    Despite the lack of randomized and prospective studies the benefit of MC therapy can not be attributed merely to patient selection, since substantial increase of survival time and long term survivors are seen only in patients who have received, and responded to treatment. In the absence of factors predicting the therapeutic response, all patients with overt MC should be treated with the possible exception of these with widespread and uncontrolled systemic neoplasia. The respective weight of systemic and intraventricular chemotherapy, and radiation therapy, remains unclear. It may depend upon the stage and the nature of the primary neoplasia.     Early diagnosis of brain metastases in an unselected population of cancerous patients

    In a series of 50 unselected cases in whom brain metastasis was later demonstrated by pathological examination, early clinical signs and symptoms, as well as paraclinical abnormalities were studied. These included EEG, brain scan, angiography and analysis of CSF. Two groups of patients could be distinguished. In the first group (22 patients) brain metastasis was heralded by focal neurological symptoms. In these patients EEG, brain scan or angiography demonstrated focal lesions in 83%. In the second group (28 patients), diffuse symptoms of involvement of CNS were first observed. In these patients focal lesions were shown by the additional diagnostic procedures in only 42%. Determinations of proteins, LDH and GOT enzymes in the CSF were of limited help. Neoplastic cells were found in only 10% of examined fluids. A striking increase of lipids was observed in 20% of the CSF. This determination was not previously performed in a series of patients with brain metastasis. The physiopathological signification of changes in lipid concentration of CSF is discussed.SCOPUS: ar.jinfo:eu-repo/semantics/publishe

    Tremor (Tr) is an involuntary rhythmic movement disorder. Its causes are multifold. The physiopathogenesis is complex and only partially understood. The classification, based on clinical grounds distinguishes: 1) Physiological Tr at 8-12 Hz, postural, involving the limbs; 2) Essential Tr at 8 Hz, often familial, postural, involving the limbs; 3) Parkinsonian Tr at 4-0 Hz, present at rest, rarely isolated, often asymmetrical, involving the limbs, sometimes the jaw; 4) Cerebellar Tr at 3-4 Hz, maximal at the end of limb movements. Therapy is based on this classification. The main points for the diagnosis are age of onset, associated signs, toxic factors and heredity.SCOPUS: sh.jinfo:eu-repo/semantics/publishe
