    Management of Esophageal Carcinoma Associated with Cirrhosis: A Retrospective Case-Control Analysis

    Objectives. Esophageal carcinoma and cirrhosis have the overlapping etiologic factors. Methods. In a retrospective analysis conducted in 2 Breton institutions we wanted to asses the frequency of this association and the outcome of these patients in a case-control study where each case (cirrhosis and esophageal cancer) was paired with two controls (esophageal cancer). Results. In a 10-year period, we have treated 958 esophageal cancer patients; 26 (2.7%) had a cirrhosis. The same treatments were proposed to the 2 groups; cases received nonsignificantly different radiation and chemotherapy dose than controls. Severe toxicities and deaths were more frequent among the cases. At the end of the treatment 58% of the cases and 67% of the controls were in complete remission; median and 2-year survival were not different between the 2 groups. All 4 Child-Pugh B class patients experienced severe side effects and 2 died during the treatment. Conclusions. This association is surprisingly infrequent in our population! Child-Pugh B patients had a dismal prognosis and a bad tolerance to radiochemotherapy; Child-Pugh A patients have the same tolerance and the same prognosis as controls and the evidence of a well-compensated cirrhosis has not modified our medical options

    Génération automatisée d'un livre numérique "sans perte sémantique"

    International audienceIn this paper, we start by considering the “lossless semantic” eBook: a closed package of content that includes the representation of the logical structure of the book and the semantic representation of the content, which can be processed by reading systems. But producing a lossless semantic eBook can be time-consuming and expensive. Therefore we propose the guidelines of an automated generation based on an initial publishing XML workflow. We experiment this approach to produce a French wine guide in EPUB format, the widely adopted standard compatible with modern reading devices and tablet applications. We introduce the automated generation of the lossless semantic EPUB from the XML source of the book using patterns written in Schematron and XSLT transformations. We conclude with a discussion of the results and the future steps of this work.Dans cet article, nous commençons par définir ce qu’est un livre numérique « sans perte sémantique » : un ensemble cloisonné de contenus, enrichi par la représentation de la structure logique du livre et par la représentation sémantique des données, pouvant être exploitées par un système de lecture. Mais produire un tel livre numérique peut être coûteux en temps et en argent. Par conséquent nous proposons une génération automatisée basée sur un workflow XML d’édition préexistant. Nous expérimentons cette approche en produisant un guide des vins au format EPUB, un standard largement répandu et compatible avec les applications et les appareils de lecture. Nous expérimentons une génération d’un EPUB sans perte sémantique à partir du fichier XML source du guide en utilisant des patrons écrits en Schematron et des transformations XSLT. Nous concluons avec une discussion de nos résultats et les étapes à venir de ce travail

    Study of scale effect in an internal erosion mechanism: centrifuge model and energy analysis

    International audienceSuffusion is an internal erosion mechanism, which means detachment and transport of fine particles within the soil skeleton due to hydraulic seepage flows. Different researchers have observed that the value of the critical hydraulic gradient required to initiate suffusion decreases with the length of the specimen tested. A specific centrifuge bench was designed to study the suffusion process and to study this scale effect under a controlled effective stress. Clayey sand specimens were subjected to centrifuge acceleration and to a downward flow under a constant hydraulic head. The study underlines the influence of specimen length on critical hydraulic gradient and also on the rate of erosion. A new energy analysis of tests is developed, linking the erosion rate to the power expended by fluid flow, and the eroded clay mass to the energy dissipation. This method permits the effect of specimen length to be avoided.La suffusion est un mécanisme d'érosion interne qui correspond au détachement et au transport de particules fines au travers du squelette granulaire sous l'action d'un écoulement. Différents chercheurs ont observé que la valeur du gradient hydraulique critique nécessaire à l'initiation de la suffusion décroît avec la longueur de l'échantil-lon testé. Un dispositif d'essais en centrifugeuse est spécialement développé afind e caractériser la suffusion et d'étudier cet effet d'échelle sous contrainte effective con-trôlée.     Centrifuge modelling of an internal erosion mechanism

    International audienceSuffusion is an internal erosion mechanism, which means the transport of fine particles within the soil due to hydraulic interflows. With the objective to characterize this mechanism and to study a scale effect presented in literature, a specific centrifuge bench is designed. Test specimens of mixture of sand and kaolinite are compacted in rigid walled cells. The specimen height is selected according to the applied gravity and the chosen effective stress. After saturation, specimens are subjected to centrifugal acceleration and to a vertical downward flow under a constant hydraulic head. The effect of the applied hydraulic gradient is examined and the study underlines that the increase of specimen height induces the decrease of critical hydraulic gradient and the increase of erosion rate. A new energy analysis of tests is developed, linking the expended power to the erosion rate. This method permits to avoid the effect of specimen height
