    Revisión crí­tica del programa Desarrollo del Pensamiento I. Posgrado Medicina Familiar y Comunitaria

    Introducción: La asignatura Desarrollo del Pensamiento I, en el posgrado de medicina familiar tiene el propósito de lograr aprendizajes significativos y productivos, que optimicen las competencias adquiridas por el posgradista   de Medicina Familiar y Comunitaria en su vida cotidiana y profesional, activando los procesos mentales generales y especí­ficos para desarrollar las capacidades académicas, mediante el uso de estrategias y técnicas de aprendizaje, que cumplan las expectativas del posgradista. Objetivo: desarrollar una revisión crí­tica del actual syllabus de la asignatura en relación con las expectativas de los posgradistas, que nos permita formular sus transformaciones. Método: Se realizó un estudio cualitativo de carácter descriptivo, se emplearon métodos teóricos como el análisis y sí­ntesis sobre el syllabus de la asignatura y empí­ricos: encuesta y grupo nominal para criterio de especialistas conformado por cuatro profesores del colectivo en el posgrado de Medicina Familiar y Comunitaria, en la Universidad Católica Santiago de Guayaquil en Ecuador, durante el perí­odo junio-agosto del 2015, las variables de estudio para evaluar la calidad del programa fueron: pertinencia de los objetivos, estructura del sumario, correspondencia de las evaluaciones y actualización de la bibliografí­a, los resultados fueron analizados por técnicas descriptivas de frecuencias y por cientos. Resultados: Sobre el programa se obtuvo, 4 suficiencias y 4 insuficiencias: exceso de evaluaciones, insuficiente transdiciplinariedad y la bibliografí­a está desactualizada. Conclusiones: el syllabus debe ser reestructurado, la bibliografí­a actualizada y se recomienda la elaboración de un material docente para la orientación bibliográfica del alumnado

    A Statistical Approach to Macrofungal Diversity in a Mediterranean Ecosystem of the Iberian Peninsula Dominated by the Holm Oak (Quercus ilex L. subsp. ballota (Desf.) Samp.)

    The fungal biodiversity associated with a stable plant community appears to vary from year to year. To analyse the annual behaviour in the formation of fruiting bodies, a statistical study of the data obtained for 4 years (2009–2012) in an apparently undisturbed Mediterranean ecosystem dominated by the holm oak (Quercus ilex L. subsp. ballota (Desf.) Samp.), located in the Midwest of the Iberian Peninsula, was carried out. These data were related to the main meteorological variables. The 150 species collected showed a significant annual, monthly, and weekly difference in their fruiting during the collection period. All this implies a variation in the annual fungal fruiting which can modify the moment of when maximum peaks of fruiting appear, their duration, and the number of species that compose them. In addition, the results make it possible to establish an annual behaviour pattern, with sporocarp formation throughout the year and four fruiting groups (two of them in the dry season).     Granuloma Piógeno: Reporte de un caso de lesión hiperplásica reactiva con aspecto tumoral

    Pyogenic granuloma is a reactive lesion in response to different local factors, including traumatism, caries dental, hormonal imbalance or poor oral hygiene, which produces an inflammatory proliferation of connective tissue. Pyogenic granuloma is frequently located in oral cavity (gums, tongue, hard palate, lips, and floor of mouth) and skin. Clinically, it presents as a vascularized hyperplastic lesion with a slow-growing pedicled or sessile base of variable size and slow growth. Histologically, shows proliferation of endothelial tissue to a vascular network with signs of chronic inflammation and granulation tissue. This case report aims to identify the clinical, imaging and histopathological characteristics of Pyogenic Granuloma with tumor appearance in the oral cavity in an adult patient of the Clínica Dental Docente Cayetano Heredia in 2017. Clinical and histopathological characteristics of granuloma allow a specific diagnosis, given that their differential diagnoses are similar. The treatment options for pyogenic granuloma include excision, laser pulsed-dye, intralesional injection of ethanol or corticosteroids, sclerotherapy with sodium tetradecyl sulfate, and cryosurgery. Reactive hyperplasia may present a tumor appearance that is ruled out by anatomopathological study. Choosing the correct approach and treatment requires a proper clinical and histopathological diagnosis.El granuloma piógeno es una lesión reactiva en respuesta a diferentes factores locales, su etiología es por traumatismo, caries dental, desequilibrio hormonal, higiene oral deficiente, etc. que produce una proliferación inflamatoria del tejido conectivo, localizada frecuentemente en cavidad oral (encías, lengua, paladar duro, labios y piso de boca) y piel. Clínicamente se presenta como una lesión hiperplásica vascularizada con base pediculada o sésil de tamaño variable y crecimiento lento. Histológicamente presenta proliferación de tejido endotelial a una red vascular con signos de inflamación crónica y tejido de granulación. El presente caso clínico tiene como objetivo identifi car las características clínicas, imagenológicas e histopatológicas de Granuloma Piógeno con aspecto tumoral en la cavidad oral en un paciente adulto de la Clínica Dental Docente Cayetano Heredia en el año 2017. Las características clínicas e histopatológicas de granuloma permiten un diagnóstico concreto, dado que su diagnóstico diferencial es similar. Como opciones de tratamiento además de exéresis, tenemos el pulsed- dye laser, inyección intralesional de etanol o corticoides, escleroterapia con tetradecil sulfato de sodio y criocirugía. Las hiperplasias reactivas pueden presentar aspecto tumoral que se descarta con el estudio anatomopatológico. El abordaje y tratamiento requiere un diagnóstico clínico e histopatológico adecuado

