    Histopathological evaluation of the subtotal laryngectomy specimen

    AbstractBackgroundThe goal of conservative surgical treatment of laryngeal cancer is to obtain oncological control with preservation of laryngeal function. The concept of laryngeal function preservation should be understood as the preservation of the patient's ability to breathe normally with neither tracheostomy nor aspiration, and maintaining intelligible speech. This can be achieved by a balance between two fundamental aspects, proper patient selection (based on tumour extension and preoperative laryngeal function), and an adequate histopathological analysis of the surgical specimen. Supracricoid subtotal laryngectomy is the voice conservative surgical technique that offers the best possibility of control in patients with locally advanced laryngeal cancer. The proper histopathological analysis allows staging and selecting patients for adjuvant therapy, avoiding unnecessary ones as well as designing monitoring and surveillance programs based on risk factors.ObjectiveTo highlight key points in the histopathological evaluation of the surgical specimen of a subtotal laryngectomy.ConclusionThe proper communication between the surgeon and pathologist, offering complete information on preoperative clinical evaluation and the knowledge of the key points in the evaluation of the surgical specimen (sites of tumour leakage and surgical resection margins) are fundamental parameters to achieve a proper histopathological evaluation of the surgical specimen

    Registros recientes de Avicennia bicolor (Acanthaceae) en la costa del Pacífico mexicano con notas sobre su distribución y estado de conservación

    Background. The mangrove Avicennia bicolor was first recorded in Mexico in 1946, however until now there is no basic information about its distribution supported by herbarium specimens. Goals. Three recent records of A. bicolor are provided at a new location in Mexico on the coastal wetlands of the Gulf of Tehuantepec, Chiapas; and the information about its known records on the American Pacific is summarized. Methods. Plant specimens were collected, which then, were dried, corroborated and deposited in four national herbaria. In the field, some structural attributes of the mangroves were determined such as density, basal area and height. A database and publications of species records were reviewed to preliminarily analyze their known distribution in the Pacific. Results. In Mexico, A. bicolor has been recorded only on the coastal wetlands of Mar Muerto and Los Patos-Solo Dios. The preliminary ecological diagnosis showed its presence in relatively dense stands (234 ± 86 trees ha-1) and with high structural development (basal area: 16.1 m2 ha-1, height: 13.9 m). Although its presence is apparently conspicuous in several localities of Panama and Costa Rica, data on the species in the coastal strip between the Gulf of Fonseca and the Gulf of Tehuantepec is lacking. Conclusions. The importance of these records is evident, considering the scarce information available on A. bicolor and its restricted distribution in Mexico. We consider it appropriate to increase the ecological studies in its distribution area and give it protection through its inclusion in the Official Mexican Standard (NOM-059).Antecedentes. El mangle Avicennia bicolor fue registrado por primera vez en México en 1946, sin embargo, hasta ahora se carece de información básica sobre su distribución respaldada en ejemplares de herbario. Objetivos. Aportar tres registros recientes de A. bicolor en una nueva localidad para México en humedales costeros del Golfo de Tehuantepec, Chiapas y resumir la información sobre sus registros conocidos en el Pacífico americano. Métodos. Se recolectaron ejemplares botánicos, los cuales fueron herborizados, corroborados y depositados en cuatro herbarios nacionales. En campo se determinaron algunos atributos estructurales de los manglares (densidad, área basal, altura). Se revisó una base de datos y publicaciones de registros de la especie para analizar preliminarmente su distribución conocida en el Pacífico. Resultados. En México, A. bicolor se ha registrado únicamente en los humedales costeros de Mar Muerto y Los Patos-Solo Dios. El diagnóstico ecológico preliminar demostró su presencia en rodales relativamente densos (234±86 árboles ha-1) y con elevado desarrollo estructural (área basal: 16.1 m2 ha-1; altura: 13.9 m). Aunque su presencia es apa rentemente conspicua en varias localidades de Panamá y Costa Rica, se carece de datos sobre la especie en la franja costera comprendida entre el Golfo de Fonseca y el Golfo de Tehuantepec. Conclusiones. Se evidencia la importancia de estos registros, considerando la escasa información disponible y su restringida distribución en México. Consideramos congruente incrementar los estudios ecológicos en su área de distribución y darle protección mediante su inclusión en la Norma Oficial Mexicana (NOM-059)

