    Comparison of the effects of salmeterol and ipratropium bromide on exercise performance and breathlessness in patients with stable chronic obstructive pulmonary disease

    AbstractTo compare the effects of salmeterol, an adrenergic drug, and ipratropium bromide, an anticholinergic drug, on breathlessness and gas exchange during exercise in patients with chronic obstructive lung disease (COPD), we performed a progressive treadmill exercise test on 15 patients on 3 days (24 h apart), after inhalation placebo, ipratropium bromide (120 μg) or salmeterol (50 μg) in a randomized fashion. Dyspnoea during exercise was evaluated from the regression slope between Borg scale (BS) scores and distance walked each minute on the treadmill. The regression was expressed as the distance walked at BS score 5, the threshold load of dyspnoea (TLD) and breakpoint load of dyspnoea. During and after the exercise, oxygen saturation was monitored by pulse oxymeter and we measured the lower SaO2 during exercise and the recovery time of SaO2 after exercise.In comparison to placebo inhalation we found the same small but significant improvement in airflow limitation after salmeterol or ipratropium inhalation, also the distance walked on treadmill increased after bronchodilators. After bronchodilators the magnitude of oxyhaemoglobin desaturation with exercise was similar to that observed after placebo but the duration of the recovery from sustained SaO2 desaturation after exercise was shorter to the same extent as after ipratropium or salmeterol. Dyspnoeic sensation, when assessed by the TLD and by the distance walked at BS score 5, was decreased after salmeterol and after ipratropium bromide to a similar extent.We conclude that the salmeterol, when given in conventional doses, produces significant improvement in the airway obstruction in the recovery of postexercise HbO2 desaturation and in dyspnoeic sensation in patients with COPD, effects which were similar to those observed after inhalation of the anticholinergic agent ipratropium bromide

    Adult tissue-derived neural crest‐like stem cells: Sources, regulatory networks, and translational potential: Concise review

    Neural crest (NC) cells are a multipotent stem cell population that gives rise to a diverse array of cell types in the body, including peripheral neurons, Schwann cells (SC), craniofacial cartilage and bone, smooth muscle cells, and melanocytes. NC formation and differentiation into specific lineages takes place in response to a set of highly regulated signaling and transcriptional events within the neural plate border. Pre‐migratory NC cells initially are contained within the dorsal neural tube from which they subsequently emigrate, migrating to often distant sites in the periphery. Following their migration and differentiation, some NC‐like cells persist in adult tissues in a nascent multipotent state, making them potential candidates for autologous cell therapy. This review discusses the gene regulatory network responsible for NC development and maintenance of multipotency. We summarize the genes and signaling pathways that have been implicated in the differentiation of a post‐migratory NC into mature myelinating SC. We elaborate on the signals and transcription factors involved in the acquisition of immature SC fate, axonal sorting of unmyelinated neuronal axons, and finally the path toward mature myelinating SC, which envelope axons within myelin sheaths, facilitating electrical signal propagation. The gene regulatory events guiding development of SC in‐vivo provides insights into means for differentiating NC‐like cells from adult human tissues into functional SC, which have the potential to provide autologous cell sources for the treatment of demyelinating and neurodegenerative disorders

    Avian Host-Selection by Culex pipiens in Experimental Trials

    Evidence from field studies suggests that Culex pipiens, the primary mosquito vector of West Nile virus (WNV) in the northeastern and north central United States, feeds preferentially on American robins (Turdus migratorius). To determine the contribution of innate preferences to observed preference patterns in the field, we conducted host preference trials with a known number of adult female C. pipiens in outdoor cages comparing the relative attractiveness of American robins with two common sympatric bird species, European starling, Sternus vulgaris and house sparrow, Passer domesticus. Host seeking C. pipiens were three times more likely to enter robin-baited traps when with the alternate host was a European starling (n = 4 trials; OR = 3.06; CI [1.42–6.46]) and almost twice more likely when the alternative was a house sparrow (n = 8 trials; OR = 1.80; CI = [1.22–2.90]). There was no difference in the probability of trap entry when two robins were offered (n = 8 trials). Logistic regression analysis determined that the age, sex and weight of the birds, the date of the trial, starting-time, temperature, humidity, wind-speed and age of the mosquitoes had no effect on the probability of a choosing a robin over an alternate bird. Findings indicate that preferential feeding by C. pipiens mosquitoes on certain avian hosts is likely to be inherent, and we discuss the implications innate host preferences may have on enzootic WNV transmission

