    Does the catechol-O-methyltransferase (COMT) Val158Met human polymorphism in influence procrastination?

    Genetic studies are enlightening how the expression of several genes influences neuronal activity and all facets of human normal and abnormal behaviour. Among these, a growing body of information shows that a few key genes regulating activity of central neurotransmitters have specific roles in cognitive and/or emotional processes, as ‘procrastination’. We investigated the association of the 5-HTTLPR and COMT Val158Met polymorphisms with students’ procrastination in an academic writing task. Results: showed no relationship between procrastination and the 5-HTT polymorphism but they revealed an association with the COMT Val158Met one. Particularly, the presence of the Met158 allele was found to be significantly associated with the tendency to initiate and complete the assigned task. We hypothesize that the role of central monoamines and of dopamine already identified in impulsive behaviour, extends to procrastination. Since the 158Met allele provides neurons with significantly higher basal dopamine levels when compared to the 158Val allele, our observation suggests that under normal conditions the 158Met allele provides carriers with increased inhibitory control, resulting in an increased tendency to adhere to a planned schedule and therefore reducing procrastination. On the other hand, the Val158 allele may result more effective in increasing carriers’ performances under stress conditions, namely when the schedule deadline is approaching, and dopamine release is increased. This would result in a higher tendency to procrastinate. This hypothesis can readily be tested by applying the experimental approach here employed to various samples of subjects belonging to different categories and extending the analysis to other putative neuron-expressed gene

    Pandemic 2009 H1N1 virus infection associated with purpuric skin lesions: a case report

    <p>Abstract</p> <p>Introduction</p> <p>The influenza virus infection may be severe in non-immune people. Common complications of influenza virus include upper and lower respiratory tract infections, otitis media, myocarditis, acute respiratory distress syndrome and multi-organ failure. There have been cases of vasculitis following influenza vaccination, and rash and acute purpura may occur in certain viral infections. To the best of our knowledge, there are no reports concerning cases of systemic vasculitis associated with pandemic 2009 (H<sub>1</sub>N<sub>1</sub>) infection.</p> <p>Case presentation</p> <p>A 23-year-old Caucasian woman was hospitalized at the "L. Spallanzani" National Institute for Infectious Diseases in Rome, Italy. Clinical and radiological features including laboratory findings of this case are illustrated. Notably, the patient had fever, severe abdominal pain, hematuria, arthritis, and purpuric manifestations associated with a normal platelet count. Nasopharyngeal and rectal swabs revealed pandemic 2009 (H<sub>1</sub>N<sub>1</sub>) virus by reverse-transcriptase-polymerase-chain-reaction assay. Routine laboratory analyses showed elevated inflammatory parameters. The autoimmune panel tests were normal. Steroid therapy associated with oseltamivir achieved an evident and rapid improvement. On day seven the patient chose to leave the hospital against medical advice.</p> <p>Conclusion</p> <p>Complications related to influenza infection can be life threatening, particularly in immunocompromised patients. Henoch-Schönlein purpura triggered by the novel influenza virus infection could be an attractive pathogenetic hypothesis. We have discussed both the diagnosis and the challenge of therapy protocols. Steroid therapy is part of the management of severe vasculitis. Our case suggests that steroid therapy associated with antivirals can prevent the risk of further complications such as hemorrhage and multi-organ failure during severe vasculitis, without enhancing the virulence of the influenza virus. The possible role of pandemic 2009 (H<sub>1</sub>N<sub>1</sub>) in the pathogenesis of hemorrhagic manifestations should be further investigated.</p

    Membrane-seeded autologous chondrocytes: cell viability and characterization at surgery

    The implantation of chondrocytes, seeded on matrices such as hyaluronic acid or collagen membranes, is a method that is being widely used for the treatment of chondral defects. The aim of the present study was to evaluate the distribution, viability and phenotype expression of the cells seeded on a collagen membrane just at the time of the implantation. Twelve patients who were suffering from articular cartilage lesions were treated by the MACI(Âź) procedure. The residual part of each membrane was tested by colorimetric assay (MTT) and histochemical and ultrastructural analyses were carried out. In all of the samples a large number of viable cells, quite homogenously distributed, was detected. The cells expressed the markers of the differentiated hyaline chondrocytes. These data reassure in that the MACI procedure provides a suitable engineered tissue for cartilage repair, in line with the clinical evidences emerging in the literature

    Improvement of Tuberculosis Laboratory Capacity on Pemba Island, Zanzibar: A Health Cooperation Project.

