The epidemiology of Varicella Zoster Virus infection in Italy
<p>Abstract</p> <p>Background</p> <p>The epidemiological importance of varicella and zoster and the availability of an efficacious and safe vaccine have led to an important international debate regarding the suitability of mass vaccination. The objective of the study was to describe the epidemiology of varicella and zoster in Italy and to determine whether there have been changes with respect to observations provided by an analogous study conducted 8 years ago, in order to define the most appropriate vaccination strategy.</p> <p>Methods</p> <p>A number of data sources were evaluated, a cross-sectional population-based seroprevalence study was conducted on samples collected in 2004, and the results were compared with data obtained in 1996.</p> <p>Results</p> <p>The data from active and passive surveillance systems confirm that varicella is a widespread infectious disease which mainly affects children. VZV seroprevalence did not substantially differ from that found in the previous study. The sero-epidemiological profile in Italy is different from that in other European countries. In particular, the percentage of susceptible adolescents is at least nearly twice as high as in other European countries and in the age group 20–39 yrs, approximately 9% of individuals are susceptible to VZV.</p> <p>Conclusion</p> <p>The results of this study can contribute to evaluating the options for varicella vaccination. It is possible that in a few years, in all Italian Regions, there will exist the conditions necessary for implementing a mass vaccination campaign and that the large-scale availability of MMRV tetravalent vaccines will facilitate mass vaccination.</p
The epidemiology of mumps in Italy
In Italy, although vaccination has been recommended for a number of years, vaccination coverage for mumps is still sub-optimal. The objective of the present study was to evaluate the seroprevalence of mumps antibodies in the Italian population, stratified by age, gender and geographical area. The proportion of individuals positive for mumps antibodies remained stable in the age classes 0-11 months and 1 year (25.4% and 30.8%, respectively) and showed a continuous increase after the second year of life. The percentage of susceptible individuals was higher than 20% in persons 2-14 years of age and exceeded 10% in persons 15-39 years of age. No statistically significant differences were observed by gender or geographical area. Comparison between these results and the data obtained from a 1996 survey showed a statistically significant increase in seroprevalence in the age class 2-4 years. No changes were observed in the other age-groups. The results of this study confirm that the efforts made in recent years to improve vaccination coverage within the second year of life should be strengthened. \ua9 2008 Elsevier Ltd. Abnormal deactivation of the inferior frontal gyrus during implicit emotion processing in youth with bipolar disorder: Attenuated by medication
Previous neuroimaging studies of youth with bipolar disorder(BD) have identified abnormalities in emotion regulation circuitry. Using data from the Longitudinal Assessment of Manic Symptoms Cohort (a clinical sample recruited for behavioral and emotional dysregulation), we examined the impact of BD and medication on activation in these regions. Functional neuroimaging data were obtained from 15 youth with BD who currently were unmedicated with a mood stabilizer or antipsychotic (U-BD), 19 youth with medicated BD (M-BD), a non-bipolar clinical sample with high rates of disruptive behavioral disorders (non-BD, n=59), and 29 healthy controls (HC) while they were shown task-irrelevant morphing emotional faces and shapes. Whole brain analysis was used to identify clusters that showed differential activation to emotion vs. shapes across group. To assess pair-wise comparisons and potential confounders, mean activation data were extracted only from clusters within regions previously implicated in emotion regulation (including amygdala and ventral prefrontal regions). A cluster in the right inferior frontal gyrus (IFG) showed group differences to emotion vs. shapes (159 voxels, corrected p<.05). Within this cluster, U-BD youth showed decreased activation relative to HC (p=.007) and non-BD (p=.004) youth. M-BD also showed decreased activation in this cluster relative to HC and non-BD youth, but these differences were attenuated. Results were specific to negative emotions, and not found with happy faces. IFG findings were not explained by other medications (e.g. stimulants) or diagnoses. Compared to both HC and a non-BD sample, U-BD is associated with abnormally decreased right IFG activation to negative emotions
Three-row versus two-row circular staplers for left-sided colorectal anastomosis: a propensity score-matched analysis of the iCral 2 and 3 prospective cohorts