    Resumen Hoy en día los servicios de almacenamiento en la nube son usados de forma masiva. Con fines de protección de la información, se ofrecen niveles de seguridad basados en protocolos de seguridad informática y de la información respectivamente, para hacer frente a diversos tipos de ataques cibernéticos. Ante este escenario, surge la necesidad de desarrollar herramientas tecnológicas que coadyuven con las ya existentes en lo que concierne a la obtención de una protección adicional. En el contexto de imágenes digitales, una de estas herramientas es la técnica de ocultamiento reversible de datos en dominio cifrado, en este sentido, el presente artículo propone un par de estas técnicas que utilizan el cifrado de flujo AES y RC4 como dominios de inserción, este último combinado con mezcla caótica. Los resultados experimentales muestran su efectividad en escenarios reales de operación, garantizando la integridad de datos y al mismo tiempo no interfiere con los protocolos de seguridad de los servicios en la nube. Abstract Nowadays cloud storage services are used in a massive way. To protect the information in them and to face different types of cyber-attacks that could violate, and compromise stored data, they offer specific levels of security and information security protocols. In this scenario, there is an urgent need to develop technological tools that contribute to those one already developed and to offer additional protection. In the context of digital images, one of these tools is the reversible data hiding technique in the encrypted domain. In this article two techniques are being proposed that use stream encryption as the insertion domain, one of them combined with chaotic mixing. The experimental results are subject to real operational problems, guaranteeing the integrity of the data and at the same time without interfering with the security protocols used in the cloud services.     Collet Sicard syndrome secondary to granulomatosis with polyangiitis

    Granulomatosis with polyangiitis (GPA) is a systemic vasculitis that affects small to medium-sized blood vessels. Although uncommon, cranial nerve (CN) involvement can result in CN palsy. We present a clinical case of a female with collet Sicard syndrome (CSS), displaying left-sided CN IX, X, XI, and XII involvement, which we determined to be caused by GPA. Our patient met 7 points on the American College of Rheumatology 2018 (ACR) criteria for GPA with positive ANCA, bilateral mastoiditis, and decreased sensorineural hearing. Early identification of GPA is crucial due to systemic CN involvement, as it can lead to dysphonia, dysphagia, and other complications. Early treatment can improve the functional prognosis of patients, requiring intensive induction immunosuppression due to frequent bilateral progression and worse prognosis. Despite its infrequency, timely diagnosis is critical for better patient outcomes

    Relación entre parámetros edáficos y criterios de calidad postcosecha de frutos de Prunus persica (L.) Batsch por análisis multivariado

    This study was carried out in the main Escarcha variety peach producing area in the state of Tlaxcala, Mexico. Soil parameters were evaluated in plots with different management and some quality criteria in the fruit, to later relate them through a Principal Component Analysis (PCA). The results of the soil characterization evidenced a predominance of the sand fraction, low concentration of organic matter and total N, and few exchange sites. Yields of 10.88 to 11.66 t ha-1 were obtained. Through the PCA it was observed that the predictor variables in the case of the soil, in the main component one (CP1) were organic matter, organic carbon, total nitrogen and phosphorus; in CP2 were the apparent density, porosity, pH and the humification index. In the case of the fruit, in CP1 were weight, equatorial diameter, maturity index and number of fruits, and in CP2 were total titratable acidity and firmness. The PCA allowed identifying the soil variables that influence the quality of the fruit. The producer is recommended to increase the contents of organic matter, total N and P to obtain a greater number of fruits per tree and fruits with an adequate size, weight and maturity index in the evaluated plots. The post-harvest quality criteria showed that this variety is suitable for fresh consumption, for the preparation of preserves, compotes, jams and syrups; in addition to being classified as sweet, according to the maturity index.Este estudio se llevó a cabo en la principal zona productora de durazno variedad Escarcha, Tlaxcala, México. Se evaluaron parámetros del suelo en parcelas con diferente manejo y algunos criterios de calidad en el fruto, para posteriormente relacionarlos a través de un Análisis de Componentes Principales (ACP). Los resultados de la caracterización del suelo evidenciaron un predominio de la fracción arena, baja concentración de materia orgánica y de N total, y pocos sitios de intercambio. Se obtuvieron rendimientos de 10,88 a 11,66 t ha-1. A través de los ACP se observó que las variables predictoras en el caso del suelo, en el componente principal 1 (CP1) fueron materia orgánica, carbono orgánico, nitrógeno total y fósforo; en el CP2 la densidad aparente, porosidad, pH y el índice de humificación. En el caso del fruto, en el CP1 fue el peso, diámetro ecuatorial, índice de madurez y número de frutos y en CP2 acidez total titulable y firmeza. El ACP permitió identificar las variables del suelo que influyen en la calidad del fruto. Se recomienda al productor incrementar los contenidos de materia orgánica, N total y P para obtener mayor número de frutos por árbol y frutos con un índice de madurez, tamaño y peso adecuados en las parcelas evaluadas. Los criterios de calidad postcosecha mostraron que esta variedad es apta para su consumo en fresco, para la elaboración de conservas, compotas, mermeladas y jarabes; además de clasificarse como dulces, según el índice de madurez

