    Time series analysis as input for clinical predictive modeling: Modeling cardiac arrest in a pediatric ICU

    BACKGROUND: Thousands of children experience cardiac arrest events every year in pediatric intensive care units. Most of these children die. Cardiac arrest prediction tools are used as part of medical emergency team evaluations to identify patients in standard hospital beds that are at high risk for cardiac arrest. There are no models to predict cardiac arrest in pediatric intensive care units though, where the risk of an arrest is 10 times higher than for standard hospital beds. Current tools are based on a multivariable approach that does not characterize deterioration, which often precedes cardiac arrests. Characterizing deterioration requires a time series approach. The purpose of this study is to propose a method that will allow for time series data to be used in clinical prediction models. Successful implementation of these methods has the potential to bring arrest prediction to the pediatric intensive care environment, possibly allowing for interventions that can save lives and prevent disabilities. METHODS: We reviewed prediction models from nonclinical domains that employ time series data, and identified the steps that are necessary for building predictive models using time series clinical data. We illustrate the method by applying it to the specific case of building a predictive model for cardiac arrest in a pediatric intensive care unit. RESULTS: Time course analysis studies from genomic analysis provided a modeling template that was compatible with the steps required to develop a model from clinical time series data. The steps include: 1) selecting candidate variables; 2) specifying measurement parameters; 3) defining data format; 4) defining time window duration and resolution; 5) calculating latent variables for candidate variables not directly measured; 6) calculating time series features as latent variables; 7) creating data subsets to measure model performance effects attributable to various classes of candidate variables; 8) reducing the number of candidate features; 9) training models for various data subsets; and 10) measuring model performance characteristics in unseen data to estimate their external validity. CONCLUSIONS: We have proposed a ten step process that results in data sets that contain time series features and are suitable for predictive modeling by a number of methods. We illustrated the process through an example of cardiac arrest prediction in a pediatric intensive care setting

    Illegal Shooting is Now a Leading Cause of Death of Birds Along Power Lines in the Western USA

    Human actions, both legal and illegal, affect wildlife in many ways. Inaccurate diagnosis of cause of death undermines law enforcement, management, threat assessment, and mitigation. We found 410 dead birds collected along 196 km of power lines in four western USA states during 2019ā€“2022. We necropsied these carcasses to test conventional wisdom suggesting that electrocution is the leading cause of death of birds at electrical infrastructure. Of 175 birds with a known cause of death, 66% died from gunshot. Both raptors and corvids were more likely to die from gunshot than from other causes, along both transmission and distribution lines. Past mitigation to reduce avian deaths along power lines has focused almost exclusively on reducing electrocutions or collisions. Our work suggests that, although electrocution and collision remain important, addressing illegal shooting now may have greater relevance for avian conservation

    Evaluation of range of motion restriction within the hip joint

    In Total Hip Arthroplasty, determining the impingement free range of motion requirement is a complex task. This is because in the native hip, motion is restricted by both impingement as well as soft tissue restraint. The aim of this study is to determine a range of motion benchmark which can identify motions which are at risk from impingement and those which are constrained due to soft tissue. Two experimental methodologies were used to determine motions which were limited by impingement and those motions which were limited by both impingement and soft tissue restraint. By comparing these two experimental results, motions which were limited by impingement were able to be separated from those motions which were limited by soft tissue restraint. The results show motions in extension as well as flexion combined with adduction are limited by soft tissue restraint. Motions in flexion, flexion combined with abduction and adduction are at risk from osseous impingement. Consequently, these motions represent where the maximum likely damage will occur in femoroacetabular impingement or at most risk of prosthetic impingement in Total Hip Arthroplasty

    The SSTARS (STeroids and Stents Against Re-Stenosis) Trial : different stent alloys and the use of peri-procedural oral corticosteroids to prevent in-segment restenosis after percutaneous coronary intervention

    Background Stent design and technological modifications to allow for anti-proliferative drug elution influence restenosis rates following percutaneous coronary intervention (PCI). We aimed to investigate whether peri-procedural administration of corticosteroids or the use of thinner strut cobalt alloy stents would reduce rates of binary angiographic restenosis (BAR) after PCI. Methods This was a two centre, mixed single and double blinded, randomised controlled trial using a factorial design. We compared (a) the use of prednisolone to placebo, starting at least six hours pre-PCI and continued for 28ā€…days post-PCI, and (b) cobalt chromium (CoCr) to stainless steel (SS) alloy stents, in patients admitted for PCI. The primary end-point was BAR at six months. Results 315 patients (359 lesions) were randomly assigned to either placebo (nā€…=ā€…145) or prednisolone (nā€…=ā€…170) and SS (nā€…=ā€…160) or CoCr (nā€…=ā€…160). The majority (58%) presented with an ACS, 11% had diabetes and 287 (91%) completed angiographic follow up. BAR occurred in 26 cases in the placebo group (19.7%) versus 31 cases in the prednisolone group (20.0%) respectively, pā€…=ā€…1.00. For the comparison between SS and CoCr stents, BAR occurred in 32 patients (21.6%) versus 25 patients (18.0%) respectively, pā€…=ā€…0.46. Conclusion Our study showed that treating patients with a moderately high dose of prednisolone for 28ā€…days following PCI with BMS did not reduce the incidence of BAR. In addition, we showed no significant reduction in 6ā€…month restenosis rates with stents composed of CoCr alloy compared to SS
