Characterization of the population diagnosed with dengue fever between January 2012 and August 2015 in the Central Military Hospital in Bogota.
En Colombia aproximadamente 25 millones de habitantes de áreas urbanas se encuentran en riesgo de contraer la enfermedad del dengue, presentando un aumento en la morbimortalidad, por lo que se considera un problema de salud pública con gran impacto económico.
Dentro de las poblaciones expuestas están los militares , dado que las actividades relacionas con su trabajo se ejercen, en algunos casos , en zonas endémicas para dengue volviéndose por esta razón, una causa frecuente de consulta y de hospitalización a lo largo del territorio nacional. Por este motivo, se realizo una caracterización actualizada de los sujetos con esta enfermedad que son atendidos en el Hospital Militar Central de Bogotá , con el fin de buscar, promover e identificar estrategias de prevención de la enfermedad, reconocimiento de signos de severidad buscando disminuir la tasa de complicaciones secundarias al dengue y sus variantes clínicas.
Se realizó un estudio de tipo observacional de corte transversal, creando de forma retrospectiva una base de datos con las variables establecidas, de los pacientes que asistieron a este centro de referencia entre enero de 2012 y agosto de 2015.
En total, se atendieron 341 pacientes con dengue en el periodo previamente descrito, en su mayoría hombres en promedio de 30 años, provenientes de Nilo Cundinamarca principalmente seguido por el municipio de melgar Tolima. El promedio de días de fiebre, días de duración de los síntomas y días entre inicio de síntomas y consulta fue de 5 días, encontrando como síntomas mas frecuentes de consulta el dolor abdominal, la emesis y manifestaciones de sangrado menor. Dentro de las complicaciones que se evidenciaron en estos pacientes fue elevación de las transaminasas, cifras de tensión arterial disminuidas , lesión renal aguda y coinfección. El sitio donde se trataron la mayoría de los pacientes fue hospitalización piso, sin embargo un porcentaje de los pacientes tuvieron egreso el mismo día de ingreso al HOMIC. Solo se presento un caso de muerte por dengue, y fueron solo 6 del total de los pacientes que requirieron ingreso a UCI. Dentro del estudio se describe, la caracterización de los signos de gravedad y de alarma así como las similitudes encontradas con diferentes estudios nacionales e internacionales realizados igualmente en población afectada por la fiebre del dengue.In Colombia about 25 million people in urban areas are at risk of contracting dengue disease, presenting an increase in morbidity and mortality, which is considered a public health problem with major economic impact.
Within exposed populations are the military, given that relate to their work activities are performed, in some cases, dengue becoming endemic areas for this reason, a frequent cause of consultation and hospitalization throughout the country. Therefore, an updated list of subjects with the disease who are treated at the Central Military Hospital in Bogota, in order to seek, promote and identify strategies for disease prevention, recognizing signs of severity characterization was performed seeking to reduce the rate of secondary complications of dengue and its clinical variants.
A study observational cross-sectional, retrospective creating a database with the established variables of patients who attended the referral center between January 2012 and August 2015 was performed.
In total, 341 dengue patients were treated in the previously described period, mostly men averaged 30 years old, from Cundinamarca mainly Nile followed by Tolima Melgar. The average days of fever, days of duration of symptoms and days between onset of symptoms and consultation was 5 days, finding more common symptoms like abdominal pain consultation, emesis and minor bleeding events. Among the complications that became apparent in these patients was elevated transaminases, figures decreased blood pressure, acute renal injury and coinfection. The site where most of the patients were treated was hospitalized floor, however a percentage of patients had discharge the same day of admission to HOMIC. Only one death from dengue is present, and were only 6 of the total of patients who required admission to ICU. Within the study it describes the characterization of the signs of severity and alarm as well as the similarities found with several national and international studies also made to people affected by dengue fever
Diseño de una Estrategia de Auditoría Interna para Evaluar el Sistema de Gestión de la Calidad y la Seguridad Alimentaria de Productos Alimenticios Estrella Gourmet
