Anålisis de los factores genéticos y ambientales con influencia en la circunferencia escrotal en el vacuno de carne de raza Retinta
De los caracteres del vacuno de carne, los relacionados con la eficiencia reproductiva han sido considerados
de una impol1ancia fundamental. Dentro de Ă©stos, cabe
destacar la CE, parĂĄmetro correlacionado positivamente
con las caracterĂsticas cualitativas y cuantitativas del semen, asl como asociado a parĂĄmetros reproductivos en
hembras (precoc idad sexual y fert ilidad).
En este trabaja se ha estudiado la CE desde distintos
aspectos como los factores ambientales que le infl uyen,
su evolución a lo largo del tiempo, el cålculo del paråmetro genético de la heredabilidad y las relaciones con
otras variables como el peso vivo del animal. Para ello, se
realizaron 1.020 mediciones en 281 animales pertenencientes a 20 ganaderĂas del nĂșcleo de Control del plan de
mejora de esta raza.
l a media obtenida para la circunferencia escrota l fue
de 30,66 ± 0, 166 (CV. = 17,3%)y 34,38 ± 0, 140 ¥C.V.=
9, 1%) para an ima les de un año de edad, y la heredabilidad estimada para este caråcter de 0,36.
El peso vivo, y en segundo lugar la edad, fueron las
dos variables con mayor influencia en la circunferencia
escrotal en esta raza. Otros fa ctores ambientales a tener
en cuenta en los anålisis genéticos de esta variable son la
ganaderĂa y año -estaciĂłn de medida-, cuyo efecto se
ha mostrado altamente significativo. AsĂ si bien la curva
de crecimiento de la CE que mejor se ajusta al intervalo
de c recimiento ana lizado fue la logistftica 2, a efectos
pråcticos de tipificac ión al año, consideramos que la curva polinómica de segundo grado da un ajuste equiva lente, permitiendo una mayor simplificación de los cålculos

Design and validation of a three-instrument toolkit for the assessment of competence in electrocardiogram rhythm recognition
Background: Rapid and accurate interpretation of cardiac arrhythmias by nurses has been linked with safe practice and positive patient outcomes. Although training in electrocardiogram (ECG) rhythm recognition is part of most undergraduate nursing programmes, research continues to suggest that nurses and nursing students lack competence in recognising cardiac rhythms. In order to promote patient safety, nursing educators must develop valid and reliable assessment tools that allow the rigorous assessment of this competence before nursing students are allowed to practise without supervision.
Aim: To develop and psychometrically evaluate a toolkit to holistically assess competence in ECG rhythm recognition.
Methods: Following a convenience sampling technique, 293 nursing students from a nursing faculty in a Spanish university were recruited for the study. The following three instruments were developed and psychometrically tested: a knowledge assessment tool (ECG-KAT), a skills assessment tool (ECG-SAT) and a self-efficacy assessment tool (ECG-SES). Reliability and validity (content, criterion and construct) of these tools were meticulously examined.
Results: A high Cronbachâs alpha coefficient demonstrated the excellent reliability of the instruments (ECG-KAT=0.89; ECG-SAT=0.93; ECG-SES=0.98). An excellent context validity index (S-CVI/Ave>0.94) and very good criterion validity were evidenced for all the tools. Regarding construct validity, principal component analysis revealed that all items comprising the instruments contributed to measure knowledge, skills or self-efficacy in ECG rhythm recognition. Moreover, known-groups analysis showed the toolsâ ability to detect expected differences in competence between groups with different training experiences.
Conclusion: The three-instrument toolkit developed showed excellent psychometric properties for measuring competence in ECG rhythm recognition
Dextran sulfate from Leuconostoc mesenteroides B512F exerts potent antiviral activity against SARS-CoV-2 in vitro and in vivo
The emergent human coronavirus SARS-CoV-2 and its resistance to current drugs makes the need for new potent treatments for COVID-19 patients strongly necessary. Dextran sulfate (DS) polysaccharides have long demonstrated antiviral activity against different enveloped viruses in vitro. However, their poor bioavailability has led to their abandonment as antiviral candidates. Here, we report for the first time the broad-spectrum antiviral activity of a DS-based extrapolymeric substance produced by the lactic acid bacterium Leuconostoc mesenteroides B512F. Time of addition assays with SARS-CoV-2 pseudoviruses in in vitro models confirm the inhibitory activity of DSs in the early stages of viral infection (viral entry). In addition, this exopolysaccharide substance also reports broad-spectrum antiviral activity against several enveloped viruses such as SARS-CoV-2, HCoV229E, HSV-1, in in vitro models and in human lung tissue. The toxicity and antiviral capacity of DS from L. mesenteroides was tested in vivo in mouse models which are susceptible to SARS-CoV-2 infection. The described DS, administered by inhalation, a new route of administration for these types of polymers, shows strong inhibition of SARS-CoV-2 infection in vivo, significantly reducing animal mortality and morbidity at non-toxic doses. Quality of life in perimenopausal women working in the health and educational system
The objective of this study was to determine the association between the professional activity and the perceived quality of life in climacteric women who work in Health and Education. A descriptive and cross-sectional study was developed in a sample of 203 climacteric women, together with a correlation analysis of labor factorsâ influence on health-related quality of life. Significant differences were found between working womenâs quality of life and some labor conditions. The perceived quality of life in perimenopausal women who work in Education is higher than that of those working in Health (p=0.004). Nursing professionals can develop health programs that deal with the work conditions that negatively influence the perceived quality of life in climacteric women.El objetivo de este estudio fue determinar la asociaciĂłn entre la actividad profesional y la calidad de vida percibida en mujeres climatĂ©ricas que trabajan en las ĂĄreas sanitaria y educativa. Se realizĂł un estudio descriptivo y transversal, en una muestra de 203 mujeres climatĂ©ricas, junto al anĂĄlisis correlacional de la influencia de los factores laborales en la calidad de vida relacionada con la salud. Se encontraron diferencias significativas entre la calidad de vida de las trabajadoras y algunas condiciones laborales. La calidad de vida percibida por las mujeres perimenopĂĄusicas que trabajan en educaciĂłn es superior a la de las que trabajan en salud (p=0,004). Los profesionales de EnfermerĂa pueden elaborar programas de salud, que aborden las condiciones de trabajo que influyen negativamente en la calidad de vida percibida por las mujeres climatĂ©ricas.O objetivo deste estudo foi determinar a associação entre a atividade profissional e a qualidade de vida percebida em mulheres climatĂ©ricas que trabalham nas ĂĄreas da saĂșde e educação. Post-anaesthesia pulmonary complications after use of muscle relaxants (POPULAR): a multicentre, prospective observational study
Background Results from retrospective studies suggest that use of neuromuscular blocking agents during general
anaesthesia might be linked to postoperative pulmonary complications. We therefore aimed to assess whether the use
of neuromuscular blocking agents is associated with postoperative pulmonary complications.