    Low-income countries with high Tuberculosis burden have few reference laboratories able to perform TB culture. In 2006, the Zanzibar National TB Control Programme planned to decentralize TB diagnostics. The Italian Cooperation Agency with the scientific support of the "L. Spallanzani" National Institute for Infectious Diseases sustained the project through the implementation of a TB reference laboratory in a low-income country with a high prevalence of TB. The implementation steps were: 1) TB laboratory design according to the WHO standards; 2) laboratory equipment and reagent supplies for microscopy, cultures, and identification; 3) on-the-job training of the local staff; 4) web- and telemedicine-based supervision. From April 2007 to December 2010, 921 sputum samples were received from 40 peripheral laboratories: 120 TB cases were diagnosed. Of all the smear-positive cases, 74.2% were culture-positive. During the year 2010, the smear positive to culture positive rate increased up to 100%. In March 20, 2010 the Ministry of Health and Social Welfare of Zanzibar officially recognized the Public Health Laboratory- Ivo de Carneri as the National TB Reference Laboratory for the Zanzibar Archipelago. An advanced TB laboratory can represent a low cost solution to strengthen the TB diagnosis, to provide capacity building and mid-term sustainability

    The Challenge of Planning Conservation Strategies in Threatened Seascapes: Understanding the Role of Fine Scale Assessments of Community Response to Cumulative Human Pressures

    Assessing the distribution and intensity of human threats to biodiversity is a prerequisite for effective spatial planning, harmonizing conservation purposes with sustainable development. In the Mediterranean Sea, the management of Marine Protected Areas (MPAs) is rarely based on explicit consideration of the distribution of multiple stressors, with direct assessment of their effects on ecosystems. This gap limits the effectiveness of protection and is conducive to conflicts among stakeholders. Here, a fine scale assessment of the potential effects of different combinations of stressors (both land- and marine-based) on vulnerable rocky habitats (i.e. lower midlittoral and shallow infralittoral) along 40 km of coast in the western Mediterranean (Ionian Sea) has been carried out. The study area is a paradigmatic example of socio-ecological interactions, where several human uses and conservation measures collide. Significant differences in the structure of assemblages according to different combinations of threats were observed, indicating distinct responses of marine habitats to different sets of human pressures. A more complex three-dimensional structure, higher taxon richness and \u3b2-diversity characterized assemblages subject to low versus high levels of human pressure, consistently across habitats. In addition, the main drivers of change were: closeness to the harbour, water quality, and the relative extension of beaches. Our findings suggest that, although efforts to assess cumulative impacts at large scale may help in individuating priority areas for conservation purposes, the fact that such evaluations are often based on expert opinions and not on actual studies limits their ability to represent real environmental conditions at local scale. Systematic evaluations of local scale effects of anthropogenic drivers of change on biological communities should complement broad scale management strategies to achieve effective sustainability of human exploitation of marine resources

    Ultrasonic vs Drill Implant Site Preparation: Post-Operative Pain Measurement Through VAS, Swelling and Crestal Bone Remodeling: A Randomized Clinical Study.

    Background: Piezosurgery is a surgical procedure that is able to perform osteotomies by a micrometric and selective cut of the bone tissue. The objective of this investigation was to evaluate two different techniques; an ultrasonic device, and a drill approach for implant site preparation. Methods: A total of fifty patients were recruited for the randomized clinical trial to receive dental implants for fixed prosthetic restoration in the posterior mandible and were allotted to two groups. In Group A the implant site was prepared following a drilling technique, while in Group B the implant site was prepared using an ultrasonic device; moreover, the operative duration was recorded. Postoperative pain and swelling were evaluated at 1, 2, 4, and 6 days. The crestal bone resorption was measured at 3 months from implant placement by a three-dimensional tomography evaluation. Results: The findings suggest that osteotomies performed by an ultrasonic device cause less pain and swelling. On the other hand, the piezoelectric preparation was characterized by a significative increase in the operative time. No statistical differences in crestal bone resorption were evident in the two different approaches. Conclusions: According to the outcome of the study, ultrasonic implant bed preparation can be used with success in implantology and could be considered a suitable alternative to traditional drilling techniques for dental fixture placement

    Radial interpolation of GPS and leveling data of ground deformation in a resurgent caldera: application to Campi Flegrei (Italy)

    This study presents a new method, called the Radial Interpolation Method, to interpolate data characterized by an approximately radial pattern around a relatively constrained central zone, such as the ground deformation patterns shown in many active volcanic areas. The method enables the fast production of short-term deformation maps on the base of spatially sparse ground deformation measurements and can provide uncertainty quantification on the interpolated values, fundamental for hazard assessment purposes and deformation source reconstruction. The presented approach is not dependent on a priori assumptions about the geometry, location and physical properties of the source, except for the requirement of a locally radial pattern, i.e., allowing multiple centers of symmetry. We test the new method on a synthetic point source example, and then, we apply the method to selected time intervals of real geodetic data collected at the Campi Flegrei caldera during the last 39 years, including examples of leveling, Geodetic Precise Traversing measurements and Global Positioning System. The maps of horizontal displacement, calculated inland, show maximum values lying along a semicircular annular region with a radius of about 2–3 km in size. This semi-annular area is marked by mesoscale structures such as faults, sand dikes and fractures. The maps of vertical displacement describe a linear relation between the maximum vertical uplift measured and the volume variation. The multiplicative factor in the linear relation is about 0.3 × 106 m3/cm if we estimate the proportion of the ΔV that is captured by the GPS network onland and we use this to estimate the full ΔV. In this case, the 95% confidence interval on K because of linear regression is ± 5%. Finally, we briefly discuss how the new method could be used for the production of short-term vent opening maps on the base of real-time geodetic measurements of the horizontal and vertical displacements