No aplicaCon el fin de garantizar la seguridad y la calidad de los alimentos producidos por una empresa, es necesario llevar a cabo una auditoría interna, la cual, en el caso de la industria alimentaria, debe ser desarrollada utilizando los principios proporcionados por la Norma ISO 22000:2018, los cuales, pueden aplicar medidas de seguridad alimentaria como componente de un sistema de gestión de calidad, aumentando la sensación de seguridad y confianza de todos. Las auditorías internas también son esenciales para evaluar y confirmar la eficacia de los controles y procesos internos, identificar riesgos y áreas de mejora, y garantizar el cumplimiento de las políticas y normativas, al prevenir problemas importantes, pérdidas monetarias, daños a la reputación e incluso repercusiones legales, estas auditorías fomentan la seguridad y la confianza en toda la organización. En este contexto, con el fin de evaluar el desempeño del Sistema de Gestión de Seguridad Alimentaria de Productos Alimenticios Estrella Gourmet, el desarrollo de este trabajo consistió en realizar un plan de auditoria en el marco de la norma ISO 22000:2018 a empresa en mención.In order to ensure the safety and quality of the food produced by a company, it is necessary to carry out internal audit, which, in the case of the food industry, should be developed using the principles provided by the ISO 22000:2018 standard. These principles can apply food safety measures as a component of a quality management system, increasing the sense of safety and confidence for all stakeholders. Internal audits are also essential to evaluate and confirm the effectiveness of internal controls and processes, identify risks and areas for improvement, and ensure compliance with policies and regulations. By preventing major issues, financial losses, damage to reputation, and even legal repercussions, these audits promote safety and trust throughout the organization. In this context, in order to assess the performance of the Food Safety Management System of Estrella Gourmet Food Products, the development of this work consisted of conducting an audit plan within in accordance with ISO 22000:2018 standard for the mentioned company
The immunogenetic diversity of the HLA system in Mexico correlates with underlying population genetic structure
We studied HLA class I (HLA-A, -B) and class II (HLA-DRB1, -DQB1) allele groups and alleles by PCR-SSP based typing in a total of 15,318 mixed ancestry Mexicans from all the states of the country divided into 78 sample sets, providing information regarding allelic and haplotypic frequencies and their linkage disequilibrium, as well as admixture estimates and genetic substructure. We identified the presence of 4268 unique HLA extended haplotypes across Mexico and find that the ten most frequent (HF > 1%) HLA haplotypes with significant linkage disequilibrium (Δ’≥0.1) in Mexico (accounting for 20% of the haplotypic diversity of the country) are of primarily Native American ancestry (A*02~B*39~DRB1*04~DQB1*03:02, A*02~B*35~DRB1*08~DQB1*04, A*68~B*39~DRB1*04~DQB1*03:02, A*02~B*35~DRB1*04~DQB1*03:02, A*24~B*39~DRB1*14~DQB1*03:01, A*24~B*35~DRB1*04~DQB1*03:02, A*24~B*39~DRB1*04~DQB1*03:02, A*02~B*40:02~DRB1*04~DQB1*03:02, A*68~B*35~DRB1*04~DQB1*03:02, A*02~B*15:01~DRB1*04~DQB1*03:02). Admixture estimates obtained by a maximum likelihood method using HLA-A/-B/-DRB1 as genetic estimators revealed that the main genetic components in Mexico as a whole are Native American (ranging from 37.8% in the northern part of the country to 81.5% in the southeastern region) and European (ranging from 11.5% in the southeast to 62.6% in northern Mexico). African admixture ranged from 0.0 to 12.7% not following any specific pattern. We were able to detect three major immunogenetic clusters correlating with genetic diversity and differential admixture within Mexico: North, Central and Southeast, which is in accordance with previous reports using genome-wide data. Our findings provide insights into the population immunogenetic substructure of the whole country and add to the knowledge of mixed ancestry Latin American population genetics, important for disease association studies, detection of demographic signatures on population variation and improved allocation of public health resources.Fil: Barquera, Rodrigo. Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis
BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration. Psicología: aspectos fundamentales de la formación y la profesión
El aspecto de la práctica clínica ha sido abordado primeramente por la doctora Córdoba Basulto, desde la experiencia, y de manera muy breve profundiza en el ámbito de la Psicología de la salud, rescata y enfatiza el aspecto de la atención e intervención al paciente de urología, colocándolo en la parte de sujeto y no de enfermedad. Nuevamente señalando el trabajo multidisciplinario donde cada agente contribuye con la atención. Desde una perspectiva meramente clínica y específicamente con la experiencia del psicoanálisis, Márquez, Guadarrama, López y Huitrón nos permiten observar la postura de la adolescencia en el proceso terapéutico, indicando que la mejor forma de tratar a un adolescente no es lo individual, ni lo familiar, sino lo grupal.Este libro es una compilación de aportaciones que permiten observar el estado actual de la psicología en México, permitiendo vislumbrar los retos reales que no solamente tienen los alumnos que asumir en su formación profesional, sino todos aquellos que se encuentran laborando en esta basta disciplina. En este sentido, el mundo globalizado y el empleo exagerado de la tecnología se convierten en nuevas exigencias que propician a su vez nuevas formas, no sólo de atención, sino de estudio e investigación que le permitan la comprensión del comportamiento humano. La Psicología mexicana en los últimos años ha dejado de ser dogmática para convertirse en una ciencia amplia y de visión, lo que le ha permitido incursionar en nuevos campos de conocimiento como el de la psicología de la salud o de la psicología criminal. Es decir ha dejado de ser una psicología de diagnóstico y clínica para convertirse en una ciencia que asume al individuo como un ser social, complejo o bien como algunos ya lo han señalado como un ente psicosociobiológico

