    The Right to a Safe and Healthy Birth

    The right to a safe and healthy birth is included in the declaration of human rights- the intrinsic allowances that humans are obligated to be protected by on a global scale. These rights, however, for some pregnant women have been submersed by lack of transportation, education and skilled birth attendants. Financial constraints and difficulty in obtaining adequate healthcare are also issues of concern. A review of literature pertaining to three different countries, the United States, China and India will be examined in light of safe and healthy birthing techniques for women worldwide. These countries were chosen due to their diverse injustice issues as it pertains to birthing rights. Education and advocacy efforts in relation to reproductive rights on a global scale will be discussed. Review of the literature not only reveals grave injustices for women and children but it also illuminates ways in which individuals can get involved to help promote the right to a safe and healthy birth. Several movements will be highlighted in order to provide the audience with practical advocacy, education, and relief effort implementations

    Physicianā€“Patient Communication About Sexual Functioning in Patients with Multiple Sclerosis

    Sexual dysfunction is quite common among individuals with multiple sclerosis (MS); however, severity of dysfunction alone does not account for the tremendous variation in sexual satisfaction across individuals living with MS. Individual characteristics, relationships with intimate partners, and environmental factors all likely contribute to the multidimensional experience of sexual satisfaction. Health care provider variables, including how one communicates with providers about sexual concerns, may also be influential. The purpose of this study was to examine factors that are associated with patientsā€™ sex-related communications with their MS physicians and to overall patient sexual satisfaction. Individuals in an MS clinic (n = 73) completed a survey packet which included measures of physical and mental health, sexual dysfunction, sexual satisfaction, sex communication, health care provider relationships, and health care satisfaction. Findings suggest that while more than half of patients with MS reported experiencing sexual dysfunction, only a third of patients indicated addressing their sexual concerns with their physician during the past year. Interestingly, the frequency of communication about sexual concerns was associated with satisfaction with physician variables, whereas selfefficacy for these interactions was associated with emotional health variables. These results indicate that when considering interventions to increase confidence for communication and frequency of communication that differing factors may be taken into account

    Identification of Cellular Genes Targeted by KSHV-Encoded MicroRNAs

    MicroRNAs (miRNAs) are 19 to 23 nucleotideā€“long RNAs that post-transcriptionally regulate gene expression. Human cells express several hundred miRNAs which regulate important biological pathways such as development, proliferation, and apoptosis. Recently, 12 miRNA genes have been identified within the genome of Kaposi sarcomaā€“associated herpesvirus; however, their functions are still unknown. To identify host cellular genes that may be targeted by these novel viral regulators, we performed gene expression profiling in cells stably expressing KSHV-encoded miRNAs. Data analysis revealed a set of 81 genes whose expression was significantly changed in the presence of miRNAs. While the majority of changes were below 2-fold, eight genes were down-regulated between 4- and 20-fold. We confirmed miRNA-dependent regulation for three of these genes and found that protein levels of thrombospondin 1 (THBS1) were decreased >10-fold. THBS1 has previously been reported to be down-regulated in Kaposi sarcoma lesions and has known activity as a strong tumor suppressor and anti-angiogenic factor, exerting its anti-angiogenic effect in part by activating the latent form of TGF-Ī². We show that reduced THBS1 expression in the presence of viral miRNAs translates into decreased TGF-Ī² activity. These data suggest that KSHV-encoded miRNAs may contribute directly to pathogenesis by down-regulation of THBS1, a major regulator of cell adhesion, migration, and angiogenesis

    Trajectories of depressive symptoms and adult educational and employment outcomes