    Spur cells in liver cirrhosis are predictive of acute-on-chronic liver failure and liver-related mortality regardless of severe anaemia

    Lay SummaryChronic anaemia is a frequent finding in liver cirrhosis and spur cell anaemia has been shown to be an uncommon non-immune haemolytic disease typically related to advanced-liver disease. In our study, spur cell anaemia (spur cells &gt;5%) was found in almost 10% of outpatient cirrhotics and was significantly related to more decompensated disease, higher incidence of ACLF and 1 year liver-related mortality. More importantly, the vast majority of patients with high percentage of spur cells did not have severe anaemia. Therefore, spur cells should be searched for in patients with advanced liver disease by a simple blood smear evaluation, even in the absence of significant anaemia, because of relevant prognostic impact and in order to prioritize patients to intensive management and possibly liver transplantation.Chronic anaemia in advanced liver disease is a frequent finding. The aim was to explore the clinical impact of spur cell anaemia, a rare entity typically associated with end-stage of the disease. One-hundred and nineteen patients (73.9% males) with liver cirrhosis of any etiology were included. Patients with bone marrow diseases, nutrients deficiencies and hepatocellular carcinoma were excluded. In all patients, a blood sample was collected to check for the presence of spur cells on blood smear. A complete blood biochemical panel was recorded together with Child-Pugh (CP) score and Model for End-Stage Liver Disease (MELD) score. For each patients, clinically relevant events, such as acute-on-chronic liver failure (ACLF) and 1 year liver-related mortality, were registered. Patients were then grouped according to the percentage of spur cells at smear (&gt; 5%, 1-5%, &lt; 1%). Severe anaemia was defined as haemoglobin levels lower than 8 g/dL. 9.2% of subjects had &gt; 5% spur cells, only 2 had evidence of haemolysis. In patients with &gt; 5% spur cells, haemoglobin and albumin were lower compared with the other sub-group, while MELD score, CP score, International Normalized Ratio, ferritin, creatinine and unconjugated bilirubin were higher. Patients with more spur cells were more decompensated and developed more frequently ACLF. ACLF and liver-related mortality were significantly and independently associated with the presence of &gt; 5% spur cells but not with baseline severe anaemia. Cirrhotic patients have a fairly high prevalence of spur cells, not always associated with severe haemolytic anaemia. The presence of spur red cells is per se associated with a worse prognosis and, therefore, should be always evaluated to prioritize patients for intensive management and eventually liver transplantation

    The application of the Weight-Of-Evidence approach for an integrated ecological risk assessment of marine protected sites

    The effective management of marine ecosystems in the face of growing anthropogenic pressures requires the integration of data from different ecological components. Holistic approaches to evaluate the ecological status of marine ecosystems are still scarce, likely due to the challenge of integrating the complex information from a variety of indicators. In this study, we provided an application of a quantitative Weight-Of-Evidence (WOE) model based on the Sediqualsoft ¼ software, combining environmental and biological data to assess ecological risk in soft-bottom habitats within Natura sites 2000 in the Northern Adriatic Sea (Mediterranean Sea). Here, the WOE approach combined three lines of evidence (LOE): chemical characterization (LOE1), ecotoxicological properties (LOE4), and benthic community status (LOE5). A separate hazard quotient was derived for each LOE prior to a weightedintegration into a synthetic WOE assessment. The chemical analysis of the sediments revealed concentrations of pollutants far lower the reference limits, except for As and Hg and for polycyclic aromatic hydrocarbons which determined a ‘Slight’ to ‘Severe’ chemical hazard in coastal sites. Ecotoxicological hazard was rated as ‘Absent ’ at all sampling stations, and the analysis of benthic communities indicated ‘undisturbed’ conditions for most sites. The WOE approach classified the overall ecological risk to be ‘Absent’ for offshore sites and ‘Slight’ in nearshore sites. Although results suggested a general low ecological risk, the potential for future risks is recognized, especially in coastal areas, due to well-known sediment pollution in the region. The application of the WOE approach may represent a valuable tool for managing marine protected sites, and to char acterize the overall ecological status of these areas and improve conservation strategies in highly anthropized environmental contexts
