    Serratia marcescens in a neonatal intensive care unit: two long-term multiclone outbreaks in a 10-year observational study

    We investigated two consecutive Serratia marcescens (S. marcescens) outbreaks which occurred in a neonatal intensive care unit (NICU) of a tertiary level hospital in North Italy in a period of 10 years (January 2003-December 2012). Risk factors associated with S. marcescens acquisition were evaluated by a retrospective case-control study. A total of 21,011 clinical samples was examined: S. marcescens occurred in 127 neonates: 43 developed infection and 3 died. Seven clusters were recorded due to 12 unrelated clones which persisted for years in the ward, although no environmental source was found. The main epidemic clone A sustaining the first cluster in 2003 reappeared in 2010 as an extended spectrum ?-lactamase (ESBL)-producing strain and supporting the second epidemic. Birth weight, gestational age, use of invasive devices and length of stay in the ward were significantly related to S. marcescens acquisition. The opening of a new ward for non-intensive care-requiring neonates, strict adherence to alcoholic hand disinfection, the timely identification and isolation of infected and colonized neonates assisted in containing the epidemics. Genotyping was effective in tracing the evolution and dynamics of the clones demonstrating their long-term persistence in the ward

    Acute appendicitis or gynecological disease? The role of videolaparoscopic approach

    Background: The laparoscopic approach for suspected appendicitis is increasingly gaining acceptance even if its remain controversial. Many Authors suggest the usefulness of laparoscopic approach in women of reproductive age because of the high rate of wrong diagnosis for gynecological diseases.Material and Methods: during a period of 2 years at our Institution 130 patients underwent to appendectomy for suspected acute appendicitis. Of those 40 were female in reproductive age and underwent to laparoscopic procedure. In 36 cases a preoperative gynecological examination was carried out and was negative. Results: in all cases the procedure was completed laparoscopically and the postoperative complication rate was 0%. The definitive diagnosis was: acute appendicitis in 15cases (37,5%), complicated appendicitis with abscess or peritonitis in 15 cases (37,5%) and gynecological unknown disease in 10 cases (25%) associated with chronic appendicitis. In the last group of patients both appendicitis and gynecological disease were treated in the same procedure.Conclusions: laparoscopic appendectomy is to be considered as safe as open and if in the male patients it hasn\u2019t got obvious advantages, in women of reproductive age is able to perform a correct differential diagnosis with pelvic diseases and it permit to treat different disorders during a sole procedure and with the same mini-invasive accesses. The high rate of false negative in the diagnosis of pelvic diseases in the preoperative diagnostic tests justified the systematic use of laparoscopic approach in female patients with suspected acute appendicitis

    A comparative study of a surgical method and scaling and root planing using the Odontoson(R)

    This investigation compared a traditional periodontal surgical method with a non-surgical treatment of scaling and root planing by an ultrasonic device (Odontoson M(R)) using irrigation with an iodised solution. 8 adult patients with periodontal disease were each treated at 2 randomly chosen quadrants by a Widman flap type surgical technique, and the remaining non-surgically. The patients then attended bi-monthly oral hygiene sessions over a period of 1 year. At that stage, clinical evaluation revealed that the Odontoson M(R) plus an ionized irrigant solution achieved a statistically comparable outcome to that of surgical treatment, even in the pockets initially up to 7 mm in depth
