Parents' knowledge and behaviour concerning sunning their babies; a cross-sectional, descriptive study
BACKGROUND: For centuries, sunlight has been used for therapeutic purposes. Parents still sun their infants to treat neonatal jaundice, nappy rash or mostly to supply vitamin D for bone development as a consequence of health beliefs. In this study we aimed to assess knowledge and behaviour of parents about benefits of sunlight and sun protection. METHODS: In this study, parents attending to governmental primary healthcare units for their children's routine vaccinations, upon their informed consent, were consecutively enrolled during one month. Data were collected by a semi-structured questionnaire. RESULTS: The mean age of 118 enrolled parents and their babies were 27.9 ± 6.5 years and 8.3 ± 5.8 months, respectively. Most of the participants were mothers (93.2%), housewives (81.4%) with an educational level of ≥6 years (71.2%). Sunlight was considered beneficial for bone development (86.4%), diaper rash (5.9%) and neonatal jaundice (12.7%). In case of neonatal jaundice 72.0% of the participants reported that they would consult a physician. Most of the participants (82.2%) were sunning their babies outdoors. Nearly half (49.7%) of them got this information from medical staff. Fifty two percent of the parents were sunning their babies before 10–11 a.m. and/or after 3 p.m. Only 13.6% of parents reported using sunscreen for their babies, and the majority of them were using sun protecting factor ≥ 15. One forth of the sunscreen users was using sunscreen according to their physicians' advice. CONCLUSION: Most of the participants were aware of the benefits of sunlight; especially for bone development. However they were displaying inappropriate behaviour while sunning their babies for health reasons. More education should be given to parents about the danger of sunlight at primary health care units while advising to sun their babies, if any
The association of metabolic syndrome components with anthropometric measurements

All rights reserved.Aim: This study aims to research the association of metabolic syndrome components with anthropometric measurements like arm circumference, neck circumference, hip circumference, waist-hip ratio, and waist-to-height ratio, which are applied rarely. Methods: This cross-sectional study was conducted with 292 patients in November-December 2019. The arm circumference, neck circumference, waist circumference, hip circumference, waist-hip ratio, waist-to-height ratio, and body mass index measurements of the patients were made. Glucose, high-density lipoprotein, low-density lipoprotein, systolic, and diastolic blood pressure measurements were also recorded. The association between metabolic syndrome components and anthropometric measurements was analyzed. Results: Metabolic syndrome is diagnosed in 32.8% of the participants. According to body mass index, 18.6% of the patients were normal, 34.2% were overweight, and 47.2% were obese. There was a significant difference between the patients with and without metabolic syndrome in terms of laboratory parameters, blood pressure values, and anthropometric measurements. In the diagnosis of metabolic syndrome, optimal cut-off values for arm circumference, neck circumference, hip circumference, waist-hip ratio, and waist-to-height ratio were determined as 31.75 (AUC=0.703), 34.85 (AUC=0.763), 113.75 (AUC=0.757), 0.90 (AUC=0.701), 0.61 (AUC=0.769) for females while they were 35.75 (AUC=0.573), 39.75 (AUC=0.795), 111.5 (AUC=0.607), 0.96 (AUC=0.888), 0.61 (AUC=0.888) for males respectively. Conclusion: A significant correlation was detected between arm circumference, neck circumference, hip circumference, waist-hip ratio, and waist-to-height ratio, and metabolic syndrome components. However, low-density lipoprotein was not correlated with neck circumference and high-density lipoprotein with waist-to-height ratio

Differences between rural and urban primary care units in Turkey: Implications on residents′ training
Context: Family practice training takes place at primary care based training centers linked to Education and Research State Hospitals in Turkey. There is a discussion if these units are adequate to train primary care staff and if the patients of these units reflect the applicants of primary care. Aims: The aim of our study is to investigate the demographic characteristics, the effect of distance on primary care utilization, and most common diagnosis of the patients who applied to two different outpatient clinics: One urban and one rural. Settings and Design: Study was conducted from the electronic health records of the patients applied to outpatient clinics of Ankara Diskapi Yildirim Beyazit Training and Research Hospital Department of Family Medicine between 1 January and 31 December 2009. Results: Total number of patients applied to both of the outpatient clinics was 34,632 [urban clinic: 16.506 (47.7%), rural clinic: 18.126 (52.3%)]. Leading three diagnoses were upper respiratory tract infection (URTI), general medical examination (GME), and hypertension (HT) in the most common 10 diagnosis. Conclusion: In our study, the rural outpatient clinic is regarded as a primary care unit in the neighborhood of living area and the urban clinic as close to working environment. We found statistically meaningful differences in most common diagnosis, gender, age, and consultation time between the rural and urban clinics. According to our results, family practitioners′ field training should take place at different primary care units according to sociodemographic characteristics of each country