As the knowledge base and importance of mitochondrial physiology to evolution, health and disease expands, the necessity for harmonizing the terminology concerning mitochondrial respiratory states and rates has become increasingly apparent. The chemiosmotic theory establishes the mechanism of energy transformation and coupling in oxidative phosphorylation. The unifying concept of the protonmotive force provides the framework for developing a consistent theoretical foundation of mitochondrial physiology and bioenergetics. We follow the latest SI guidelines and those of the International Union of Pure and Applied Chemistry (IUPAC) on terminology in physical chemistry, extended by considerations of open systems and thermodynamics of irreversible processes. The concept-driven constructive terminology incorporates the meaning of each quantity and aligns concepts and symbols with the nomenclature of classical bioenergetics. We endeavour to provide a balanced view of mitochondrial respiratory control and a critical discussion on reporting data of mitochondrial respiration in terms of metabolic flows and fluxes. Uniform standards for evaluation of respiratory states and rates will ultimately contribute to reproducibility between laboratories and thus support the development of data repositories of mitochondrial respiratory function in species, tissues, and cells. Clarity of concept and consistency of nomenclature facilitate effective transdisciplinary communication, education, and ultimately further discovery

Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world
Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic.
Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality.
Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States.
Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis.
Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection
Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2
The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Surgical site infection after gastrointestinal surgery in high-income, middle-income, and low-income countries: a prospective, international, multicentre cohort study
Background: Surgical site infection (SSI) is one of the most common infections associated with health care, but its importance as a global health priority is not fully understood. We quantified the burden of SSI after gastrointestinal surgery in countries in all parts of the world.
Methods: This international, prospective, multicentre cohort study included consecutive patients undergoing elective or emergency gastrointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating centres were stratified into high-income, middle-income, and low-income groups according to the UN's Human Development Index (HDI). Data variables from the GlobalSurg 1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using Bayesian multilevel logistic regression models. This trial is registered with ClinicalTrials.gov, number NCT02662231.
Findings: Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58·5%) patient were from high-HDI countries (193 hospitals in 30 countries), 3918 (31·2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (10·2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12·3%) patients had SSI within 30 days of surgery. The incidence of SSI varied between countries with high (691 [9·4%] of 7339 patients), middle (549 [14·0%] of 3918 patients), and low (298 [23·2%] of 1282) HDI (p < 0·001). The highest SSI incidence in each HDI group was after dirty surgery (102 [17·8%] of 574 patients in high-HDI countries; 74 [31·4%] of 236 patients in middle-HDI countries; 72 [39·8%] of 181 patients in low-HDI countries). Following risk factor adjustment, patients in low-HDI countries were at greatest risk of SSI (adjusted odds ratio 1·60, 95% credible interval 1·05–2·37; p=0·030). 132 (21·6%) of 610 patients with an SSI and a microbiology culture result had an infection that was resistant to the prophylactic antibiotic used. Resistant infections were detected in 49 (16·6%) of 295 patients in high-HDI countries, in 37 (19·8%) of 187 patients in middle-HDI countries, and in 46 (35·9%) of 128 patients in low-HDI countries (p < 0·001).
Interpretation: Countries with a low HDI carry a disproportionately greater burden of SSI than countries with a middle or high HDI and might have higher rates of antibiotic resistance. In view of WHO recommendations on SSI prevention that highlight the absence of high-quality interventional research, urgent, pragmatic, randomised trials based in LMICs are needed to assess measures aiming to reduce this preventable complication
