    HRCT findings of pulmonary sarcoidosis; relation to pulmonary function tests

    BACKGROUND: Chest-X-ray has several limitations in detecting the extent of pulmonary disease in sarcoidosis. It might not reflect the degree of pulmonary involvement in patients with sarcoidosis when compared to computed tomography of the thorax. We aimed to investigate the HRCT findings of pulmonary sarcoidosis and to find out the existence of possible relations between HRCT findings and PFTs. In addition, we aimed to investigate the accordance between HRCT findings and conventional chest-X-ray staging of pulmonary sarcoidosis. METHOD: 45 patients with sarcoidosis with a mean age 29.7+/− 8.4 years were evaluated. Six of them were female and 39 were male. The type, distribution and extent of the parameters on HRCT/CTs were evaluated and scored. Chest-X-rays were evaluated for the stage of pulmonary sarcoidosis. Correlations were investigated between HRCT/CT parameter scores, Chest X-Ray stages and pulmonary function parameters. RESULTS: Nodule, micronodule, ground glass opacity and consolidation were the most common HRCT findings. There were significant correlations between pulmonary function parameters, HRCT pattern scores, and chest-X-ray stages. A significant correlation between chest-x-ray score and total HRCT score was found. CONCLUSIONS: Pulmonary sarcoidosis patients might have various pulmonary parenchymal changes on HRCT. Thorax HRCT was superior to chest-X-ray in detecting pulmonary parenchymal abnormalities. The degree of pulmonary involvement might be closely related to the loss of pulmonary function measured by PFTs. Chest-X-ray is considered to have a role in the evaluation of pulmonary sarcoidosis

    Relation of maternal vitamin D status with gestational diabetes mellitus and perinatal outcome

    Objective: To investigate the relationship between maternal vitamin D status and glucose intolerance, and its impact on pregnant women and their newborns. Methods: A cohort of pregnant women were divided into three groups: women with gestational diabetes mellitus, ones with normal results both after the 50 gr and 100 gr OGTT (CG-1) and ones having a positive result after the 50 gr OGTT screening but negative results for gestational diabetes mellitus (GDM) after the 100 gr OGTT (CG-2) Results: The newborn length in CG-1 was greater than in GDM and CG-2 (p= 0.002 and p= 0.02). Fasting blood glucose and insulin resistance (IR) were negatively correlated with length of the newborns (r=-0.3, p=0.03 and r=-0.3, p=0.01). The newborns of women with GDM had lower APGAR-1 and 5 scores than those of CG-1 and CG-2 (APGAR-1 p= 0.001 and p= 0.004, APGAR-5 p=0.005 and p=0.007, respectively). APGAR scores were correlated negatively with IR (APGAR-1 r=- 0.32, p=0.01, APGAR-5 r=-0.3, p=0.03) and positively with 25OHD levels (APGAR-1 r=0.3, p=0.01, APGAR-5 r=0.3, p=0.02). Conclusion: Vitamin D deficiency, gestational diabetes and insulin resistance are interrelated. Severe vitamin D deficiency during pregnancy is associated with poor pregnancy and neonatal outcome

    The Influence of The Armenian Diaspora on The American Foreign Policy

    After the weakening of Turkish-Armenian relations and intensive American missionary activities, Armenians began to leave their homelands for educational, economic and political reasons. Emigration to the United States intensified in particular in response to the 1915 Techir (Relocation and Resettlement) Law. After achieving political rights in the United States, Armenian immigrants formed groups that began trying to influence U.S. government policy, working to win recognition of the alleged Armenian Genocide, financial assistance for Armenia, and other policies favorable to Armenia. The process that began resolution the alleged Armenia Genocide was removed from the historical dimension and moved to the political dimension and an international policy instrument was created. In the United States, the Armenian Diaspora strives to influence government policy systematically through diaspora organizations that carefully follow the international scene and advocate for United States foreign policies in favor of Armenia. In this study, the ultimate aims of the Armenia Diaspora over American politics and policy-making, and the activities of the Armenian Lobby will be examined

    Modeling Loss-of-Flow Accidents and Their Impact on Radiation Heat Transfer

    Long-term high payload missions necessitate the need for nuclear space propulsion. The National Aeronautics and Space Administration (NASA) investigated several reactor designs from 1959 to 1973 in order to develop the Nuclear Engine for Rocket Vehicle Application (NERVA). Study of planned/unplanned transients on nuclear thermal rockets is important due to the need for long-term missions. In this work, a system model based on RELAP5 is developed to simulate loss-of-flow accidents on the Pewee I test reactor. This paper investigates the radiation heat transfer between the fuel elements and the structures around it. In addition, the impact on the core fuel element temperature and average core pressure was also investigated. The following expected results were achieved: (i) greater than normal fuel element temperatures, (ii) fuel element temperatures exceeding the uranium carbide melting point, and (iii) average core pressure less than normal. Results show that the radiation heat transfer rate between fuel elements and cold surfaces increases with decreasing flow rate through the reactor system. However, radiation heat transfer decreases when there is a complete LOFA. When there is a complete LOFA, the peripheral coolant channels of the fuel elements handle most of the radiation heat transfer. A safety system needs to be designed to counteract the decay heat resulting from a post-LOFA reactor scram

    Steam generator performance improvements for integral small modular reactors

    Background: Steam generator (SG) is one of the significant components in the nuclear steam supply system. A variety of SGs have been designed and used in nuclear reactor systems. Every SG has advantages and disadvantages. A brief account of some of the existing SG designs is presented in this study. A high surface to volume ratio of a SG is required in small modular reactors to occupy the least space. In this paper, performance improvement for SGs of integral small modular reactor is proposed. Aims/Methods: For this purpose, cross-grooved microfins have been incorporated on the inner surface of the helical tube to enhance heat transfer. The primary objective of this work is to investigate thermal–hydraulic behavior of the proposed improvements through modeling in RELAP5-3D. Results and Conclusions: The results are compared with helical-coiled SGs being used in IRIS (International Reactor Innovative and Secure). The results show that the tube length reduces up to 11.56% keeping thermal and hydraulic conditions fixed. In the case of fixed size, the steam outlet temperature increases from 590.1 K to 597.0 K and the capability of power transfer from primary to secondary also increases. However, these advantages are associated with some extra pressure drop, which has to be compensated

    Videoendoscopic single-port axillary dissection

    Videoendoscopy is newly used in breast and axillary surgery. Single-port surgery is one of the newest methods of minimally invasive surgery. This report describes the first case of videoendoscopic single-port axillary dissection. In histopathological evaluation, 24 lymph nodes were identified and one node was infiltrated by the cancer cells. Videoendoscopic single-port axillary dissection is a precise and improvable technique. Single-port videoendoscopic axillary dissection could be more feasible with individual tools that will be designed for minimally invasive breast surgery
