    Perforated peptic ulcer (PPU) treatment: an Italian nationwide propensity score-matched cohort study investigating laparoscopic vs open approach

    BackgroundPerforated peptic ulcer (PPU) remain a surgical emergency accounting for 37% of all peptic ulcer-related deaths. Surgery remains the standard of care. The benefits of laparoscopic approach have been well-established even in the elderly. However, because of inconsistent results with specific regard to some technical aspects of such technique surgeons questioned the adoption of laparoscopic approach. This leads to choose the type of approach based on personal experience. The aim of our study was to critically appraise the use of the laparoscopic approach in PPU treatment comparing it with open procedure.MethodsA retrospective study with propensity score matching analysis of patients underwent surgical procedure for PPU was performed. Patients undergoing PPU repair were divided into: Laparoscopic approach (LapA) and Open approach (OpenA) groups and clinical-pathological features of patients in the both groups were compared.ResultsA total of 453 patients underwent PPU simple repair. Among these, a LapA was adopted in 49% (222/453 patients). After propensity score matching, 172 patients were included in each group (the LapA and the OpenA). Analysis demonstrated increased operative times in the OpenA [OpenA: 96.4 +/- 37.2 vs LapA 88.47 +/- 33 min, p = 0.035], with shorter overall length of stay in the LapA group [OpenA 13 +/- 12 vs LapA 10.3 +/- 11.4 days p = 0.038]. There was no statistically significant difference in mortality [OpenA 26 (15.1%) vs LapA 18 (10.5%), p = 0.258]. Focusing on morbidity, the overall rate of 30-day postoperative morbidity was significantly lower in the LapA group [OpenA 67 patients (39.0%) vs LapA 37 patients (21.5%) p = 0.002]. When stratified using the Clavien-Dindo classification, the severity of postoperative complications was statistically different only for C-D 1-2.ConclusionsBased on the present study, we can support that laparoscopic suturing of perforated peptic ulcers, apart from being a safe technique, could provide significant advantages in terms of postoperative complications and hospital stay

    Gastrointestinal emergencies (GE) are frequently encountered in emergency department (ED), and patients can present with wide-ranging symptoms. more than 3 million patients admitted to US hospitals each year for EGS diagnoses, more than the sum of all new cancer diagnoses. In addition to the complexity of the urgent surgical patient (often suffering from multiple co-morbidities), there is the unpredictability and the severity of the event. In the light of this, these patients need a rapid decision-making process that allows a correct diagnosis and an adequate and timely treatment. The primary endpoint of this Italian nationwide study is to analyze the clinicopathological findings, management strategies and short-term outcomes of gastrointestinal emergency procedures performed in patients over 18. Secondary endpoints will be to evaluate to analyze the prognostic role of existing risk-scores to define the most suitable scoring system for gastro-intestinal surgical emergency. The primary outcomes are 30-day overall postoperative morbidity and mortality rates. Secondary outcomes are 30-day postoperative morbidity and mortality rates, stratified for each procedure or cause of intervention, length of hospital stay, admission and length of stay in ICU, and place of discharge (home or rehabilitation or care facility). In conclusion, to improve the level of care that should be reserved for these patients, we aim to analyze the clinicopathological findings, management strategies and short-term outcomes of gastrointestinal emergency procedures performed in patients over 18, to analyze the prognostic role of existing risk-scores and to define new tools suitable for EGS. This process could ameliorate outcomes and avoid futile treatments. These results may potentially influence the survival of many high-risk EGS procedure

    Digitization of Cultural and Museum Heritage represents one of the most engaging challenges that would ensure a sustainable and ethical approach for next generations; digital technology’s pervasiveness imposes a comprehensive management of architectural heritage by producing facsimiles of buildings and artworks and by testing robust methodologies, with the final result of providing effective multipurpose models. In this context, the main goal of the present paper is to develop a semantically aware HBIM model that includes an intelligent objects parametrization, leveraging Extended Reality (XR) technologies and digital curation of contents to pursue the preservation of Cultural Heritage (CH) as a whole. This research is implemented in the case study of the Ducal Palace of Urbino that houses the National Gallery of Marche. It was chosen as a remarkable example of a museum located in an architectural complex with a relevant historical background and fine detail of shapes and mouldings. In Italy, as in other European scenarios, museums and their collections need suitable dissemination and management systems that take advantage of the recent digital paradigms. The challenging approach is to exploit existing platforms and software and to adopt a cognitive modelling process, able to develop tools supporting managers and museum curators while enabling user experiences using immersive and interactive features. In order to stress the workflow, this work proposes the use of families with high Level of Detail (LOD) and high Level of Information (LOI). The present article provides, as well, an accurate data enrichment process specifically designed for a gallery’s artworks such as paintings and sculptures, in line with the national and international policies. The study presents a robust and reproducible methodology for digital musealization and management, focusing, as future overall objectives, towards a greater merging between the HBIM approach and XR technologies, also facilitated by training new professional figures with more in-depth digital skills

    Il presente lavoro costituisce uno step significativo e dagli aspetti innovativi nel testing di soluzioni per l’interazione con facsimili digitali, in particolare in esso vengono indagate le potenzialità delle applicazioni di Mixed reality per la comunicazione di progetti di modellazione tridimensionale. In questo caso la ricostruzione 3D del complesso Rocca-Torrione di F. di Giorgio Martini a Cagli è il caso studio ideale su cui sperimentare i principi delle carte di riferimento in ambito di Digital Cultural Heritage (DCH) ma anche riflettere su metodologie di narrazione del patrimonio materiale e immateriale. Inoltre tali studi, unitamente alle reti territoriali locali e regionali, costituiscono la precondizione per la costituzione di un Centro Studi su Francesco di Giorgio Martini che avrà sede a Cagli

    The famous painting “Sposalizio della Vergine” by Giovanni Francesco Barbieri, also known as Guercino, was realized in 1649 for the Mariotti family altar in the San Paterniano church in Fano (Italy). During the XIX century it was stolen but then fortunately recovered, even if in extremely poor conditions. The painting was restored and acquired by Fondazione Cassa di Risparmio di Fano in 1969. Through the application of non-invasive techniques, it was evaluated its conservation state and a mapping of the pictorial reintegrations was achieved. Multispectral imaging analyses (Infrared Refl ectography, Infrared False Color photography, Ultraviolet Fluorescence, Raking Light) provided also information on the painting technique that is coherent with the Guercino modus operandi already reported in literature. For example, the underdrawing is absent as in other Guercino’s paintings. Moreover, raking light images and macro-photography revealed the characteristics of the original canvas support, no more visible due to the lining intervention carried out in a previous restoration. Lastly, as far as concern the analysis of the materials, XRF (X-ray Fluorescence spectroscopy), FT-IR (Fourier Transform Infrared Spectroscopy) and FORS (UVVIS- NIR Fiber Optics Refl ectance Spectroscopy) were combined to characterize the artist’s palette. As blue pigment, the precious lapis lazuli was extensively used by the artist, and this suggested a very high price for the execution of the painting. Alongside the non-invasive diagnostic analyses, a 3D digitalization of the altarpiece was exploited to document its whole structure but also fi nest details, providing an effective medium to enable virtual ways to enjoy the painting. Indeed, thanks to a digital facsimile, it is possible to develop solutions able to generate different relationships between the work of art and its observer, letting him know its contents, its techniques, its expressive and aesthetic meaning.