Evaluation of Blood Pressure Control Levels and Treatment Compliances of Hypertensive Patients
Purpose: The aim of the study is to evaluate the knowledge of patients about hypertension (HT), compliance with lifestyle changes and to determine their blood pressure levels under antihypertensive therapy. Method: Hypertensive patients that applied to Family medicine outpatient clinics of Diskapi Yildirim Beyazit Education and Research Hospital for the first time, in 2012 (between February 1 to April 30), were included in our cross-sectional study. Patients are evaluated primarily for the control and the factors that can affect high blood pressure and the rate on achieving treatment goals. Results: Three hundred and forty patients were included in the study. The mean age of the patients was 60.5±10.7 years and 222 of them (65.3%) were female. The blood pressure was not under control in 108 (31.8%) patients. The mean body weight of male patients was statistically higher than females (p=0.015). The number of the patients that knew the normal blood pressure value was 249 (73.2%). Among those 155 (62.2%) were women and 179 (71.9%) were younger than 65 years of age (p=0.0001). Forty one patients (12.1%) were still smoking, 46 (13.5%) patients were exercising regularly and 32 (9.4%) were consuming regular diet. Blood pressure was better controlled in the group that was exercising regularly (p=0.001). The rate of male patients that were on regular exercise and diet, were higher than females (p=0.09). Only 86 patients (25.3%) knew the name of their antihypertensive medication and 65 of patients (19.1%) knew the dose of their medication. Female patients knew the name of their medication better than males (p=0.002). 156 patients (45.9%) were using two kinds of antihypertensive medication. 58 patients (17.1%) were skipping doses a few times a week. Conclusion: The knowledge of hypertensive patients on their illness and their compliance on non-drug treatments were inadequate. Knowledge about Human Papilloma Virus (HPV) and HPV Vaccine at Reproductive Age in Primary Care
Purpose: The aim of this study was to identify the knowledge and awareness about human papilloma virus (HPV) and human papilloma virus (HPV) vaccine of women in reproductive age.
Material and Methods: The study covered 294 women aged between 15 and 49. A questionnaire was prepared by the researchers based on the literature review.
Results: The mean age of the participants was 30.5 +/- 8.9 years. Only 24.5% had heard about HPV infection and 28.2% had heard HPV vaccine. Of the participants, 188 (63.9%) got zero point from the knowledge questions.
Conclusion: This study indicates that the women who apply primary care units have low knowledge levels; it is apperent that personal and social education is needed. Paying importance to patient education on HPV and cervical cancer in primary care health politics will increase knowledge and awareness for HPV infection and HPV vaccination
Factors Affecting Choice of Specialty Among First-year Medical Students of Four Universities in Different Regions of Turkey
Aim To determine the factors affecting medical students’ choice of the
specialty of family medicine.
Methods The study was conducted in the period from 2004-2006 and
comprised 770 first-year medical students from Ondokuz Mayis, Karadeniz Technical, Kocaeli, and Adnan Menderes Universities, Turkey. The questionnaire included questions on demographic data and 6 “yes/no” or open-ended questions on students’ career aspirations and the specialty of family medicine. Results The response rate was 93.1% (n = 717, 54.7% male). Nearly all
students (n = 714, 99.6%) showed an intention to specialize after receiving the medical doctor degree. A total of 187 students (26.2%) showed an intention to work in primary care without specialization “for a temporary period” to “gain some experience.” Family medicine was the least preferred specialty (n = 7, 0.9%). The most important reasons for the choice of specialty were “better financial opportunities” and “prestige” (n = 219, 30.5%), followed by “personal development” (n = 149, 20.8%), “more benefits for the patient” (n = 128, 17.9%), and “wish to work in an urban area” (n = 32, 4.5%). The most preferred specialties were cardiology (n = 179, 25.0%), pediatrics (n = 121, 16.9%), ophthalmology (n = 47,6.6%), physical therapy and rehabilitation (n = 34, 4.7%), and obstetrics and gynecology (n = 32, 4.5%). Conclusion Prestige, money, and personal development are important factors in career decision-making among medical students in Turkey. This should be taken into consideration when conducting reforms at the primary level