    Background Depressive symptoms show different trajectories throughout childhood and adolescence that may have different consequences for adult outcomes. Aims To examine trajectories of childhood depressive symptoms and their association with education and employment outcomes in early adulthood. Method We estimated latent trajectory classes from participants with repeated measures of self-reported depressive symptoms between 11 and 24 years of age and examined their association with two distal outcomes: university degree and those not in employment, education or training at age 24. Results Our main analyses (n = 9399) yielded five heterogenous trajectories of depressive symptoms. The largest group found (70.5% of participants) had a stable trajectory of low depressive symptoms (stableā€“low). The other four groups had symptom profiles that reached full-threshold levels at different developmental stages and for different durations. We identified the following groups: childhoodā€“limited (5.1% of participants) with full-threshold symptoms at ages 11ā€“13; childhoodā€“persistent (3.5%) with full-threshold symptoms at ages 13ā€“24; adolescent onset (9.4%) with full-threshold symptoms at ages 17ā€“19; and early-adult onset (11.6%) with full-threshold symptoms at ages 22ā€“24. Relative to the majority ā€˜stableā€“lowā€™ group, the other four groups all exhibited higher risks of one or both adult outcomes. Conclusions Accurate identification of depressive symptom trajectories requires data spanning the period from early adolescence to early adulthood. Consideration of changes in, as well as levels of, depressive symptoms could improve the targeting of preventative interventions in early-to-mid adolescence. </jats:sec

    Paraneoplastic thrombocytosis in ovarian cancer

    &lt;p&gt;Background: The mechanisms of paraneoplastic thrombocytosis in ovarian cancer and the role that platelets play in abetting cancer growth are unclear.&lt;/p&gt; &lt;p&gt;Methods: We analyzed clinical data on 619 patients with epithelial ovarian cancer to test associations between platelet counts and disease outcome. Human samples and mouse models of epithelial ovarian cancer were used to explore the underlying mechanisms of paraneoplastic thrombocytosis. The effects of platelets on tumor growth and angiogenesis were ascertained.&lt;/p&gt; &lt;p&gt;Results: Thrombocytosis was significantly associated with advanced disease and shortened survival. Plasma levels of thrombopoietin and interleukin-6 were significantly elevated in patients who had thrombocytosis as compared with those who did not. In mouse models, increased hepatic thrombopoietin synthesis in response to tumor-derived interleukin-6 was an underlying mechanism of paraneoplastic thrombocytosis. Tumorderived interleukin-6 and hepatic thrombopoietin were also linked to thrombocytosis in patients. Silencing thrombopoietin and interleukin-6 abrogated thrombocytosis in tumor-bearing mice. Antiā€“interleukin-6 antibody treatment significantly reduced platelet counts in tumor-bearing mice and in patients with epithelial ovarian cancer. In addition, neutralizing interleukin-6 significantly enhanced the therapeutic efficacy of paclitaxel in mouse models of epithelial ovarian cancer. The use of an antiplatelet antibody to halve platelet counts in tumor-bearing mice significantly reduced tumor growth and angiogenesis.&lt;/p&gt; &lt;p&gt;Conclusions: These findings support the existence of a paracrine circuit wherein increased production of thrombopoietic cytokines in tumor and host tissue leads to paraneoplastic thrombocytosis, which fuels tumor growth. We speculate that countering paraneoplastic thrombocytosis either directly or indirectly by targeting these cytokines may have therapeutic potential. &lt;/p&gt

    Ack1 Mediated AKT/PKB Tyrosine 176 Phosphorylation Regulates Its Activation

    The AKT/PKB kinase is a key signaling component of one of the most frequently activated pathways in cancer and is a major target of cancer drug development. Most studies have focused on its activation by Receptor Tyrosine Kinase (RTK) mediated Phosphatidylinositol-3-OH kinase (PI3K) activation or loss of Phosphatase and Tensin homolog (PTEN). We have uncovered that growth factors binding to RTKs lead to activation of a non-receptor tyrosine kinase, Ack1 (also known as ACK or TNK2), which directly phosphorylates AKT at an evolutionarily conserved tyrosine 176 in the kinase domain. Tyr176-phosphorylated AKT localizes to the plasma membrane and promotes Thr308/Ser473-phosphorylation leading to AKT activation. Mice expressing activated Ack1 specifically in the prostate exhibit AKT Tyr176-phosphorylation and develop murine prostatic intraepithelial neoplasia (mPINs). Further, expression levels of Tyr176-phosphorylated-AKT and Tyr284-phosphorylated-Ack1 were positively correlated with the severity of disease progression, and inversely correlated with the survival of breast cancer patients.
