Complex regional pain syndrome with associated chest wall dystonia: a case report
Patients with complex regional pain syndrome (CRPS) often suffer from an array of associated movement disorders, including dystonia of an affected limb. We present a case of a patient with long standing CRPS after a brachial plexus injury, who after displaying several features of the movement disorder previously, developed painful dystonia of chest wall musculature. Detailed neurologic examination found palpable sustained contractions of the pectoral and intercostal muscles in addition to surface allodynia. Needle electromyography of the intercostal and paraspinal muscles supported the diagnosis of dystonia. In addition, pulmonary function testing showed both restrictive and obstructive features in the absence of a clear cardiopulmonary etiology. Treatment was initiated with intrathecal baclofen and the patient had symptomatic relief and improvement of dystonia. This case illustrates a novel form of the movement disorder associated with CRPS with response to intrathecal baclofen treatment

The Willingness of U.S. Emergency Medical Technicians to Respond to Terrorist Incidents
A nationally representative sample of basic and paramedic emergency medical service providers in the United States was surveyed to assess their willingness to respond to terrorist incidents. EMTs were appreciably (9-13%) less willing than able to respond to such potential terrorist-related incidents as smallpox outbreaks, chemical attacks, or radioactive dirty bombs (p < 0.0001). EMTs who had received terrorism-related continuing medical education within the previous 2 years were twice as likely (OR = 1.9, 95% CI 1.9, 2.0) to be willing to respond to a potential smallpox dissemination incident as those who indicated that they had not received such training. Timely and appropriate training, attention to interpersonal concerns, and instilling a sense of duty may increase first medical provider response rates

Preparing Health Professions Students for Terrorism, Disaster, and Public Health Emergencies: Core Competencies
The recent increased threat of terrorism, coupled with the ever-present dangers posed by natural disasters and public health emergencies, clearly support the need to incorporate bioterrorism preparedness and emergency response material into the curricula of every health professions school in the nation. A main barrier to health care preparedness in this country is a lack of coordination across the spectrum of public health and health care communities and disciplines. Ensuring a unified and coordinated approach to preparedness requires that benchmarks and standards be consistent across health care disciplines and public health, with the most basic level being education of health professions students. Educational competencies establish the foundation that enables graduates to meet occupational competencies. However, educational needs for students differ from the needs of practitioners. In addition, there must be a clear connection between departments of public health and all other health care entities to ensure proper preparedness. The authors describe both a process and a list of core competencies for teaching emergency preparedness to students in the health care professions, developed in 2003 and 2004 by a team of experts from the four health professions schools of Columbia University in New York City. These competencies are directly applicable to medical, dental, nursing, and public health students. They can also easily be adapted to other health care disciplines, so long as differences in levels of proficiency and the need for clinical competency are taken into consideration
An Analysis Of Brand Equity Determinants: Gross Profit, Advertising, Research, And Development
The topic of brand equity continues to be of great importance to private sector firms in the creation and development of both product and company brand strategy. This study analyzes the relationship of different variables and brand equity with the purpose of providing useful insight into brand management and advancement. This research approach is unique in its use of private sector generated measures of brand equity as the main data source. The methodology entails collecting financial information from a list of publicly traded companies evident on the annual Interbrand ranking of top brand values, then executing statistical analysis using correlation and regression. Results make evident a significant positive correlation between brand equity and gross profit, advertising expense, and research and development expense. However, findings reveal that advertising is not as important a driver of brand equity in the short-term as put forward in previous research and evidence suggesting the presence of dynamics and non-linear effects exists.

