    A resorbable device for ligation of blood vessels

    Maintaining haemostasis during surgery is vital for a successful outcome. The objectives of this thesis were to develop a resorbable device, which would enable safe ligation and less surgical stress than the conventional method. For manufacturing of the device, the resorbable polymer polydioxanone was injection moulded. The device was constructed as a flexible band attached to a self-locking mechanism. The band formed a loop around the tissue, the end of the band was inserted into the locking mechanism and was pulled through until the loop was closed. The design allowed complete closure of the loop and the device resisted ligature slip-off from renal arteries at 10 N. Tissue reactions and surgical stress responses to ligation with the device were studied in healthy dogs subjective to elective neutering. For evaluation of haemostatic efficiency the ovarian pedicles of 14 dogs were ligated bilaterally (9) or unilaterally (5). All pedicles were successfully ligated, but one device first after being further tightened. The dogs were examined by ultrasound for up to twelve months with no adverse observations recorded. In two dogs histological examinations revealed local and transient tissue reactions around the devices. Systolic blood pressure and heart rate were registered during ligation of ovarian pedicles bilaterally in nine dogs and unilaterally in five dogs using the device and in 26 dogs using conventional techniques. In 16 of the 26 dogs laparoscopic and open abdomen techniques were compared and in the remaining 10 dogs ligation of the two ovaries was compared. Plasma vasopressin was analysed in the latter study. The increase in systolic blood pressure was greater in the open abdomen group compared to the laparoscopic group and the device-group. Ligation of the first ovarian pedicle caused a greater cardiovascular response than ligation of the second pedicle. However, plasma vasopressin concentration changed in synchrony with systolic blood pressure and the combination may be useful for evaluation of surgical stress. In conclusion, a resorbable device which efficiently ligated both single vessels and ovarian pedicles was successfully constructed. Whether use of the device enables less surgical stress than a conventional ligation technique remains to be clarified

    Chromogranin A and cortisol at intraoperative repeated noxious stimuli: surgical stress in a dog model

    Objectives: Biomarkers representing sympathetic tone and the surgical stress response are measured to objectively evaluate surgical techniques and anaesthetic protocols. If a part of the intraoperative procedure is repeated on the contralateral organ, one animal may potentially serve as its own control and, if so, may minimize the problem of individual differences of the stress response to anaesthesia and surgery. This study aimed to investigate the use of chromogranin A for measurement of the intraoperative sympathetic tone. Additional aims were to investigate chromogranin A and cortisol as indicators of the intraoperative surgical stress response caused by repeated noxious stimuli in dogs subjected to ovariohysterectomy and thereby to investigate the possibility of one dog serving as its own control. Methods: Experiments were carried out on 10 dogs subjected to ovariohysterectomy. Perioperative blood samples (0-6) were collected after premedication, immediately before induction of anaesthesia (0), after induction of anaesthesia and before incision (1), before (2) and after (3) removal of the first ovary, after a 15-min pause before removal of the second ovary (4), after removal of the second ovary (5) and after closing the abdomen (6). Plasma chromogranin A and cortisol were analysed. Results: Plasma chromogranin A did not change. Plasma cortisol concentration did not change between before anaesthesia and opening of the abdomen. Plasma cortisol increased at removal of the first ovary. Cortisol did not change at removal of the second ovary but remained increased compared to initial sample. Conclusion: The results suggest chromogranin A is a poor indicator of intraoperative sympathetic tone during elective surgery in dogs. Cortisol measurement was useful for assessment of intraoperative noxious stimuli. However, at these test conditions, neither plasma chromogranin A nor plasma cortisol was useful for assessment of repeated intraoperative noxious stimuli where one dog served as its own control.