Background: Since most anastomoses after left-sided colorectal resections are performed with a circular stapler, any technological change in stapling devices may influence the incidence of anastomotic adverse events. The aim of the present study was to analyze the effect of a three-row circular stapler on anastomotic leakage and related morbidity after left-sided colorectal resections. Materials and methods: A circular stapled anastomosis was performed in 4255 (50.9%) out of 8359 patients enrolled in two prospective multicenter studies in Italy, and, after exclusion criteria to reduce heterogeneity, 2799 (65.8%) cases were retrospectively analyzed through a 1:1 propensity score-matching model including 20 covariates relative to patient characteristics, to surgery and to perioperative management. Two well-balanced groups of 425 patients each were obtained: group (A) – true population of interest, anastomosis performed with a three-row circular stapler; group (B) – control population, anastomosis performed with a two-row circular stapler. The target of inferences was the average treatment effect in the treated (ATT). The primary endpoints were overall and major anastomotic leakage and overall anastomotic bleeding; the secondary endpoints were overall and major morbidity and mortality rates. The results of multiple logistic regression analyses for the outcomes, including the 20 covariates selected for matching, were presented as odds ratios (OR) and 95% confidence intervals (95% CI). Results: Group A versus group B showed a significantly lower risk of overall anastomotic leakage (2.1 vs. 6.1%; OR 0.33; 95% CI 0.15–0.73; P = 0.006), major anastomotic leakage (2.1 vs. 5.2%; OR 0.39; 95% CI 0.17–0.87; P = 0.022), and major morbidity (3.5 vs. 6.6% events; OR 0.47; 95% CI 0.24–0.91; P = 0.026). Conclusion: The use of three-row circular staplers independently reduced the risk of anastomotic leakage and related morbidity after left-sided colorectal resection. Twenty-five patients were required to avoid one leakage
White Matter Structure in Youth With Behavioral and Emotional Dysregulation Disorders: A Probabilistic Tractographic Study
Psychiatric disorders in youth characterized by behavioral and emotional dysregulation are often comorbid and difficult to distinguish. An alternative approach to conceptualizing these disorders is to move toward a diagnostic system based on underlying pathophysiologic processes that may cut across conventionally defined diagnoses. Genetic effects on gene expression across human tissues
Characterization of the molecular function of the human genome and its variation across individuals is essential for identifying the cellular mechanisms that underlie human genetic traits and diseases. The Genotype-Tissue Expression (GTEx) project aims to characterize variation in gene expression levels across individuals and diverse tissues of the human body, many of which are not easily accessible. Here we describe genetic effects on gene expression levels across 44 human tissues. We find that local genetic variation affects gene expression levels for the majority of genes, and we further identify inter-chromosomal genetic effects for 93 genes and 112 loci. On the basis of the identified genetic effects, we characterize patterns of tissue specificity, compare local and distal effects, and evaluate the functional properties of the genetic effects. We also demonstrate that multi-tissue, multi-individual data can be used to identify genes and pathways affected by human disease-associated variation, enabling a mechanistic interpretation of gene regulation and the genetic basis of diseas

Problemi neurocognitivi e psicosociali nella sindrone di Turner
Scopo di questo nostro studio è quello di dare un contributo personale alla definizione ed all’approfondimento dei problemi neurocognitivi e psicosociali nelle pazienti con Sindrome di Turner (ST). Abbiamo sottoposto a valutazione psicometrica 24 soggetti di età compresa tra 1.6 e 28.0 anni (X = 14.2 +/- 6.4). La valutazione è stata effettuata mediante l’impiego di alcuni test psicometrici, differenti per le diverse fasce di età, ed un colloquio psicologico. Dai risultati ottenuti è emerso che: a) il quoziente intellettivo (Q.I.) medio era ai limiti inferiori della normalità (79 +/- 19.7, range 41-101); b) 8/24 pazienti (33%) presentavano un quadro di insufficienza mentale di grado variabile (Q.I. compreso tra 41 e 77, X = 61.4 +/- 14.5); e) 2/24 pazienti (8.3%) presentavano un’insufficienza mentale di grado severo e perciò non quantizzabile; d) il Q.I. era mediamente più basso nelle pazienti con monosomia X (68.1 +/- 19.7, range 4-196) rispetto a quelle con mosaicismo (86.0 +/- 14.0, range 61-101, p <
0.0025) o con riarrangiamenti strutturali della X (95.0 +/- 4.2, range 91-101, p < 0.0025); e) i deficit specifici più frequentemente rilevati riguardavano: la coordinazione visuo-motoria, la memoria, la concentrazione, l’attenzione e il linguaggio; f) anche in alcuni pazienti senza insufficienza mentale erano rilevabili deficit specifici relativi alle suddette aree; g) il rendimento scolastico nella nostra casistica non era nel complesso soddisfacente e molte pazienti dovevano ricorrere all’insegnante di sostegno; h) il grado di socializzazione era sufficiente nel 33% dei casi.
Dall’analisi dei nostri dati emerge come i problemi neurocognitivi e comportamentali abbiano una notevole rilevanza clinica nella ST. Il riscontro abbastanza frequente di deficit specifici a carico di alcune aree neurocognitive induce a pensare che tali deficit possano rappresentare una caratteristica intrinseca della suddetta sindrome senza peraltro negare un certo ruolo negativo svolto da fattori sfavorevoli generici quali il disagio psicologico e l’emarginazione sociale conseguiti al grave deficit staturale