Preparing for disasters: What should you know, and when should you know it?
Three waves of terrorism have ushered in a new era in public health, safety, and health care in the United States: The February 26, 1993 explosions at New York's World Trade Center, the April 19, 1995 bombing at the Murrah Federal Building in Oklahoma City, and the September 11, 2001 attacks, along with the anthrax cases that followed. Those events have awakened federal, state, and local governments to the urgency of improving their disaster-response capabilities. The health professions have also stepped forward to delineate the roles of their members in disasters, establish minimum competencies, and develop new training programs. Physician assistants have a strong tradition of responding to crisis. In all likelihood, PAs will be among the first responders to any disaster in the United States. It is incumbent on the profession and on individual PAs to understand what their roles would be in a disaster and to prepare accordingly
Transit Detection in the MEarth Survey of Nearby M Dwarfs: Bridging the Clean-First, Search-Later Divide
In the effort to characterize the masses, radii, and atmospheres of
potentially habitable exoplanets, there is an urgent need to find examples of
such planets transiting nearby M dwarfs. The MEarth Project is an ongoing
effort to do so, as a ground-based photometric survey designed to detect
exoplanets as small as 2 Earth radii transiting mid-to-late M dwarfs within 33
pc of the Sun. Unfortunately, identifying transits of such planets in
photometric monitoring is complicated both by the intrinsic stellar variability
that is common among these stars and by the nocturnal cadence, atmospheric
variations, and instrumental systematics that often plague Earth-bound
observatories. Here we summarize the properties of MEarth data gathered so far,
and we present a new framework to detect shallow exoplanet transits in wiggly
and irregularly-spaced light curves. In contrast to previous methods that clean
trends from light curves before searching for transits, this framework assesses
the significance of individual transits simultaneously while modeling
variability, systematics, and the photometric quality of individual nights. Our
Method for Including Starspots and Systematics in the Marginalized Probability
of a Lone Eclipse (MISS MarPLE) uses a computationally efficient semi-Bayesian
approach to explore the vast probability space spanned by the many parameters
of this model, naturally incorporating the uncertainties in these parameters
into its evaluation of candidate events. We show how to combine individual
transits processed by MISS MarPLE into periodic transiting planet candidates
and compare our results to the popular Box-fitting Least Squares (BLS) method
with simulations. By applying MISS MarPLE to observations from the MEarth
Project, we demonstrate the utility of this framework for robustly assessing
the false alarm probability of transit signals in real data. [slightly

Development and Validation of Molecular Markers for \u3cem\u3ePhytophthora medicaginis\u3c/em\u3e Resistance in Lucerne
Resistance to Phytophthora medicaginis is an essential attribute to incorporate into lucerne (Medicago sativa) cultivars which are likely to be grown on heavy soils or in conditions where the soil remains excessively wet for prolonged periods. Current breeding strategies rely on recurrent phenotypic selection to maintain adequate levels of resistance in newly developed synthetic cultivars. However, little is known about the source or mechanism(s) of genetic resistance operating in the cultivar. A genetic linkage map was generated from a tetraploid M. sativa population using SSR markers anchored to existing genetic and physical maps. Large effect QTL were identified on linkage groups 2, 5, 6 and 7, each of which contributed between 11-30% of the phenotypic variation. Evaluation of the marker-trait associations in another sampling of the same population was undertaken, using a different isolate of P. medicaginis. The findings indicate that in the lucerne genotype examined in this study, a network of interactions involving at least three common loci, contribute to resistance to P. medicaginis. An alignment of the resistance loci identified in this study with those previously identified provided a framework for cataloguing the diversity of resistance loci present in lucerne, and will be used to guide future lucerne breeding efforts
Influence of longswing technique on the kinematics and key release parameters of the straddle Tkachev on uneven bars
Tkachev on uneven bars is a release and re-grasp skill performed using variations of preparatory longswing techniques; the reasons why different techniques are chosen remains unclear. This study examined kinematic and key release parameters specific to three distinct techniques with the aim of understanding the relative benefits of each. During two international artistic gymnastics competitions six arch, straddle and pike longswings preceding the straddle Tkachev were recorded using twin video cameras. Calibration and movement images were digitised and reconstructed using 3D DLT. Shoulder and hip angular kinematics, angular momentum and key release parameters were compared between techniques. In the arch longswing, the first and second hip functional phases began significantly earlier than the straddle or pike. No significant differences were established for release parameters although large effect sizes for horizontal release velocity and angular momenta about the mass centre and bar were reported between the arch and other two variants. Therefore, the arch variant may provide the opportunity to develop more complex combinations following the Tkachev. Providing insight into mechanical advantages of specific longswing techniques, and highlighting those that elicit desirable characteristics offers the potential to provide coaches with objective data on technique selection and ultimately skill development