    Identification of latexin by a proteomic analysis in rat normal articular cartilage

    <p>Abstract</p> <p>Background</p> <p>Osteoarthritis (OA) is characterized by degeneration of articular cartilage. Animal models of OA induced are a widely used tool in the study of the pathogenesis of disease. Several proteomic techniques for selective extraction of proteins have provided protein profiles of chondrocytes and secretory patterns in normal and osteoarthritic cartilage, including the discovery of new and promising biomarkers. In this proteomic analysis to study several proteins from rat normal articular cartilage, two-dimensional electrophoresis and mass spectrometry (MS) were used. Interestingly, latexin (LXN) was found. Using an immunohistochemical technique, it was possible to determine its localization within the chondrocytes from normal and osteoarthritic articular cartilage.</p> <p>Results</p> <p>In this study, 147 proteins were visualized, and 47 proteins were identified by MS. A significant proportion of proteins are involved in metabolic processes and energy (32%), as well as participating in different biological functions including structural organization (19%), signal transduction and molecular signaling (11%), redox homeostasis (9%), transcription and protein synthesis (6%), and transport (6%). The identified proteins were assigned to one or more subcellular compartments.</p> <p>Among the identified proteins, we found some proteins already recognized in other studies such as OA-associated proteins. Interestingly, we identified LXN, an inhibitor of mammalian carboxypeptidases, which had not been described in articular cartilage. Immunolabeling assays for LXN showed a granular distribution pattern in the cytoplasm of most chondrocytes of the middle, deep and calcified zones of normal articular cartilage as well as in subchondral bone. In osteoarthritic cartilage, LXN was observed in superficial and deep zones.</p> <p>Conclusions</p> <p>This study provides the first proteomic analysis of normal articular cartilage of rat. We identified LXN, whose location was demonstrated by immunolabeling in the chondrocytes from the middle, deep and calcified zones of normal articular cartilage, and superficial and deep zones of osteoarthritic cartilage.</p

