    The Relationship Between Extracurricular Activities and the Well-Being of Undergraduate Students

    Extracurricular involvement is a large part of the undergraduate student experience. Mental health can be affected when students are both mandated and encouraged to meet certain requirements, whether they are for an academic program or for experience and well-being. The current study was designed to analyze the relationship between extracurricular participation and well-being. The sample included 70 undergraduate students from the University of Western Ontario. Several hypotheses were proposed based on the type of extracurricular activity and well-being measure. The findings supported two out of the five hypotheses advanced, but also produced several other noteworthy findings. The most significant variable related to overall well-being was quality of sleep. Further, having part-time jobs was associated with higher depression scores. Future research is needed to establish the effects of each type and level of extracurricular involvement on one’s well-being

    Cut-off points of the visceral adiposity index (VAI) identifying a visceral adipose dysfunction associated with cardiometabolic risk in a Caucasian Sicilian population

    <p>Abstract</p> <p>Background</p> <p>The Visceral Adiposity Index (VAI) is a sex-specific mathematical index, based on Waist Circumference (WC), Body Mass Index (BMI), triglycerides (TG) and HDL cholesterol (HDL) levels, indirectly expressing visceral adipose function and insulin sensitivity. Our aim was to find the optimal cut-off points of VAI identifying a visceral adipose dysfunction (VAD) associated with cardiometabolic risk in a Caucasian Sicilian population.</p> <p>Methods</p> <p>Medical check-up data of 1,764 Primary Care patients (PC patients) were retrospectively and cross-sectionally examined using a receiver-operating characteristic (ROC) curve to determine appropriate stratified-for-age cut-off of VAI, for the identification of PC patients with Metabolic Syndrome (MetS) according to the NCEP-ATP III criteria. The PC patients with higher VAI scores were subdivided into three groups according to VAI tertiles (i.e. PC patients with mild VAD, moderate VAD or severe VAD). Finally, VAD classes were compared to classical cardio- and cerebrovascular risk factors as independent predictors of coronary heart disease and/or myocardial infarction, transient ischemic attack and/or ischemic stroke.</p> <p>Results</p> <p>Moderate and severe VADs proved to be independently associated with cardiovascular events [(OR: 5.35; 95% CI: 1.92-14.87; p = 0.001) and (OR: 7.46; 95% CI: 2.64-21.05; p < 0.001) respectively]. Mild, moderate and severe VADs were found to be independently associated with cerebrovascular events [(OR: 2.73; 95% CI: 1.12-6.65; p = 0.027), (OR: 4.20; 95% CI: 1.86-9.45; p = 0.001) and (OR: 5.10; 95% CI: 2.14-12.17; p < 0.001) respectively].</p> <p>Conclusions</p> <p>Our study suggests that among Caucasian Sicilian subjects there are clear cut-off points of VAI able to identify a VAD strongly associated with cardiometabolic risk.</p

    Quality of life and psychological impact in patients with atopic dermatitis

    Atopic dermatitis (AD) is a dermatological disorder that affects patients' mental health and psychological state in complex ways. The importance of understanding the entire scope of this burden is well recognized, but there is limited comprehensive information about the resulting stress on adult patients with AD. This study aimed to determine the degree of psychological stress in patients with AD compared to healthy participants. A total of 352 adult patients participated in this cross-sectional study-174 with AD and 178 healthy participants. Demographic and clinical data were collected. Itch and sleep disturbance were assessed using a numeric rating scale and a visual analogue scale. The 20-item Toronto Alexithymia Scale (TAS-20) and Beck Depression Inventory (BDI) questionnaires were administered to assess the symptoms of alexithymia and depression. Quality of life (QOL) was assessed in AD patients using the Dermatology Quality Index. In our study, we found high TAS-20 and BDI scores among patients with AD. The prevalence of alexithymic personality features was 56.3% in patients with AD versus 21.3% in healthy controls (p &lt; 0.001). Based on BDI scoring (BDI-21 &gt; 13), depression was suspected in a significantly higher number of patients with AD than in the control group (56.9% (99/174) vs. 15.7% (28/178); p &lt; 0.0001). Eczema Area and Severity Index (EASI) score did not show any significant correlations with psychological parameters. Among clinical parameters, only sleep disturbance was positively correlated with depression (R = 0.307, p &lt; 0.005). Our data show that the severity index score as a representative factor of skin involvement has a limited role in predicting the effect of skin diseases on mental status. Screening and assessment for psychiatric disorders, QOL, and sleep disturbance in patients with atopic dermatitis cannot be neglected by physicians and they should be treated in clinical practice with the consideration of psychosomatic approaches

