Response Generation in Longitudinal Dialogues: Which Knowledge Representation Helps?
Longitudinal Dialogues (LD) are the most challenging type of conversation for
human-machine dialogue systems. LDs include the recollections of events,
personal thoughts, and emotions specific to each individual in a sparse
sequence of dialogue sessions. Dialogue systems designed for LDs should
uniquely interact with the users over multiple sessions and long periods of
time (e.g. weeks), and engage them in personal dialogues to elaborate on their
feelings, thoughts, and real-life events. In this paper, we study the task of
response generation in LDs. We evaluate whether general-purpose Pre-trained
Language Models (PLM) are appropriate for this purpose. We fine-tune two PLMs,
GePpeTto (GPT-2) and iT5, using a dataset of LDs. We experiment with different
representations of the personal knowledge extracted from LDs for grounded
response generation, including the graph representation of the mentioned events
and participants. We evaluate the performance of the models via automatic
metrics and the contribution of the knowledge via the Integrated Gradients
technique. We categorize the natural language generation errors via human
evaluations of contextualization, appropriateness and engagement of the user
Plant diversity in old-growth woods: the case of the forest edges of the Favorita Park in Palermo (north-western Sicily, Italy)
This article presents the results of a study on plant diversity at different levels in residual forest stands, located in the historical Favorita Park in Palermo, Italy (established and named in 1799 by King Ferdinand IV of the House of Bourbon). These forest aspects have naturally evolved for over two centuries, under minimal conditions of anthropogenic disturbance (e.g. deforestation, fires, grazing activities, etc.). This is especially true in the area known as “Bosco Niscemi”, spread over about 8.5 hectares, in the centre of the park. Bosco Niscemi is characterized by the widespread presence of old trees, abundant necromass and litter. In this study, four different soil profiles were analysed, and classified as follows: (i) Solimovic Regosol (Arenic); ii) Eutric Arenosol (Chromic); (iii) and (iv) Skeletic Regosol (Ochric). From a phytosociological point of view, four forest communities have been identified, two of which are described as new associations (Viburno tini-Phillyreetum latifoliae ass. nova and Teucrio flavi-Phillyreetum latifoliae ass. nova). The species richness was also found to be noteworthy, with the co-occurrence of several taxa (phanerogams and cryptogams) that are of biogeographic interest or rare in Sicily, including: i) vascular flora (e.g. Viburnum tinus and Arbutus unedo, both very rare throughout the western sector of Sicily); ii) bryophytes [Cryphaea heteromalla (new record of a very rare species in Sicily) as well as Hypnum cupressiforme and Leptodon smithii, also never previously found at such low altitudes]; iii) lichens (e.g. Bacidia rosella, Gyalecta derivata, Ramalina roesleri and Waynea stoechadiana); iv) mushrooms (e.g. Eichleriella leucophaea, only known location in Italy). Based on the scientific documentation produced in this study, these woods can be reported as “old-growth forests” to be included in the “National Network” that has been recently established in Italy (Ministerial Decree of 23 June 2023). This area might serve as an ideal control for urban environmental studies, given its pristine ecological setting
Nafion-layered silicate nanocomposite membrane for fuel cell application
Direct methanol fuel cells (DMFCs) using a proton exchange membrane as electrolyte is an attractive option for electricity generation. The most widely used membrane in the DMFC system is based on a perfluorinated polymer bearing sulfonic acid functions like Nafion®. The latter combines chemical, mechanical and thermal stability and high protonic conductivity but shows elevated methanol permeability. We propose the preparation of a novel type of hybrid membranes to tentatively solve this problem. This innovative material results from the homogeneous dispersion of a nano-scaled inorganic filler within Nafion. The filler consists of stacks of negatively charged alumino-silicate layers (Cloisite), with a positive counter-ion in the interlamellar space. The purpose of the addition of this filler is to decrease methanol diffusion through the polymer membrane without decreasing too much the ionic conductivity
The Prognostic Value of F-FDG PET Imaging at Staging in Patients with Malignant Pleural Mesothelioma: A Literature Review
