. The chart was made with optotype sizes in steps with a constant ratio 1: 1.2589 (geometric progression). This chart has become the standard for visual acuity testing worldwide. It has been recommended by the following bodies: International Council of Ophthalmology (ICO), 1984 and 2002; World Health Organisation (WHO), 2003; International Organisation for Standardisation (ISO) 1996, and Croatian Standard Insitute homologised the ISO standard as nHRN EN ISO 8596:1996. For normative, medical and practical reasons it is advised that all school medicine, occupational health and ophthalmology departments adopt the logMAR charts system

    Subklinički optički neuritis

    Acute optic neuritis is often associated with multiple sclerosis. It is considered to be the most common ocular symptom of multiple sclerosis. In addition to acute optic neuritis, in patients with multiple sclerosis, subclinical optic neuritis is also described. It is characterized by slow progression and bilateral involvement, thus being unnoticed by the patient. The purpose of the present study was to assess vision impairment in multiple sclerosis patients without a history of acute optic neuritis, using a number of functional tests including visual field testing by Octopus 101 perimetry N1 program, contrast sensitivity testing by Pelli Robson chart, and color vision by Ishihara pseudoisochromatic plates. The study included 35 multiple sclerosis patients aged 18-50 years, without subjective signs of vision impairment and visual acuity 1.0 according to Snellen. Visual field defects were found in 28 patients. The most common defects of visual fields were retinal sensitivity depression in peripheral zone and nerve fiber bundle defect. Reduced contrast sensitivity was found in 30 (86%) patients. Study results indicated multiple sclerosis patients free from signs of optic neuritis to suffer vision function impairment, as demonstrated by Octopus perimetry and contrast sensitivity testing with Pelli Robson charts.Akutni optički neuritis je često povezan s multiplom sklerozom. Smatra se najčeŔćom oftalmoloÅ”kom manifestacijom te bolesti. Uz akutni optički neuritis opisani su i bolesnici sa supkliničkim oblikom. Svrha ovog istraživanja bila je procijeniti oÅ”tećenje vida kod bolesnika s multiplom sklerozom bez povijesti akutnog optičkog neuritisa primjenom brojnih funkcionalnih testova uključujući testiranje vidnog polja pomoću programa perimetrije Octopus 101 N1, testiranje kontrastne osjetljivosti Pelli Robsonovim tablicama i ispitivanje kolornog vida pseudoizokromnim tablicama prema Ishihari. Ispitivanje je uključilo 35 bolesnika s multiplom sklerozom u dobi od 18-50 godina bez subjektivnih znakova oÅ”tećenja vida i oÅ”trine vida 1,0 prema Snellenu. OÅ”tećenje vidnog polja pronađeno je u 28 bolesnika. NajčeŔća oÅ”tećenja su depresija osjetljivosti mrežnice u perifernoj zoni i defekt snopa živčanih vlakana. Smanjena kontrastna osjetljivost pronađena je u 30 (86%) bolesnika. Rezultati ispitivanja pokazali su da se u bolesnika s multiplom sklerozom bez subjektivnih znakova oÅ”tećenja vida mogu naći supklinička oÅ”tećenja pomoću perimetrije Octopus i Pelli Robsonovim tablicama za ispitivanje kontrastne osjetljivosti
