The Effect of Virtual Learning as a Result of COVID-19
In the early spring of 2020, the United States began to experience the effects of the question-inspiring, novel COVID-19 viral illness, with the onset of massive business, school, and entertainment closures, initiating a shift towards virtual learning, communication, and professional interactions. This paper is intended to identify the common experiences of two nursing students throughout the COVID-19 pandemic by utilizing a personal narrative, or autoethnographic approach, coupled with reviews of previously published literature relating to shared themes of the authors, including the rapid changes to an online environment from traditional classroom settings, the stressors that were experienced as a result of that alteration, and the personal development and growth which accompanied the transitions of this time period. Noting an increased resilience to rapid, unexpected changes affecting daily life, as well as enhanced knowledge of self-motivation and organization, the authors have experienced a number of constructive takeaways that will likely be implemented in the many years ahead. While the COVID-19 pandemic led to a variety of uncomfortable and detrimental outcomes and an array of losses which will not be forgotten, positive consequences can still be observed, leading to an improved future
Complications among colorectal cancer survivors: SF-6D preference-weighted quality of life scores
Background
Societal preference-weighted health-related quality of life (HRQOL) scores enable comparing multi-dimensional health states across diseases and treatments for research and policy.
Objective
To assess the effects of living with a permanent intestinal stoma, compared to a major bowel resection, among colorectal cancer (CRC) survivors.
Research Design
Cross-sectional multivariate linear regression analysis to explain preference-weighted HRQOL scores.
Subjects
Six-hundred-forty CRC survivors (≥5 years) from three group-model HMOs; ostomates and non-ostomates with colorectal resections for CRC were matched on gender, age (±5 years), time since diagnosis, and tumor site (rectum vs. colon).
Measures
SF-6D scoring system applied to Medical Outcomes Study Short Form-36 version 2 (SF-36v2); City of Hope Quality of Life-Ostomy (mCOH-QOL-O); Charlson-Deyo comorbidity index.
Methods
Survey of CRC survivors linked to respondents’ clinical data extracted from HMO files.
Results
Response rate was 52%. Ostomates and non-ostomates had similar sociodemographic characteristics. Mean SF-6D score was 0.69 for ostomates, compared to 0.73 for non-ostomates (p <.001), but other factors explained this difference. Complications of initial cancer surgery, and prior-year comorbidity burden and hospital use were negatively associated with SF-6D scores, while household income was positively associated.
Conclusions
CRC survivors’ SF-6D scores were not associated with living with a permanent ostomy after other factors were taken into account. Surgical complications, comorbidities, and metastatic disease lowered the preference-weighted HRQOL of CRC survivors with and without ostomies. Further research to understand and reduce late complications from CRC surgeries as well as associated depression is warranted
Clinical Sequencing Exploratory Research Consortium: Accelerating Evidence-Based Practice of Genomic Medicine
Despite rapid technical progress and demonstrable effectiveness for some types of diagnosis and therapy, much remains to be learned about clinical genome and exome sequencing (CGES) and its role within the practice of medicine. The Clinical Sequencing Exploratory Research (CSER) consortium includes 18 extramural research projects, one National Human Genome Research Institute (NHGRI) intramural project, and a coordinating center funded by the NHGRI and National Cancer Institute. The consortium is exploring analytic and clinical validity and utility, as well as the ethical, legal, and social implications of sequencing via multidisciplinary approaches; it has thus far recruited 5,577 participants across a spectrum of symptomatic and healthy children and adults by utilizing both germline and cancer sequencing. The CSER consortium is analyzing data and creating publically available procedures and tools related to participant preferences and consent, variant classification, disclosure and management of primary and secondary findings, health outcomes, and integration with electronic health records. Future research directions will refine measures of clinical utility of CGES in both germline and somatic testing, evaluate the use of CGES for screening in healthy individuals, explore the penetrance of pathogenic variants through extensive phenotyping, reduce discordances in public databases of genes and variants, examine social and ethnic disparities in the provision of genomics services, explore regulatory issues, and estimate the value and downstream costs of sequencing. The CSER consortium has established a shared community of research sites by using diverse approaches to pursue the evidence-based development of best practices in genomic medicine
Determinants of Adoption of Fusarium Head Blight Management Techniques in Wheat
Fusarium head blight (FHB), commonly known as scab, is a disease affecting all classes of wheat and other small grains. We detect, and estimate the effect of, factors on the choice of number of technologies wheat farmers in North Dakota and Minnesota adopt for scab management. Farm characteristics and availability of technical information about best scab management practices are important variables. Availability of technical information and land ownership increase the odds producers concentrate scab management programs into fewer techniques. Increased wheat acreage on owned land has the opposite effect