Malignant pleural mesothelioma (MPM) is an aggressive malignancy, frequently diagnosed at locally-advanced/metastatic stages. Due to a very poor prognosis and limited treatment options, the need to identify new prognostic markers represents a great clinical challenge. The prognostic role of metabolic information derived from Positron Emission Tomography (PET) with F-Fluoro-deoxy-glucose (F-FDG) has been investigated in different MPM settings, however with no definitive consensus. In this comprehensive review, the prognostic value of FDG-PET imaging exclusively performed at staging in MPM patients was evaluated, conducting a literature search on PubMed/MEDLINE from 2010 to 2020. From the 19 selected studies, despite heterogeneity in several aspects, staging FDG-PET imaging emerges as a valuable prognostic biomarker, with higher tumor uptake predictive of worse prognosis, and with volumetric metabolic parameters like Metabolic Tumor Volume, (MTV) and Total Lesion Glycolisis (TLG) performing better than SUVmax. However, PET uptake parameters were not always confirmed as independent prognostic factors, especially in patients previously treated with pleurodesis and with a non-epithelioid histotype. Future prospective studies in larger and clinically homogeneous populations, and using more standardized methods of PET images analysis, are needed to further validate the value of staging FDG-PET in the prognostic MPM stratification, with a potential impact on better patient-tailored treatment planning, in the perspective of personalized medicine
High-intensity interval training improves bone remodeling, lipid profile, and physical function in multiple sclerosis patients
Multiple sclerosis (MS) is a demyelinating and neurodegenerative disease due to an autoimmune chronic inflammatory response, yet the etiology is currently not completely understood. It is already known that physical activity plays an essential role in improving quality of life, especially in neuropathological conditions. The study was aimed to investigate the possible benefits of high-intensity interval training (HIIT) in bone and lipid metabolism markers, and neuromotor abilities in MS patients. 130 participants were recruited; 16 subjects with MS met the inclusion criteria and were included in the data analysis. The patients were randomly assigned to two groups: a Control group (CG) (34.88 +/- 4.45 yrs) that didn't perform any physical activity and the Exercise group (EG) (36.20 +/- 7.80 yrs) that performed HIIT protocol. The training program was conducted remotely by a kinesiologist. It was performed three times a week for 8 weeks. At the beginning (T0) and the end of the study (T1) physical function tests, bone remodelling markers, and lipid markers analyses were performed. After 8 weeks of training the wall squat (s) (T0 = 27.18 +/- 4.21; T1 = 41.68 +/- 5.38, p <= 0.01) and Time Up and Go test (s) (T0 = 7.65 +/- 0.43; T1 = 6.34 +/- 0.38 p <= 0.01) performances improved; lipid markers analysis showed a decrease in Total (mg/dl) (T0 = 187.22 +/- 15.73; T1 = 173.44 +/- 13.03, p <= 0.05) and LDL (mg/dl) (T0 = 108 +/- 21.08; T1 = 95.02 +/- 17.99, p < 0.05) cholesterol levels. Additionally, the levels of osteocalcin (g/L), a marker of bone formation increased (T0 = 20.88 +/- 4.22; T1 = 23.66 +/- 6.24, p < 0.05), 25-OH Vitamin D (g/L) improved after 8 weeks (T0 = 21.11 +/- 7.11; T1 = 27.66 +/- 7.59, p < 0.05). HIIT had an effect on lower limb strength and gait control, improved bone formation, and lipid management, in MS patients
Obesity and Circulating Levels of Vitamin D before and after Weight Loss Induced by a Very Low-Calorie Ketogenic Diet
Background: Vitamin D plays a pivotal role in calcium and phosphorus metabolism, also influencing bone tissue. Several studies have reported that vitamin D blood levels were significantly lower in people with obesity, probably due to its uptake by the adipose tissue. Clinical studies that investigated the changes of circulating levels of vitamin D following weight loss reported controversial data. A very low-calorie ketogenic diet is acknowledged as a reliable treatment to achieve a rapid weight loss. Therefore, we investigated the effect of weight loss, consequent to a very low-calorie ketogenic diet, on vitamin D blood concentrations. Methods: A cohort of 31 people with obesity underwent a very low-calorie ketogenic diet for 10-12 weeks. The serum concentrations of vitamin D, parathormone, calcium and phosphorous were measured before and after weight loss; they were compared to a control group of 20 non-obese, non-diabetic, age- and gender-matched persons. Results: Patients with obesity had a higher habitual intake of vitamin D than the control group (p < 0.05). However, the vitamin D blood levels of the obese group were significantly lower than those of the control group (p < 0.005) and they increased after weight loss (p < 0.001). At baseline, vitamin D blood concentrations of the persons with obesity were significantly correlated with both fat mass-kg (r = -0.40; p < 0.05) and body mass index (r = -0.47; p < 0.01). Following very low-calorie ketogenic diet, the change in vitamin D serum concentrations was correlated only with the change in fat mass-kg (r = -0.43; p < 0.01). Conclusion: This study confirmed that patients with obesity have lower vitamin D levels that normalize after significant weight loss, supporting the hypothesis that vitamin D is stored in the adipose tissue and released following weight loss