    Comprehensive Modeling and Experimental Testing of Fault Detection and Management of a Nonredundant Fault-Tolerant VSI

    This paper presents an investigation and a comprehensive analysis on fault operations in a conventional three-phase voltage source inverter. After an introductory section dealing with power converter reliability and fault analysis issues in power electronics, a generalized switching function accounting for both healthy and faulty conditions and an easy and feasible method to embed fault diagnosis and reconfiguration within the control algorithm are introduced. The proposed system has simple and compact implementation. Experimental results operating both at open- and closed-loop current control, obtained using a test bench realized using a dSPACE system and the fault-tolerant inverter prototype demonstrate that the proposed solution is effective and feasible and makes all faults easily managed by the controller itself

    Metabolic parameters and adipokine profile during GH replacement therapy in children with GH deficiency

    Objective: GH replacement therapy in children with GH deficiency (GHD) mainly promotes linear growth. Not only have very few studies fully analyzed the metabolic consequences of GH therapy, but also the question as to whether GH may affect adipokine secretion has been insufficiently investigated. Our aim was to study the effects of GH replacement therapy on auxological data, lipid and glycemic profiles, insulin homeostasis (HOMA-IR) and serum adipokines in children. Methods: This was a 1-year prospective study. Thirty-four GHD children (11.6G2.6 years) and thirty healthy matched controls were enrolled. Children affected by GHD were studied both before beginning continuous GH replacement therapy and again at 12 months. Results: At the beginning of the study, total and LDL cholesterol were higher in GHD children than in controls (P!0.001), whereas HDL cholesterol, triglycerides, insulin, HOMA-IR, leptin, and adiponectin were similar. At 12 months of continuous GH replacement therapy in the GHD group, there was a significant increase in both auxological data and IGF-I (P!0.001); total cholesterol (P!0.001), LDL (P!0.001), triglycerides (P!0.005), and leptin (P!0.001) decreased significantly; HDL (P!0.003), insulin (P!0.001), HOMA-IR (P!0.001) increased while adiponectin was unmodified. Furthermore, IGF-ID showed an inverse correlation with leptin D (rZK0.398, PZ0.02). Conclusions: In GHD children, the evaluation of metabolic parameters proves to be a useful tool for the evaluation of auxological parameters during GH replacement therapy. In our study, GH replacement therapy in GHD children improved final height, restored IGF-I levels, reduced leptin levels, and improved the lipid profile, without producing any unfavorable effects on glucose metabolism

    Numerical modeling of bifacial PV string performance: Perimeter effect and influence of uniaxial solar trackers

    The bifacial photovoltaic (PV) systems have recently met large interest. The performance of such systems heavily depends on the installation conditions and, in particular, on the albedo radiation collected by the module rear side. Therefore, it is of crucial importance to have an accurate performance model. To date, in the scientific literature, numerous models have been proposed and experimental data collected to study and optimize bifacial PV system performance. Currently, 3D and 2D models of bifacial PV devices exist. Though the former are more mathematically complex, they can lead to more accurate results, since they generally allow to fully consider the main aspects influencing a bifacial PV system performance. Recently, we have proposed and validated through experimental data a 3D model tested as a function of module height, tilt angle, and ground albedo. In this work, through such a model, we studied the role played by the perimeter zones surrounding the PV string, by considering PV strings of 30 or 60 modules. We considered the cases of fixed installation with optimal PV module tilt and of installation with uniaxial horizontal solar tracker. We evaluated the PV energy yield as a function of the size of the perimeter zones for the two cases, i.e., both with and without the solar tracker. In optimal perimeter conditions, we then studied the behavior of bifacial and mono-facial PV strings by varying the geographical location in a large latitude range

    Efficiency Enhancement of Permanent Magnet Synchronous Motor Drives by on-Line Loss Minimization Approaches

