Quantum non-malleability and authentication
In encryption, non-malleability is a highly desirable property: it ensures
that adversaries cannot manipulate the plaintext by acting on the ciphertext.
Ambainis, Bouda and Winter gave a definition of non-malleability for the
encryption of quantum data. In this work, we show that this definition is too
weak, as it allows adversaries to "inject" plaintexts of their choice into the
ciphertext. We give a new definition of quantum non-malleability which resolves
this problem. Our definition is expressed in terms of entropic quantities,
considers stronger adversaries, and does not assume secrecy. Rather, we prove
that quantum non-malleability implies secrecy; this is in stark contrast to the
classical setting, where the two properties are completely independent. For
unitary schemes, our notion of non-malleability is equivalent to encryption
with a two-design (and hence also to the definition of Ambainis et al.). Our
techniques also yield new results regarding the closely-related task of quantum
authentication. We show that "total authentication" (a notion recently proposed
by Garg, Yuen and Zhandry) can be satisfied with two-designs, a significant
improvement over the eight-design construction of Garg et al. We also show
that, under a mild adaptation of the rejection procedure, both total
authentication and our notion of non-malleability yield quantum authentication

A repurposing strategy for Hsp90 inhibitors demonstrates their potency against filarial nematodes
Novel drugs are required for the elimination of infections caused by filarial worms, as most commonly used drugs largely target the microfilariae or first stage larvae of these infections. Previous studies, conducted in vitro, have shown that inhibition of Hsp90 kills adult Brugia pahangi. As numerous small molecule inhibitors of Hsp90 have been developed for use in cancer chemotherapy, we tested the activity of several novel Hsp90 inhibitors in a fluorescence polarization assay and against microfilariae and adult worms of Brugia in vitro. The results from all three assays correlated reasonably well and one particular compound, NVP-AUY922, was shown to be particularly active, inhibiting Mf output from female worms at concentrations as low as 5.0 nanomolar after 6 days exposure to drug. NVP-AUY922 was also active on adult worms after a short 24 h exposure to drug. Based on these in vitro data, NVP-AUY922 was tested in vivo in a mouse model and was shown to significantly reduce the recovery of both adult worms and microfilariae. These studies provide proof of principle that the repurposing of currently available Hsp90 inhibitors may have potential for the development of novel agents with macrofilaricidal properties
Immunohistochemical evaluation of human epidermal growth factor receptor 2 and estrogen and progesterone receptors in breast carcinoma in Jordan
INTRODUCTION: Although breast carcinoma (BC) is the most common malignancy affecting Jordanian females and the affected population in Jordan is younger than that in the West, no information is available on its biological characteristics. Our aims in this study are to evaluate the expression of estrogen receptor (ER) and progesterone receptor (PR) and Her-2/neu overexpression in BC in Jordan, and to compare the expression of these with other prognostic parameters for BC such as histological type, histological grade, tumor size, patients' age, and number of lymph node metastases. METHOD: This is a retrospective study conducted in the Department of Pathology at Jordan University of Science and Technology. A confirmed 91 cases of BC diagnosed in the period 1995 to 1998 were reviewed and graded. We used immunohistochemistry to evaluate the expression of ER, PR, and Her-2. Immunohistochemical findings were correlated with age, tumor size, grade and axillary lymph node status. RESULTS: Her-2 was overexpressed in 24% of the cases. The mean age of Her-2 positive cases was 42 years as opposed to 53 years among Her-2 negative cases (p = 0.0001). Her-2 expression was inversely related to ER and PR expression. Her-2 positive tumors tended to be larger than Her-2 negative tumors with 35% overexpression among T3 tumors as opposed to 22% among T2 tumors (p = 0.13). Her-2 positive cases tended to have higher rates of axillary metastases, but this did not reach statistical significance. ER and PR positive cases were seen in older patients with smaller tumor sizes. CONCLUSION: Her-2 overexpression was seen in 24% of BC affecting Jordanian females. Her-2 overexpression was associated with young age at presentation, larger tumor size, and was inversely related to ER and PR expression. One-fifth of the carcinomas were Her-2 positive and ER negative. This group appears to represent an aggressive form of BC presenting at a young age with large primary tumors and a high rate of four or more axillary lymph node metastases