Methods We did a multicentre, prospective observational cohort study. Patients were recruited from 211 hospitals in
28 European countries. We included patients (aged â„18 years) who received general anaesthesia for any in-hospital
procedure except cardiac surgery. Patient characteristics, surgical and anaesthetic details, and chart review at discharge
were prospectively collected over 2 weeks. Additionally, each patient underwent postoperative physical examination
within 3 days of surgery to check for adverse pulmonary events. The study outcome was the incidence of postoperative
pulmonary complications from the end of surgery up to postoperative day 28. Logistic regression analyses were
adjusted for surgical factors and patientsâ preoperative physical status, providing adjusted odds ratios (ORadj) and
adjusted absolute risk reduction (ARRadj). This study is registered with ClinicalTrials.gov, number NCT01865513.
Findings Between June 16, 2014, and April 29, 2015, data from 22803 patients were collected. The use of neuromuscular
blocking agents was associated with an increased incidence of postoperative pulmonary complications in patients who
had undergone general anaesthesia (1658 [7·6%] of 21694); ORadj 1·86, 95% CI 1·53â2·26; ARRadj â4·4%, 95% CI
â5·5 to â3·2). Only 2·3% of high-risk surgical patients and those with adverse respiratory profiles were anaesthetised
without neuromuscular blocking agents. The use of neuromuscular monitoring (ORadj 1·31, 95% CI 1·15â1·49;
ARRadj â2·6%, 95% CI â3·9 to â1·4) and the administration of reversal agents (1·23, 1·07â1·41; â1·9%, â3·2 to â0·7)
were not associated with a decreased risk of postoperative pulmonary complications. Neither the choice of sugammadex
instead of neostigmine for reversal (ORadj 1·03, 95% CI 0·85â1·25; ARRadj â0·3%, 95% CI â2·4 to 1·5) nor extubation at
a train-of-four ratio of 0·9 or more (1·03, 0·82â1·31; â0·4%, â3·5 to 2·2) was associated with better pulmonary outcomes.
Interpretation We showed that the use of neuromuscular blocking drugs in general anaesthesia is associated with an
increased risk of postoperative pulmonary complications. Anaesthetists must balance the potential benefits of
neuromuscular blockade against the increased risk of postoperative pulmonary complications
The impact of surgical delay on resectability of colorectal cancer: An international prospective cohort study
AimThe SARS-CoV-2 pandemic has provided a unique opportunity to explore the impact of surgical delays on cancer resectability. This study aimed to compare resectability for colorectal cancer patients undergoing delayed versus non-delayed surgery.MethodsThis was an international prospective cohort study of consecutive colorectal cancer patients with a decision for curative surgery (January-April 2020). Surgical delay was defined as an operation taking place more than 4âweeks after treatment decision, in a patient who did not receive neoadjuvant therapy. A subgroup analysis explored the effects of delay in elective patients only. The impact of longer delays was explored in a sensitivity analysis. The primary outcome was complete resection, defined as curative resection with an R0 margin.ResultsOverall, 5453 patients from 304 hospitals in 47 countries were included, of whom 6.6% (358/5453) did not receive their planned operation. Of the 4304 operated patients without neoadjuvant therapy, 40.5% (1744/4304) were delayed beyond 4âweeks. Delayed patients were more likely to be older, men, more comorbid, have higher body mass index and have rectal cancer and early stage disease. Delayed patients had higher unadjusted rates of complete resection (93.7% vs. 91.9%, PÂ =Â 0.032) and lower rates of emergency surgery (4.5% vs. 22.5%, PâConclusionOne in 15 colorectal cancer patients did not receive their planned operation during the first wave of COVID-19. Surgical delay did not appear to compromise resectability, raising the hypothesis that any reduction in long-term survival attributable to delays is likely to be due to micro-metastatic disease