    In this paper, a new loss minimization control algorithm for inverter-fed permanent-magnet synchronous motors (PMSMs), which allows for the reduction of the power losses of the electric drive without penalty on its dynamic performance, is analyzed, experimentally realized, and validated. In particular, after a brief recounting of two loss minimization control strategies, namely, the "search control" and the "loss-model control," both a new modified dynamic model of the PMSM (which takes into account the iron losses) and an innovative "loss-model" control strategy are presented. Experimental tests on a specific PMSM drive employing the proposed loss minimization algorithm have been performed, aiming to validate the actual implementation. The main results of these tests confirm that the dynamic performance of the drive is maintained, and in small motors enhancement up to 3.5% of the efficiency can be reached in comparison with the PMSM drive equipped with a more traditional control strategy

    Janus kinase (JAK) 2 V617F mutation as the cause of primary thrombocythemia in acromegaly with severe visceromegaly and divergence between growth hormone and insulin-like growth factor-1 concentrations during the follow-up: causal or casual association?

    OBJECTIVE: An increased prevalence of hematological abnormalities is reported in acromegaly, but to date no reports about the presence of the Janus Kinase (JAK) 2 mutation in acromegalic patients have been described. DESIGN: We report the complex clinical presentation of the unique case, never described, of acromegaly due to GH-secreting pituitary adenoma associated with JAK2 V617F mutation. RESULTS: The patient shows primary thrombocythemia and myelofibrosis, due to JAK2 V617F mutation, severe visceromegaly and a peculiar clinical course of the disease characterized by discrepant values of GH and IGF-1 during somatostatin analog (SA) treatment despite a significant reduction in pituitary adenoma size and therapeutic resistance both to SA and pegvisomant. CONCLUSIONS: The presence of JAK2 V617F mutation is a cause of primary thrombocythemia and myelofibrosis in acromegaly. In this patient, a peculiar clinical course of acromegaly was observed, with the difficulty in controlling the disease.     Implementing quality by design for biotech products: are regulators on track?

    Quality by design (QbD) is an innovative approach to drug development that has started to be implemented into the regulatory framework, but currently mainly for chemical drugs. The recent marketing authorization of the first monoclonal antibody developed using extensive QbD concepts in the European Union paves the way for future further regulatory approvals of complex products employing this cutting-edge technological concept. In this paper, we report and comment on insights and lessons learnt from the non-public discussions in the European Medicines Agency's Biologicals Working Party and Committee for Medicinal Products for Human Use on the key issues during evaluation related to the implementation of an extensive QbD approach for biotechnology-derived medicinal products. Sharing these insights could prove useful for future developments in QbD for biotech products in general and monoclonal antibodies in particular

    Allelic Variants of HLA-C Upstream Region, PSORS1C3, MICA, TNFA and Genes Involved in Epidermal Homeostasis and Barrier Function Influence the Clinical Response to Anti-IL-12/IL-23 Treatment of Patients with Psoriasis

    Several biologic therapies have been developed to treat moderate-to-severe psoriasis, with patients exhibiting different clinical benefits, possibly due to the heterogeneity of pathogenic processes underlying their conditions. Ustekinumab targets the IL-12/IL-23-p40 subunit and inhibits type-1 and type-17 T-cell responses. Although ustekinumab is effective as both short- and long-term treatment, therapeutic response varies considerably among patients. Ustekinumab biosimilars will be commercialized in the very next future, likely broadening the use of this drug in the treatment of psoriasis patients. Our pharmacogenomic study evaluated the influence of 417 single-nucleotide polymorphisms (SNPs) in psoriasis-risk alleles on the clinical response to ustekinumab in a cohort of 152 patients affected by moderate-to-severe plaque-type psoriasis. Differences in SNP pattern characterizing HLA-Cw6(+) or HLA-Cw6(-) patients, showing high or low responses to ustekinumab, were also analysed. We identified twelve SNPs in HLA-C upstream region (rs12189871, rs4406273, rs9348862 and rs9368670), PSORS1C3 (rs1265181), MICA (rs2523497), LCE3A-B intergenic region (rs12030223, rs6701730), CDSN (rs1042127, rs4713436), CCHCR1 (rs2073719) and in TNFA (rs1800610) genes associated with excellent response to ustekinumab. We also found that HLA-Cw6(+) and HLA-Cw6(-) patients carried out distinct patterns of SNPs associated with different clinical responses. The assessment of HLA-C alleles, together with other genetic variants, could be helpful for defining patients who better benefit from anti-IL-12/IL-23 therapy
