Pola Produksi Dan Nutrisi Rumput Kume (Shorgum Plumosum Var. Timorense) Pada Lingkungan Alamiahnya
The purpose of the study was to know the production system and nutrition content of Kume grass (Shorgum plumosum var. Timorense). The study used experimental method. The data were analyzed descriptively to know the production system and nutrition content of Kume grass. Two variables namely independent variables (cutting age) and dependent variables (nutritive values, plant height, dry matter production, organic matter and crude protein) were analysed statistically using correlation. The growth pattern of Kume grass from 14 to 112 days was polinomial. The increase of cutting age was followed by the increase of dry matter, organic matter, crude fiber and the decrease of crude protein. The crude fiber also increased together with the cutting age
Prehospital triage accuracy in a criteria based dispatch centre.
BACKGROUND: Priority dispatch accuracy is a key issue in optimizing the match between patients' medical needs and pre-hospital resources. This study measures the accuracy of a Criteria Based Dispatch (CBD) system, by evaluating discrepancies between dispatch priorities and ambulance crews' severity evaluations.
METHODS: This is a retrospective study conducted from January 2011 to December 2011. We ruled that a National Advisory Committee for Aeronautics (NACA) score > 3 (injuries/diseases which can possibly lead to deterioration of vital signs) to 7 (lethal injuries/ diseases) should require a priority dispatch with lights and siren (L&S), while NACA scores < 4 should require a priority dispatch without L&S. Over triage was defined as the proportion of L&S dispatches with a NACA score < 4, and under triage as the proportion of dispatches without L&S with a NACA score > 3.
RESULTS: There were 29,008 primary missions in 2011, 1122 were excluded. Of the 15,749 L&S missions, 12,333 patients had a NACA score < 4, leading to an over triage rate of 78 %; 561 missions out of 12,137 missions without L&S had a NACA score > 3, leading to an under triage rate of 4.6 %. Sensitivity was 86 % (95 % confidence interval: 85.6-86.4 %), specificity 48 % (47.4-48.6 %), positive predictive value 21.7 % (21.2-22.2 %), and negative predictive value 95.4 % (95.2-95.6 %).
CONCLUSION: The rates of over triage and under triage in our CBD are 78 and 4.6 % respectively. The lack of consistent or universal metrics is perhaps the most important limitation in dispatch accuracy research. This is mainly due to the large heterogeneity of dispatch systems and prehospital emergency system
Anti-inflammatory Effect of Aqueous Extract of Coffee plant leaves (Coffea canephora) in Rats.
The anti-inflammatory and analgesic activities of aqueous extract of coffea canephora leaves was investigated in rats. Egg albumin- induced hind paw oedema was used to assess the anti- inflammatory activity of the extract. The results of the anti-inflammatory study revealed that 60, 120 and 180 mg/kg of the extract reduced the formalin-induced oedema significantly (P<0.05) at the beginning of 2 hours when compared to the control group. The present study showed that the aqueous extract of Coffea canephoras has anti-inflammatory activity that could be mediated via modulators of pain and inflammation. This research aims to comprehend Employee Performance, Work Motivation, Transformational Leadership, Employee Competency, and Organizational Culture at the Office of the Chief Port and Port Authority Class III Kupang, as well as to examine the influence of variables related to Work Motivation as a mediator and employee performance as a dependent variable. This research is quantitative in nature, utilizing both primary and secondary data, and employing descriptive and inferential statistical analyses. The results indicate that Transformational Leadership, Employee Competency, and Organizational Culture significantly influence employee work motivation; Work Motivation significantly affects employee performance; and Work Motivation mediates the influence of Transformational Leadership, Employee Competency, and Organizational Culture on Employee Performance at the Office of the Chief Port Class III and Port Authority Kupang
Prevalence and clinical significance of point of care elevated lactate at emergency admission in older patients: a prospective study.
Patients who are over 65 years old represent up to 24% of emergency department (ED) admissions. They are at increased risk of under-triage due to impaired physiological responses. The primary objective of this study was to assess the prevalence of elevated lactate by point of care testing (POCT) in this population. The secondary objective was to assess the additional value of lactate level in predicting an early poor outcome, as compared to and combined with common clinical scores and triage scales.
This monocentric prospective study recruited ED patients who were over 65 years old between July 19th 2019 and June 17th 2020. Patients consulting for seizures or needing immediate assessment were excluded. POCT lactates were considered elevated if ≥ 2.5 mmol/L. A poor outcome was defined based on certain complications or therapeutic decisions.
In total, 602 patients were included; 163 (27.1%) had elevated lactate and 44 (7.3%) had a poor outcome. There was no association between poor outcome and lactate level. Modified Early Warning Score (MEWS) was significantly associated with poor outcome, alongside National Early Warning Score (NEWS). Logistic regression also associated lactate level combined with MEWS and poor outcome.
