    An impact model to understand and improve work-life balance in early-career researchers in radiation oncology

    Purpose: The COVID-19 pandemic had a substantial effect on mental health and work productivity of early-career researchers working in Radiation Oncology (RO). However, the underlying mechanisms of these effects are unclear. The aim of the current qualitative study was therefore to achieve a better understanding of how these effects arose and could be managed in the future.Methods: This study was conducted jointly by RO and qualitative health researchers. Data was collected in four online Focus Groups with 6-11 RO researchers (total N = 31) working in Europe. The transcripts were analysed through a qualitative cross-impact analysis.Results: Causal relations were identified between seventeen variables that depict the impact of disrupted working conditions. Mental health and work productivity were indeed the most important affected variables, but relations between variables towards these impacts were complex. Relations could either be positive or negative and direct or indirect, leading to a cascade of interrelated events which are highly personal and could change over time. We developed the model 'impact of disrupted working conditions' depicting the identified variables and their re-lations, to allow more individual assessment and personalised solutions.Conclusion: The impacts of disrupted working conditions on RO researchers varied due to the complexity of interrelated variables. Consequently, collective actions are not sufficient, and a more personal approach is needed. Our impact model is recommended to help guide conversations and reflections with the aim of improving work/life balance. The participants showed high levels of personal responsibility towards their own mental health and work productivity. Although being an individual issue, a collective responsibility in devel-oping such approaches is key due to the dependency on organizational variables

    Professional quality of life and burnout amongst radiation oncologists:The impact of alexithymia and empathy

    Background and purpose: Different factors may influence the professional quality of life of oncology professionals. Among them, personality traits, as alexithymia and empathy, are underinvestigated. Alexithymia is about deficits in emotion processing and awareness. Empathy is the ability to understand another's 'state of mind'/emotion. The PROject on BurnOut in RadiatioN Oncology (PRO BONO) assesses professional quality of life, including burnout, in the field of radiation oncology and investigates alexithymia and empathy as contributing factors. Material and methods: An online survey was conducted amongst ESTRO members. Participants completed 3 validated questionnaires for alexithymia, empathy and professional quality of life: (a) Toronto Alexithymia Scale; (b) Interpersonal Reactivity Index; (c) Professional Quality of Life Scale. The present analysis, focusing on radiation/clinical oncologists, evaluates Compassion Satisfaction (CS), Secondary Traumatic Stress (STS) and Burnout and correlates them with alexithymia and empathy (empathic concern, perspective taking and personal distress) with generalized linear modeling. Significant covariates on univariate linear regression analysis were included in the multivariate linear regression model. Results: A total of 825 radiation oncologists completed all questionnaires. A higher level of alexithymia was associated to decreased CS (beta:-0.101; SE: 0.018; p <0.001), increased STS (beta: 0.228; SE: 0.018; p <0.001) and burnout (beta: 0.177; SE: 0.016; p <0.001). A higher empathic concern was significantly associated to increased CS (beta: 0.1.287; SE: 0.305; p = 0.001), STS (beta: 0.114; SE: 0.296; p <0.001), with no effect on burnout. Personal distress was associated to decreased CS (beta:-1.423; SE: 0.275; p <0.001), increased STS (beta: 1.871; SE: 0.283; p <0.001) and burnout (beta: 1.504; SE: 0.245; p <0.001). Conclusions: Alexithymic personality trait increased burnout risk, with less professional satisfaction. Empathic concern was associated to increased stress, without leading to burnout, resulting in higher professional fulfillment. These results may be used to benchmark preventing strategies, such as work-hour restrictions, peer support, debriefing sessions, and leadership initiatives for professionals at risk.     The transcriptional response of Caenorhabditis elegans to ivermectin exposure identifies novel genes involved in the response to reduced food intake

    We have examined the transcriptional response of Caenorhabditis elegans following exposure to the anthelmintic drug ivermectin (IVM) using whole genome microarrays and real-time QPCR. Our original aim was to identify candidate molecules involved in IVM metabolism and/or excretion. For this reason the IVM tolerant strain, DA1316, was used to minimise transcriptomic changes related to the phenotype of drug exposure. However, unlike equivalent work with benzimidazole drugs, very few of the induced genes were members of xenobiotic metabolising enzyme families. Instead, the transcriptional response was dominated by genes associated with fat mobilization and fatty acid metabolism including catalase, esterase, and fatty acid CoA synthetase genes. This is consistent with the reduction in pharyngeal pumping, and consequential reduction in food intake, upon exposure of DA1316 worms to IVM. Genes with the highest fold change in response to IVM exposure, cyp-37B1, mtl-1 and scl-2, were comparably up-regulated in response to short–term food withdrawal (4 hr) independent of IVM exposure, and GFP reporter constructs confirm their expression in tissues associated with fat storage (intestine and hypodermis). These experiments have serendipitously identified novel genes involved in an early response of C. elegans to reduced food intake and may provide insight into similar processes in higher organisms