    The epidemiology of osteochondrosis in an insured Swedish dog population

    Osteochondrosis (OC) is a focal disturbance of endochondral ossification due to a failure of blood supply to the epiphyseal growth cartilage. In dogs, OC most commonly affects the shoulder joint, followed by the elbow, tarsal, and stifle joints. The condition is associated with clinical signs such as lameness and pain and the prognosis varies depending on the affected joint. Most epidemiologic studies of OC in dogs were performed over 20 years ago, and updated estimates of disease incidence are lacking. Therefore, the objectives of this study were to provide population -based estimates of the incidence rate, cause -specific mortality rate, and age at diagnosis of appendicular OC (AOC, including OC of the shoulder, elbow, stifle, and tarsal joints) and stifle and tarsal OC separately, using data from Agria Djurforsakring in Sweden (2011 -2016). Further, the study aimed to evaluate the risk of OC in subgroups divided by breed and sex and describe previous, concurrent, and subsequent diagnoses of the affected joint in dogs with stifle or tarsal joint OC. The study population included just over 600,000 dogs, of which 685 were affected by AOC. Stifle joint OC (n = 113) was more common than tarsal joint OC (n = 80). The incidence rate of AOC was 3.77 (95% confidence interval (CI): 3.49 -4.07) cases per 10,000 dog-years at risk, while the incidence rate of stifle and joint tarsal OC was 0.64 (95% CI: 0.53 -0.77) and 0.43 (95% CI: 0.34 -0.54) cases per 10,000 dog -years at risk, respectively. All breeds at increased risk of AOC were large or giant, and male dogs had an increased risk of AOC compared to female dogs (RR 1.76, 95% CI: 1.50 -2.07, p < 0.001). The median age at first diagnosis during the study period was 0.74 (0.32 -11.5) years for AOC, 2.62 (0.45 -8.82) years for stifle joint OC, and 0.73 (0.35 -7.35) years for tarsal joint OC. Of the dogs with stifle or tarsal joint OC, 30.2% and 15.0% had a previous diagnosis of stifle/tarsal joint pain or other unspecific clinical signs, respectively, and 13.8% of the dogs with stifle joint OC suffered subsequent cruciate ligament rupture. Osteochondrosis was the most common reason for euthanasia in the affected dogs. In total, 77 dogs were euthanised due to AOC during the study period

    The epidemiology of patellar luxation in an insured Swedish dog population

    Patellar luxation (PL) is a common orthopaedic condition in dogs. This study aimed to evaluate the incidence and cause-specific mortality rate, age at diagnosis, and risk factors for medial PL (MPL), lateral PL (LPL), and bidirectional PL (BPL). Other diagnoses in dogs with PL were also explored. The study population included just over 600,000 dogs insured by Agria Pet Insurance in Sweden (2011–2016). There were 2726 dogs with PL. Medial patellar luxation affected 90 % of the dogs with PL, followed by LPL (5.9 %), BPL (2.4 %), and unspecified PL (1.6 %). The median age at first diagnosis during the study period was 2.8 years for MPL, 2.7 years for LPL, and 1.5 years for BPL. In total, 168 (6.2 %) of the dogs with PL had cruciate ligament rupture. There were substantial breed-specific differences in the risk of PL: almost all breeds at increased risk of MPL were small-sized, while several of the breeds at increased risk of LPL were large-sized.     Short term effect of ovariohysterectomy on urine serotonin, cortisol, testosterone and progesterone in bitches

    Objective This study aimed to investigate the short-term effect of ovariohysterectomy on urine levels of serotonin and its relation to levels of cortisol, testosterone and progesterone in female dogs. Seven bitches were studied before surgical ovariohysterectomy and then once a week during 4 weeks. Spontaneously voided urine samples were collected and concentration ratios of hormone/creatinine in urine were analysed. Results The bitches had significantly lower levels of cortisol, testosterone, and progesterone 1 week after ovariohysterectomy compared with before and the levels stayed low throughout the study (P ≤ 0.05). Interestingly, serotonin levels tended to increase 4 weeks after surgery (P = 0.08). A positive correlation between cortisol and progesterone was found before and after surgery. After surgery, serotonin was positively correlated with cortisol and progesterone (P ≤ 0.05)

    Complications of canine tonsillectomy by clamping technique combined with monopolar electrosurgery – a retrospective study of 39 cases