A Taxonomy of Explainable Bayesian Networks
Artificial Intelligence (AI), and in particular, the explainability thereof,
has gained phenomenal attention over the last few years. Whilst we usually do
not question the decision-making process of these systems in situations where
only the outcome is of interest, we do however pay close attention when these
systems are applied in areas where the decisions directly influence the lives
of humans. It is especially noisy and uncertain observations close to the
decision boundary which results in predictions which cannot necessarily be
explained that may foster mistrust among end-users. This drew attention to AI
methods for which the outcomes can be explained. Bayesian networks are
probabilistic graphical models that can be used as a tool to manage
uncertainty. The probabilistic framework of a Bayesian network allows for
explainability in the model, reasoning and evidence. The use of these methods
is mostly ad hoc and not as well organised as explainability methods in the
wider AI research field. As such, we introduce a taxonomy of explainability in
Bayesian networks. We extend the existing categorisation of explainability in
the model, reasoning or evidence to include explanation of decisions. The
explanations obtained from the explainability methods are illustrated by means
of a simple medical diagnostic scenario. The taxonomy introduced in this paper
has the potential not only to encourage end-users to efficiently communicate
outcomes obtained, but also support their understanding of how and, more
importantly, why certain predictions were made
Comparing the probability of stroke by the Framingham risk score in hypertensive Korean patients visiting private clinics and tertiary hospitals
<p>Abstract</p> <p>Background</p> <p>The purpose of this study was to investigate the pattern of distribution of risk factors for stroke and the 10-year probability of stroke by the Framingham risk score in hypertensive patients visiting private clinics vs. tertiary hospitals.</p> <p>Methods</p> <p>A total of 2,490 hypertensive patients who attended 61 private clinics (1088 patients) and 37 tertiary hospitals (1402 patients) were enrolled. The risk factors for stroke were evaluated using a series of laboratory tests and physical examinations, and the 10-year probability of stroke was determined by applying the Framingham stroke risk equation.</p> <p>Results</p> <p>The proportion of patients who had uncontrolled hypertension despite the use of antihypertensive agents was 49% (66 and 36% of patients cared for at private clinics and tertiary hospitals, respectively; p < 0.001). The average 10-year probability of stroke by the Framingham risk score in hypertensive patients was 21% (approximately 2.2 times higher than of the risk of stroke in the Korean Cancer Prevention Study [KCPS] cohort) and was higher in patients attending tertiary hospitals compared to private clinics (16 and 24% of patients attending private clinics and tertiary hospitals, respectively; p < 0.001).</p> <p>Conclusions</p> <p>Since the 10-year probability of stroke by the Framingham risk score in hypertensive patients attending tertiary hospitals was higher than the risk for patients attending private clinics. We suggest that the more aggressive interventions are needed to prevent and early detect an attack of stroke in hypertensive patients attending tertiary hospitals.</p
Prevalence, evaluation and management of overactive bladder in primary care
<p>Abstract</p> <p>Background</p> <p>Patients with overactive bladder (OAB) are under-diagnosed in the primary care setting. Primary care physicians (PCP) approach to the patient and appropriate patient disclosure may contribute to under-diagnosis.</p> <p>Methods</p> <p>An outpatient primary care setting was used to determine the prevalence and characteristics of OAB. Patients who visited the family medicine outpatient clinic were invited to answer a self-administered questionnaire. It included questions on evidence of lower urinary tract symptoms (modified Overactive Bladder-Validated 8-question Screener [OAB-V8]), relevant medical and surgical history, and demographic data. Relationship between OAB and other independent variables were analyzed using chi-square and risk ratio (RR) analysis.</p> <p>Results</p> <p>Of 325 questionnaires distributed, 311 were returned completed. Patients ranged from 18 to 97 years, the majority women (74.0%) and African American (74.3%). OAB was present in 60.5% of men and 48.3% of women (<it>p </it>= 0.058). OAB was significantly associated with obesity (BMI > = 30) in women (<it>p </it>= 0.018, RR = 1.72), specifically obese premenopausal women (age < 55 years) (<it>p </it>= 0.011, RR = 1.98).</p> <p>Conclusion</p> <p>OAB prevalence is more than double and higher in men than previously reported. The relative risk for OAB is significantly greater in obese premenopausal women.</p