Available on open access from Palgrave via the DOI in this recordThis chapter draws on instances of disordered breathing in Hamlet in order to examine the cultural signifcance of sighs in the early modern period, as well as in the context of current work in the feld of medical humanities. Tracing the medical history of sighing in ancient and early modern treatises of the passions, the chapter argues that sighs, in the text and the performance of the tragedy, exceed their conventional interpretation as symptoms of pain and disrupt meaning on the page and on stage. In the light of New Materialist theory, the air circulating in Hamlet is shown to dismantle narratives of representation, posing new questions for the future of medical humanities

    Professional quality of life and burnout among medical physicists working in radiation oncology: The role of alexithymia and empathy

    Background and purpose: The professional quality of life of radiation oncology professionals can be influenced by different contributing factors, including personality traits. Alexithymia involves deficits in emotion processing and awareness. Empathy is the ability to understand another’s ‘state of mind/emotion’. We investigated professional quality of life, including burnout, in radiation oncology, exploring the role of alexithymia and empathy and targeting the population of medical physicists (MPs), since this professional category is usually underrepresented in surveys exploring professional well-being in radiation oncology and MPs may experience professional distress given the increasing complexity of multimodal cancer care. Material and methods: An online survey was addressed to ESTRO members. Participants filled out three questionnaires to evaluate alexithymia, empathy and professional quality of life: a) Toronto Alexithymia Scale (TAS20); b) Interpersonal Reactivity Index (IRI); c) Professional Quality of Life Scale (ProQoL). Professional quality of life as per ProQoL was considered as dependent variable. The three domains of the ProQoL, namely compassion satisfaction (CS), secondary traumatic stress (STS) and burnout were correlated with alexithymia (as per TAS-20) and empathy (as per IRI with three subcategories: empathic concern, perspective taking and personal distress) and demographic/professional characteristics as independent variables. Generalized linear modeling was used. Significant covariates on univariate linear regression analysis were included in the multivariate linear regression model. Results: A total of 308 medical physicists completed all questionnaires. Alexithymia as per TAS-20 was correlated to decreased CS (β = −0.25, p < 0 0.001), increased likelihood for STS (β = 0.26, p < 0 0.001) and burnout (β = 0.47, p < 0 0.001). With respect to empathy, the ‘Empatic Concern’ subscale of the IRI was found to be a significant predictor for increased CS (β = 0.19, p = 0 0.001) and increased STS (β = 0.19, p < 0 0.001), without significant correlation with burnout. The individual’s perception of being valued by own’s supervisor was correlated to increased CS (β = 0.23, p < 0.001), and decreased burnout (β = −0.29, p < 0.001). Conclusions: Alexithymic personality trait increased the likelihood to develop burnout

    I In 1597, André Hurault, Sieur De Maisse and Ambassador Extraordinary from Henri IV, noted that although the English people still professed love for their aging queen, the sentiments of the nobility were such that &quot;the English would never again submit to the rule of a woman.&quot; 1 There may have been more coincidence between high and low opinion than de Maisse thought. On the evening of Elizabeth&apos;s death six years later, the streets of London were lit by festive bonfires and punctuated by cries of &quot;We have a king!&quot; 2 The advent of an orderly and Protestant succession does not in itself account for such a celebratory spirit; in fact, it was a significant transformation in the body politic, a reincorporation and regendering of monarchy, that was being heralded. Rather than a seamless transition of power reminding all the populace that the corporate body of the monarch was immortal, unchanging, and unaltered by the demise of a particular sovereign, the death of Elizabeth marked a breach in the body politic as much as a continuation of it, and one that could be figured, at least by some, as a welcome discontinuity. The queen is dead-long live the king. There were extensive and sincere eulogies, to be sure, heartfelt expressions of grief over the passing of Elizabeth, but during the last years of her reign the &quot;political misogynism of the early years&quot; 3 had also resurfaced strongly throughout her court and beyond its confines. It would not take many years of Jacobean rule to complicate the desire for a male sovereign, of course. As Christopher Haigh has noted, an idealized portrait of Elizabeth as a shrewd ruler and capable strategist emerged gradually over the first decade of James&apos;s reign, oftentimes in the form of a &quot;coded commentary&quot; on the defects of that reign. 4 But the recuperation and even reinvention of such a queen-Gloriana, the Virgin Queen, who had reigned for a remarkable span of forty-five years-seems a more complicated cultural process than Haigh&apos;s pragmatic account suggests. It is this process of accommodation and revision, marked as it is by an uncertain economy between mourning and misogyny, that I wish to examine here; I am interested not only in Elizabeth herself but also in the complex and ambivalent affective process that her death allows us to glimpse-a process that might be called mourning under the sign of patriarchy. Indeed, the possibility I wish to entertain is that, for the Renaissance, (male) mourning is sometimes difficult to dissociate from misogyny: that misogyny may in fact be an integral part of the mourning process when the lost object or ideal being processed is a woman, especially but not exclusively when that woman is a queen of England, too.