    BackgroundCanine tonsillectomy is performed due to acute or chronic tonsillitis, neoplasia, trauma or occasionally brachycephalic obstructive airway syndrome. Several tonsillectomy techniques are used but information about surgical complications is scarce. This retrospective study of patient records at the University Animal Hospital aimed to investigate complications related to canine tonsillectomy performed by 20-min clamping combined with monopolar electrosurgery.Inclusion criteria were bilateral tonsillectomy performed with “20-min clamping technique combined with monopolar electrosurgery without suture or ligation”. Exclusion criteria were unilateral tonsillectomy, tonsillar neoplasia, additional surgical procedures other than tonsillectomy, cases where sutures were used initially, and cases where unspecified or other methods of tonsillectomy were used. The search of the patient records of the University Animal Hospital included a 10-year period. Complications that required additional anaesthesia were defined as major complications. Minor complications were handled during surgery or after surgery without surgical intervention.ResultsOf 39 dogs that fulfilled the inclusion criteria, 11 dogs had complications and out of those 1 dog had two complications. Altogether, of the 12 complications, 2 were classified as major complications and 10 as minor.The most frequent complication was bleeding from the surgical site, in total 11 incidences; 10 dogs had an incidence of bleeding and out of those, 1 dog bled twice, both during and after surgery. Of these 10 dogs that bled, seven incidences of bleeding occurred during surgery and four incidences occurred after surgery. The two dogs with major complications were re-anaesthetized due to bleeding after surgery. No lethal complications occurred and all dogs survived to discharge.ConclusionsBleeding during and after surgery was a common complication in dogs after bilateral tonsillectomy using “20-min clamping technique combined with monopolar electrocautery”. Revision intervention was often needed, sometimes urgently. Although no comparison was made with another technique, the studied technique should be used with caution

    A preliminary investigation of the subcutaneous tissue reaction to a 3D printed polydioxanone device in horses

    BackgroundA 3D printed self-locking device made of polydioxanone (PDO) was developed to facilitate a standardized ligation technique. The subcutaneous tissue reaction to the device was evaluated after implantation in ten horses of mixed age, sex and breed and compared to loops of poly(lactic-co-glycolic acid) (PLGA). In two of the horses, the implants were removed before closing the skin. The appearance of the implants and surrounding tissue was followed over time using ultrasonography. Implants were removed after 10 and 27 (± 1) days for histologic examination.ResultsOn macroscopic inspection at day 10, the PDO-device was fragmented and the surrounding tissue was oedematous. On ultrasonographic examination, the device was seen as a hyperechoic structure with strong acoustic shadowing that could be detected 4 months post-implantation, but not at 7 months. Histology revealed a transient granulomatous inflammation, i.e., a foreign body reaction, which surrounded both PDO and PLGA implants. The type and intensity of the inflammation varied between individuals and tissue category.ConclusionsThe 3D printed PDO-device caused a transient inflammatory reaction in the subcutaneous tissue and complete resorption occurred between 4 and 7 months. Considering the intended use as a ligation device the early fragmentation warrants further adjustments of both material and the 3D printing process before the device can be used in a clinical setting

    Risk factors for severe postoperative complications in dogs with cranial cruciate ligament disease-A survival analysis

    Objective: To analyze the effect of surgical technique and other risk factors on severe postoperative complications in dogs with cranial cruciate ligament disease (CCLD). Materials and Methods: A cohort study of 255 dogs (287 stifles) surgically treated for CCLD at two veterinary university hospitals (2011 & ndash;2016) was performed. The electronic medical records were reviewed and dog owners and referring veterinarians contacted for additional information. The complications were classified as minor, major and catastrophic, where major and catastrophic were considered severe. A multivariable Cox proportional hazards model was applied to assess risk factors for severe postoperative complications.Results: Three surgical techniques were used; lateral fabellotibial suture (LFS, 141 stifles), tibial plateau leveling osteotomy (TPLO, 77 stifles) and tibial tuberosity advancement (TTA, 69 stifles). The most common severe postoperative complications were surgical site infections or complications related to the surgical implant. Severe postoperative complications occurred in 31 % of the stifles treated with TPLO, 22 % of the stifles treated with LFS and 25 % of the stifles treated with TTA. The multivariable Cox proportional hazards model identified surgical technique (p = 0.0258) as a risk factor for severe postoperative complications; TPLO had a significantly lower hazard than LFS (hazard ratio (HR) = 0.37, p = 0.007) when controlling for body weight and age, which also were identified as risk factors (HR = 1.05, p < 0.001 and HR = 0.91, p = 0.047, respectively).Conclusion and Clinical relevance: Although TPLO procedures had the highest occurrence of severe postoperative complications, the hazard was lower than for LFS after adjusting for body weight and age. This implies that it is important to consider potential effect-modifiers when comparing postoperative complications after CCLD surgery