    The distribution of cadmium in soil and cacao beans in Peru

    Peru is the eighth largest producer of cacao beans globally, but high cadmium contents are constraining access to international markets which have set upper thresholds for permitted concentrations in chocolate and derivatives. Preliminary data have suggested that high cadmium concentrations in cacao beans are restricted to specific regions in the country, but to date no reliable maps exist of expected cadmium concentrations in soils and cacao beans. Drawing on >2,000 representative samples of cacao beans and soils we developed multiple national and regional random forest models to develop predictive maps of cadmium in soil and cacao beans across the area suitable for cacao cultivation. Our model projections show that elevated concentrations of cadmium in cacao soils and beans are large Loreto, as well as some very localized pockets in the central departments of Huánuco and San Martin. Unsurprisingly, soil cadmium was the by far most important predictor of bean cadmium. Aside from the south-eastern to north-western spatial trend of increasing cadmium values in soils and beans, the most important predictors of both variables in nation-wide models were geology, rainfall seasonality, soil pH and rainfall. At regional level, alluvial deposits and mining operations were also associated with higher cadmium levels in cacao beans. Based on our predictive map of cadmium in cacao beans we estimate that while at a national level less than twenty percent of cacao farming households might be impacted by the cadmium regulations, in the most affected department of Piura this could be as high as 89%

    Impactos en la cadena de valor del cacao en Perú como consecuencia del Reglamento UE No. 488/2014

    Este informe hace parte de los productos del componente socioeconómico del proyecto Clima-LoCa. El documento presenta los resultados agregados de una serie de análisis y estudios desarrollados por miembros de la Alianza Bioversity International y CIAT, SENASA, INIA, MIDAGRI y las cooperativas Agroindustrial Cacao Alto Huallaga (CAICAH) y NorAndino durante los años 2020 y 2021 relacionados a los efectos del reglamento UE No 488/2014 en la cadena de valor de cacao en el Perú y las medidas adelantadas para mitigarlos. Primero se presenta una contextualización de la cadena del cacao y chocolate en el Perú, así como de la problemática del cadmio, las iniciativas adelantadas a nivel nacional para hacer frente a esta situación y los resultados de una revisión de la literatura disponible relacionada a los impactos del reglamento en la cadena. El informe presenta un análisis que combina diversas metodologías y fuentes, apoyándose en la multiplicidad de enfoques, fuentes de información y perspectivas de las organizaciones participantes. *Clima-LoCa es un proyecto regional liderado por la Alianza de Bioversity International y el Centro Internacional de Agricultura Tropical (CIAT), implementado en colaboración con socios en investigación de América Latina y Europa y financiado por la Comisión Europea.     El principal objetivo de este capitulo es dar a conocer la gran diversidad de cacaos que existen en el Perú para promover tanto su uso como su conservación. El capítulo demuestra que la mayoría de los diferentes cultivares tradicionales y grupos genéticos silvestres tienen una coherencia geográfica que permitirá la implementación de una sistema de denominación de origen para el cacao nativo peruano. Para poner en práctica dicho sistema es crítico tener identificado y tener acceso a materiales puros de cada grupo genético, para así poder apoyar a los agricultores, cooperativas y asociaciones, compradores, inversionistas o autoridades públicas, entre otros, en sus esfuerzos de producir y marquetear cacaos de calidad de origen nativo puro