    Consumo De Alcohol Y Tabaco Y Estilos De Vida En Trabajadores Industriale

    Introducción: En México, escasos son los estudios que abordan aspectos vinculados con la salud del trabajador de la industria. Objetivo: Determinar el consumo de alcohol, tabaco, bebidas energizantes y estilos de vida en trabajadores de una industria del sector de los alimentos. Metodología: Estudio transversal, descriptivo, realizado en 270 trabajadores elegidos por muestreo aleatorio simple, de una población de 680. Al total de la muestra se le aplicó cédula de datos sociolaborales, cuestionario “Caracterización del consumo de bebidas energizantes en trabajadores del sector industrial alimentario” y Cuestionario PEPS-I de Nola Pender que evalúa Perfil de Estilo de Vida. A 370 trabajadores se les aplicaron los Test AUDIT y Fageström. El proyecto fue evaluado y aprobado por un Comité de Investigación y un Comité de Bioética. La investigación recibió financiamiento del programa FOPER de la UAQ. Los datos se analizaron en el programa SPSS v.20, obteniendo frecuencias, promedios y moda. Resultados: De los 270 trabajadores iniciales, se obtuvo que el 60.7% es varón. Las edades van desde los 20 a los 60 años. Consumen bebidas energizantes el 23.7% de los participantes; 21.8% y 35% de los trabajadores tienen un estilo de vida muy saludable y saludable y 40.7% poco saludable. 86.2% tiene un consumo riesgoso de alcohol. Conclusión: Se aprecian múltiples áreas de oportunidad en los trabajadores. Valdría la pena volver a hacer el levantamiento de los datos ya que la información se obtuvo 3 meses previo a la pandemia, lo que permitiría determinar si existieron alteraciones en estas áreas para mejora o perjuicio. Introduction: In Mexico, there are few studies that address aspects related to the health of industry workers. &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Objective: This paper focuses on determining the consumption of alcohol, tobacco, energized drinks, and the life style of industry workers within the food sector. Methodology: A cross-sectional descriptive study was carried out and 270 workers were selected from a population of 680 through the simple random sampling technique. A socio-labor data card was utilized and Nola PEPS-I questionnaire on “Characterization of the Consumption of Energy Drinks by Workers of the Food Industry”, which evaluates Lifestyle Profile, was administered. 370 workers participated in the AUDIT test and the Fageström test. The project was evaluated and approved by a Research Committee and a Bioethics Committee. The research received funds from the UAQ FOPER program. Data was analyzed in the SPSS v.20 program in order to obtain frequencies, averages, and mode. &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Results: The results of the study reveal that 60.7% of the sample size were males within the age range of 20 to 60 years. It was found also that 23.7% of the workers consume energy drinks, 21.8% and 35% of the workers have a very healthy lifestyle, 40.7% have an unhealthy lifestyle, and 86.2% have a risky alcohol consumption. &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Conclusion: This research was carried out in many areas which availed the opportunity to interact and work with the participants. It is recommended that the data collection be replicated since the information was obtained three months before the pandemic. This will help to determine areas that are affected or require improvement

    Physical activity and risk of Metabolic Syndrome in an urban Mexican cohort

    Abstract Background In the Mexican population metabolic syndrome (MS) is highly prevalent. It is well documented that regular physical activity (PA) prevents coronary diseases, type 2 diabetes and MS. Most studies of PA have focused on moderate-vigorous leisure-time activity, because it involves higher energy expenditures, increase physical fitness, and decrease the risk of MS. However, for most people it is difficult to get a significant amount of PA from only moderately-vigorous leisure activity, so workplace activity may be an option for working populations, because, although may not be as vigorous in terms of cardio-respiratory efforts, it comprises a considerable proportion of the total daily activity with important energy expenditure. Since studies have also documented that different types and intensity of daily PA, including low-intensity, seem to confer important health benefits such as prevent MS, we sought to assess the impact of different amounts of leisure-time and workplace activities, including low-intensity level on MS prevention, in a sample of urban Mexican adults. Methods The study population consisted of 5118 employees and their relatives, aged 20 to 70 years, who were enrolled in the baseline evaluation of a cohort study. MS was assessed according to the criteria of the National Cholesterol Education Program, ATP III and physical activity with a validated self-administered questionnaire. Associations between physical activity and MS risk were assessed with multivariate logistic regression models. Results The prevalence of the components of MS in the study population were: high glucose levels 14.2%, high triglycerides 40.9%, high blood pressure 20.4%, greater than healthful waist circumference 43.2% and low-high density lipoprotein 76.9%. The prevalence of MS was 24.4%; 25.3% in men and 21.8% in women. MS risk was reduced among men (OR 0.72; 95%CI 0.57–0.95) and women (OR 0.78; 95%CI 0.64–0.94) who reported an amount of ≥30 minutes/day of leisure-time activity, and among women who reported an amount of ≥3 hours/day of workplace activity (OR 0.75; 95%CI 0.59–0.96). Conclusion Our results indicate that both leisure-time and workplace activity at different intensity levels, including low-intensity significantly reduce the risk of MS. This finding highlights the need for more recommendations regarding the specific amount and intensity of leisure-time and workplace activity needed to prevent MS
