    Specifics of motivation of volunteers participating in in a clinical trial of the COVID-19 vaccine

    Background. The pandemic coronavirus infection has contributed to the development of effective preventive methods to slow the spread of the virus. Mass vaccination is the only way to protect oneself from infection. The development of drugs, including vaccines, involves clinical trials (CT). The problem of conducting a CT is recruiting the necessary number of volunteers. Goal: to identify specifics of motivation of patients participating in the clinical trial of COVID-19 vaccines, as it is a very important medical and sociological issue requiring more detailed study. Materials and Methods. The results of an anonymous questionnaire survey of volunteers participating in the “EpiVacCorona” vaccine CT are presented. Overall, 203 questionnaires were analyzed, containing 19 questions answered by the residents of Kaliningrad, aged 20 to 84 years. At the stage of completion of participation in the clinical trial, information was obtained about the respondent’s attitudes to the trial, the main factors of motivation or demotivation to participate, the degree of their trust in the trial. Results. Information disseminated through the employer was an effective way to organize patient recruitment for the vaccine CT; the effectiveness of using the media was quite low. The positives of participation in the CT for volunteers were ethical considerations. Negatives for participants were the risks associated with adverse reactions or receiving a placebo. Relatives had the greatest influence on the volunteers’ decision to participate in the CT, and medical professionals, media personalities, and scientists had less influence. Conclusion. The results will help to improve patient recruitment, patient awareness of the CT, and the benefits to medicine and society. Motivating and demotivating factors will improve patient recruitment

    Structural features of Ni-Cr-Si-B materials obtained by different technologies

    This study considers the structural features of Ni-Cr-Si-B (Ni - base; 15.1 % Cr; 2 % Si; 2 % B; 0.4 % C) materials obtained by different methods. The self-fluxing coatings were deposited by plasma spraying on the tubes from low carbon steel. Bulk cylinder specimens of 20 mm diameter and 15 mm height were obtained by spark plasma sintering (SPS). The structure and phase composition of these materials were investigated by optical microscopy (OM), scanning electron microscopy (SEM), transmission electron microscopy (TEM) and X-ray diffractometry (XRD). The major phases of coatings and sintered materials are [gamma]-Ni, Ni[3]B, CrB and Cr[7]C[3]. We demonstrate that the particle unmelted in the process of plasma spraying or SPS consist of [gamma]-Ni-NEB eutectic and also CrB and Cr[7]C[3] inclusions. The prolonged exposure of powder to high temperatures as well as slow cooling rates by SPS provide for the growth of the structural components as compared to those of plasma coatings materials. High cooling rates at the plasma spraying by melted particles contribute to the formation of supersaturated solid solution of Cr, Si and Fe in [gamma]-Ni. The structure of the melted particles in sintering material has gradient composition: the core constituted of Ni grains of 10 μm with [gamma]-Ni-Ni[3]B eutectic on the edges. The results of the experiment demonstrate that the sintering material has a smaller microhardness in comparison with plasma coatings (650 and 850 MPa, respectively), but at the same time the material has higher density (porosity less than 1 %) than plasma coatings (porosity about 2.. .3 %)

    Course of COVID-19 in Patients with Inflammatory Bowel Disease: Regional Experience

    Aim: to study the course of COVID-19 in patients with inflammatory bowel diseases (IBD) using the example of the region of the Republic of Tatarstan.Material and methods. The study included 101 patients diagnosed with IBD and COVID-19, who were observed in two infectious diseases hospitals in Kazan (Republican Clinical Hospital of the Ministry of Health of the Republic of Tatarstan and City Clinical Hospital No. 7) and on an outpatient basis from April 2020 to March 2022. All patients underwent physical examination, laboratory and instrumental diagnostic methods, including a PCR test for SARSCoV-2. Chest computed tomography was performed in patients with clinical signs of moderate to severe COVID-19.Results. Ulcerative colitis (UC) was diagnosed in 60 (59.4 %) patients, Crohn's disease (CD) — in 41 (40.6 %) patients. The mean age of the patients was 41.0 ± 14.7 years, of which 59 (58.4 %) were men and 42 (41.6 %) were women. A comparative analysis of patients with and without IBD and CT-verified lung disease was carried out. It was found that the development of viral pneumonia was influenced by age over 55 years (39.2 ± 9.7 vs. 46.3 ± 10.6, p < 0.05), increased Body Mass Index (BMI) (23.1 ± 5.35 vs. 30.25 ± 6.17, p < 0.05), hypertension (6 (8.3 %) vs. 8 (27.6 %), p < 0.05), diabetes mellitus (2 (2.7 %) vs. 5 (17.2 %), p < 0.05), the use of corticosteroids in the treatment of IBD (8 (11.1 %) vs. 10 (34.5 %), p < 0.05). In a comparative analysis of patients with IBD and COVID-19 from the SECURE-IBD database and own data, it was found that the average age of patients was comparable (42.7 vs 41.0). At the same time, in our group of male patients, there were slightly more people with DM, increased BMI, and an active course of IBD. The proportion of hospitalized patients was higher. In our cohort, there were fewer patients receiving biological therapy, but more patients on 5-aminosalicylic acid (5-ASA) and systemic corticosteroids. At the same time, lethal outcomes were comparable.Conclusion. In patients with IBD, the development of viral pneumonia was influenced by known risk factors for COVID-19: age over 55 years (p < 0.05, odds ratio (OR) 3.153), increased BMI (p < 0.05, OR 1.667), hypertension  (p < 0.05, OR 2.724), diabetes (p < 0.05, OR 1.489), as well as the use of systemic corticosteroids (p < 0.05, OR 1.5)

Methotrexate is a folic acid antagonist and has an inhibitory effect on the proliferation of trophoblast cells. There are various methotrexate regimens; the choice is based on the level of β-subunit of human chorionic gonadotropin, the location of the ectopic pregnancy and the patient's adherence to treatment. Therapy of ectopic tubal pregnancy with methotrexate is an effective and safe alternative to surgical treatment, does not affect the ovarian reserve and preserves the woman's fertility