    Active site specificity profiling of the matrix metalloproteinase family: Proteomic identification of 4300 cleavage sites by nine MMPs explored with structural and synthetic peptide cleavage analyses

    Secreted and membrane tethered matrix metalloproteinases (MMPs) are key homeostatic proteases regulating the extracellular signaling and structural matrix environment of cells and tissues. For drug targeting of proteases, selectivity for individual molecules is highly desired and can be met by high yield active site specificity profiling. Using the high throughput Proteomic Identification of protease Cleavage Sites (PICS) method to simultaneously profile both the prime and non-prime sides of the cleavage sites of nine human MMPs, we identified more than 4300 cleavages from P6 to P6′ in biologically diverse human peptide libraries. MMP specificity and kinetic efficiency were mainly guided by aliphatic and aromatic residues in P1′ (with a ~ 32–93% preference for leucine depending on the MMP), and basic and small residues in P2′ and P3′, respectively. A wide differential preference for the hallmark P3 proline was found between MMPs ranging from 15 to 46%, yet when combined in the same peptide with the universally preferred P1′ leucine, an unexpected negative cooperativity emerged. This was not observed in previous studies, probably due to the paucity of approaches that profile both the prime and non-prime sides together, and the masking of subsite cooperativity effects by global heat maps and iceLogos. These caveats make it critical to check for these biologically highly important effects by fixing all 20 amino acids one-by-one in the respective subsites and thorough assessing of the inferred specificity logo changes. Indeed an analysis of bona fide MEROPS physiological substrate cleavage data revealed that of the 37 natural substrates with either a P3-Pro or a P1′-Leu only 5 shared both features, confirming the PICS data. Upon probing with several new quenched-fluorescent peptides, rationally designed on our specificity data, the negative cooperativity was explained by reduced non-prime side flexibility constraining accommodation of the rigidifying P3 proline with leucine locked in S1′. Similar negative cooperativity between P3 proline and the novel preference for asparagine in P1 cements our conclusion that non-prime side flexibility greatly impacts MMP binding affinity and cleavage efficiency. Thus, unexpected sequence cooperativity consequences were revealed by PICS that uniquely encompasses both the non-prime and prime sides flanking the proteomic-pinpointed scissile bond

    Niemann-Pick C disease (NP-C) is a neurovisceral atypical lysosomal lipid storage disorder with an estimated minimal incidence of 1/120 000 live births. The broad clinical spectrum ranges from a neonatal rapidly fatal disorder to an adult-onset chronic neurodegenerative disease. The neurological involvement defines the disease severity in most patients but is typically preceded by systemic signs (cholestatic jaundice in the neonatal period or isolated spleno- or hepatosplenomegaly in infancy or childhood). The first neurological symptoms vary with age of onset: delay in developmental motor milestones (early infantile period), gait problems, falls, clumsiness, cataplexy, school problems (late infantile and juvenile period), and ataxia not unfrequently following initial psychiatric disturbances (adult form). The most characteristic sign is vertical supranuclear gaze palsy. The neurological disorder consists mainly of cerebellar ataxia, dysarthria, dysphagia, and progressive dementia. Cataplexy, seizures and dystonia are other common features. NP-C is transmitted in an autosomal recessive manner and is caused by mutations of either the NPC1 (95% of families) or the NPC2 genes. The exact functions of the NPC1 and NPC2 proteins are still unclear. NP-C is currently described as a cellular cholesterol trafficking defect but in the brain, the prominently stored lipids are gangliosides. Clinical examination should include comprehensive neurological and ophthalmological evaluations. The primary laboratory diagnosis requires living skin fibroblasts to demonstrate accumulation of unesterified cholesterol in perinuclear vesicles (lysosomes) after staining with filipin. Pronounced abnormalities are observed in about 80% of the cases, mild to moderate alterations in the remainder ("variant" biochemical phenotype). Genotyping of patients is useful to confirm the diagnosis in the latter patients and essential for future prenatal diagnosis. The differential diagnosis may include other lipidoses; idiopathic neonatal hepatitis and other causes of cholestatic icterus should be considered in neonates, and conditions with cerebellar ataxia, dystonia, cataplexy and supranuclear gaze palsy in older children and adults. Symptomatic management of patients is crucial. A first product, miglustat, has been granted marketing authorization in Europe and several other countries for specific treatment of the neurological manifestations.