The prevalence of elevated lactate was 27.1%. Lactate level alone or combined with different triage scales or clinical scores such as MEWS, NEWS and qSOFA was not associated with prediction of a poor outcome. MEWS alone performed best in predicting poor outcome. The usefulness of POCT lactate measurement at triage is questionable in the population of 65 and above
Design of TiO2-Surfactin Hybrid Systems with Multifunctional Properties
In recent years, multifunctional inorganic-organic hybrid materials have been widely investigated in order to determine their potential synergetic, antagonist, or independent effects in terms of reactivity. The aim of this study was to design and characterize a new hybrid material by coupling well-known photocatalytic TiO2 nanoparticles with sodium surfactin (SS), a biosurfactant showing high binding affinity for metal cations as well as the ability to interact with and disrupt microorganisms' cell membranes. We used both chemical and colloidal synthesis methodologies and investigated how different TiO2:SS weight ratios affected colloidal, physicochemical, and functional properties. We discovered a clear breaking point between TiO2 and SS single-component trends and identified different ranges of applicability by considering different functional properties such as photocatalytic, heavy metal sorption capacity, and antibacterial properties. At low SS contents, the photocatalytic properties of TiO2 are preserved (conversion of organic dye = 99% after 40 min), and the hybrid system can be used in advanced oxidation processes, taking advantage of the additional antimicrobial SS properties. At high SS contents, the TiO2 photoactivity is inhibited, and the hybrid can be usefully exploited as a UV blocker in cosmetics, avoiding undesired oxidative effects (UV adsorption in the range between 300-400 nm). Around the breaking point (TiO2:SS 1:1), the hybrid material preserves the high surface area of TiO2 (specific surface area around 180 m(2)/g) and demonstrates NOx depletion of up to 100% in 80 min, together with improved adhesion of hybrid antibacterial coating. The last design demonstrated the best results for the concurrent removal of inorganic, organic, and biological pollutants in water/soil remediation applications
HEMS inter-facility transfer: a case-mix analysis.
Helicopter emergency medical services (HEMS) are popular rescue systems despite inconsistent evidence in the scientific literature to support their use for primary interventions, as well as for inter-facility transfer (IFT). There is little research about IFT by HEMS, hence questions remain about the appropriateness of this method of transport. The aim of this study was to describe a case-mix of operational and medical characteristics for IFT activity of a sole HEMS base, and identify indicators of over-triage.
This is a retrospective study on HEMS IFT over 36 months, from January 1st 2013 to December 31st 2015. Medical and operational data from the database of the Emergency Department of Lausanne University Hospital, which provides the emergency physicians for this helicopter base, were reviewed. It included distance and time of flight transport, type of care during flight, and estimated distance of transport if conducted by ground.
There were 2194 HEMS missions including 979 IFT (44.6%). Most transfers involved adults (> 17 years old; 799 patients, 81.6%). Forty patients (4.1%) were classified as having benefitted from resuscitation or life-saving measures performed in flight, 615 (62.8%) from emergency treatment and 324 (33.1%) from simple clinical examination. The median distance by air between hospitals was 35.4 km. The estimated median distance by road was 47.7 km. The median duration time from origin to destination by air was 12 min.
This case-mix of IFTs by HEMS presents a high severity. There are many signs in favour of over-triage. We propose indicators to help choosing whether HEMS is the most appropriate mean of transport to perform the transfer regarding patient condition, geography, and medical competences available aboard ground ambulances; this may reduce over-triage
The Effect of Varied Doses of Nicotine on Some Morphometric Parameters of the Testis in Albino Wistar Rats
Background: This study is aimed at determining the effect of nicotine on male fertility by evaluating some morphometric parameters of male Wistar rat such as testicular weight, seminiferous tubule diameter, height of epithelium, sertoli-germ cell ratio and Johnsen’s score in order to assess the spermatogic index. Methods: 20 adult male rats were randomly divided into four groups, the test groups were administered with 0.2mg/100g, 0.4/100g and 0.6/100g body weight of nicotine base daily for 30 days using a polythene catheter orally , while the control were administered with 2mls 0.9%physiological saline. Histological slides of the testis were made and data obtained from the slides. Results: nicotine caused a significant reduction (P < 0.05) and (P< 0.01) in the mean values of the morphometric parameters of the test group compared with control. The Johnsen’s Score results were 5.4 ± 0.51*, 5.2 ± 0.37* for groups 2 and 3 respectively. For seminiferous tubule diameter the significant values were 208.0±4.22**, 184.5±11.31**in groups 2 and 3, whilst the height of epithelium showed a significant reduction of 153.9± 13.58*, 101.6 ± 1.35**, 76.6 ± 6.65** for groups 1, 2 and 3 respectively. Sertoli-germ cell ratio showed a significant reduction in groups 2 and 3 with values of 3.6 ± 0.77 ** , 1.5 ± 0.16**for groups 2, and 3 respectively, testicular weight showed significant reduction of 1.2±0.05**,1.1±0.05** for group 2 and 3.The results stated are only those that showed a significant reduction at 95% confidence level when compared with the control. Conclusion: It was concluded that nicotine exerted an adverse effects on the spermatogenic index with concomitant reduction in reproductive potentials of the male rat. Nicotine and nicotine- based products should therefore be taken with caution in cases of infertility in man and animal.