The Phenotypic Characterization of the Cammalleri Sisters, an Example of Exceptional Longevity
This article shows demographic, clinical, anamnestic, cognitive, and functional data as well as biochemical, genetic, and epigenetic parameters of two exceptional siblings: Diega (supercentenarian) and Filippa (semisupercentenarian) Cammalleri. The purpose of this study is to provide new insights into the extreme phenotypes represented by semisupercentenarians and supercentenarians. Different studies have been published on supercentenarians, but to the best of our knowledge, this is the only concerning two sisters and the most detailed from a phenotypic point of view. Our findings agree with the suggestion that supercentenarians have an increasing relative resistance to age-related diseases, approximating the limits of the functional human reserve to address successfully the acute causes of death. More interestingly, our data agree with, and extend, the suggestion that inflammation and oxidative stress predict centenarian mortality
Cure indicators and prevalence by stage at diagnosis for breast and colorectal cancer patients: A population‐based study in Italy
People alive many years after breast (BC) or colorectal cancer (CRC) diagnoses are increasing. This paper aimed to estimate the indicators of cancer cure and complete prevalence for Italian patients with BC and CRC by stage and age. A total of 31 Italian Cancer Registries (47% of the population) data until 2017 were included. Mixture cure models allowed estimation of net survival (NS); cure fraction (CF); time to cure (TTC, 5-year conditional NS >95%); cure prevalence (who will not die of cancer); and already cured (prevalent patients living longer than TTC). 2.6% of all Italian women (806,410) were alive in 2018 after BC and 88% will not die of BC. For those diagnosed in 2010, CF was 73%, 99% when diagnosed at stage I, 81% at stage II, and 36% at stages III-IV. For all stages combined, TTC was >10 years under 45 and over 65 years and for women with advanced stages, but <= 1 year for all BC patients at stage I. The proportion of already cured prevalent BC women was 75% (94% at stage I). Prevalent CRC cases were 422,407 (0.7% of the Italian population), 90% will not die of CRC. For CRC patients, CF was 56%, 92% at stage I, 71% at stage II, and 35% at stages III-IV. TTC was <= 10 years for all age groups and stages. Already cured were 59% of all prevalent CRC patients (93% at stage I). Cancer cure indicators by stage may contribute to appropriate follow-up in the years after diagnosis, thus avoiding patients' discrimination
Trends in Net Survival from Vulvar Squamous Cell Carcinoma in Italy (1990–2015)
Objective: In many Western countries, survival from vulvar squamous cell carcinoma (VSCC) has been stagnating for decades or has increased insufficiently from a clinical perspective. In Italy, previous studies on cancer survival have not taken vulvar cancer into consideration or have pooled patients with vulvar and vaginal cancer. To bridge this knowledge gap, we report the trend in survival from vulvar cancer between 1990 and 2015. (2) Methods: Thirty-eight local cancer registries covering 49% of the national female population contributed the records of 6274 patients. Study endpoints included 1- and 2-year net survival (NS) calculated using the Pohar-Perme estimator and 5-year NS conditional on having survived two years (5|2-year CNS). The significance of survival trends was assessed with the Wald test on the coefficient of the period of diagnosis, entered as a continuous regressor in a Poisson regression model. (3) Results: The median patient age was stable at 76 years. One-year NS decreased from 83.9% in 1990–2001 to 81.9% in 2009–2015 and 2-year NS from 72.2% to 70.5%. Five|2-year CNS increased from 85.7% to 86.7%. These trends were not significant. In the age stratum 70–79 years, a weakly significant decrease in 2-year NS from 71.4% to 65.7% occurred. Multivariate analysis adjusting for age group at diagnosis and geographic area showed an excess risk of death at 5|2-years, of borderline significance, in 2003–2015 versus 1990–2002. (4) Conclusions: One- and 2-year NS and 5|2-year CNS showed no improvements. Current strategies for VSCC control need to be revised both in Italy and at the global level