    Η μυΐαση είναι η τέταρτη πιο κοινή δερματική ασθένεια στον άνθρωπο που σχετίζεται με ταξίδια ενώ η δερματική μυΐαση είναι η συνηθέστερη κλινική μορφή. Αναφέρεται σε μη-ιατρογενή μόλυνση των ιστών από προνύμφες διπτέρων εντόμων και εμφανίζεται κυρίως σε ασθενείς που έχουν κακή υγιεινή, με συνθήκες κακής στέγασης, διανοητική καθυστέρηση, προχωρημένη ηλικία, διαβήτη, ακινητοποίηση και χαμηλό ανοσοποιητικό σύστημα. Ασθενής 14 ετών, ο οποίος είχε υποβληθεί πριν 6 μήνες σε εξωτερική οστεοσύνθεση μηριαίου λόγω κατάγματος διάφυσης μηριαίου, προσήλθε στα επείγοντα ιατρεία με άλγος στην περιοχή των βελονών. Κατά την εξέταση του τραύματος διαπιστώθηκε ερυθρότητα, οίδημα αλλά και η παρουσία ζωντανών προνυμφών διπτέρων εντόμων. Αφού ελέγχθηκε ακτινολογικά το κάταγμα της διάφυσης αφαιρέθηκε σε πρώτο χρόνο το σώμα της εξωτερικής οστεοσύνθεσης και έγινε σχολαστικότατος καθαρισμός. Με κατάλληλα χειρουργικά εργαλεία, απομονώθηκαν όλες οι προνύμφες από το τραύμα και τοποθετήθηκαν σε κατάλληλο υπόστρωμα για περαιτέρω εξέταση και προσδιορισμό του είδους. Έπειτα από μικροσκοπική εξέταση των προνυμφών και με βάση κατάλληλες μορφολογικές κλείδες προσδιορίστηκαν ως προνύμφες 3ης ηλικίας του είδους Lucilia sericata (Meigen) (Diptera: Calliphoridae). Έπειτα από 7 ημέρες αντιβιοτικής αγωγής ευρέως φάσματος, κλινοστατισμού, σχολαστικής περιποίησης των τραυμάτων και αφού είχαμε σαφή βελτίωση της περιοχής γύρω από τις βελόνες έγινε αφαίρεση αυτών. Κατά τις επανεξετάσεις έως και 6 μήνες μετά, τόσο κλινικά όσο και ακτινολογικά τα αποτελέσματα ήταν πολύ καλά.On examination of the wound of a patient who was treated previously with external fixation for a diaphysial fracture of the femur, we observed the presence of multiple living larvae (maggots) of dipterous flies. Treatment included removal of the maggots and use of antibiotic. The importance of hygiene and sanitation is discussed

While axonal transport dysfunction is thought to represent a primary pathogenic factor in AD and other neurodegenerative diseases, the direct molecular link between pathogenic forms of tau and deficits in axonal transport remain unclear. Recently, we demonstrated that filamentous, but not soluble, forms of wild-type tau inhibit anterograde, kinesin-based fast axonal transport (FAT) by activating axonal protein phosphatase 1 (PP1) and glycogen synthase kinase 3 (GSK3), independent of microtubule binding. Here, we demonstrate that amino acids 2–18 of tau, comprising a phosphatase-activating domain (PAD), are necessary and sufficient for activation of this pathway in axoplasms isolated from squid giant axons. Various pathogenic forms of tau displaying increased exposure of PAD inhibited anterograde FAT in squid axoplasm. Importantly, immunohistochemical studies using a novel PAD-specific monoclonal antibody in human postmortem tissue indicated that increased PAD exposure represents an early pathogenic event in AD that closely associates in time with AT8 immunoreactivity, an early marker of pathological tau. We propose a model of pathogenesis in which disease-associated changes in tau conformation lead to increased exposure of PAD, activation of PP1-GSK3, and inhibition of FAT.     Analysis on the clinical features of 22 basaloid squamous cell carcinoma of the lung

    <p>Abstract</p> <p>Background</p> <p>Basaloid squamous cell carcinoma of the lung is a rare and highly malignant tumor mostly observed in the proximal bronchi. Basaloid squamous cell carcinoma of the lung cases typically show rapid clinical progression, very poor prognosis and special pathological morphology. This project aimed to examine the clinical features of basaloid squamous cell carcinoma of the lung and the factors related to its prognosis; and to compare survival outcomes between basaloid squamous cell carcinoma and poorly differentiated squamous cell carcinomas (PDSC).</p> <p>Methods</p> <p>Between January 2004 and December 2008, pathological sections from basaloid squamous cell carcinoma and PDSC of the lung were collected and retrospectively analyzed at Tianjin Medical University Cancer Institute and Hospital. Data analysis was performed using Statistical Package for the Social Sciences (SPSS11.0). The Kaplan-Meier method was used to calculate the survival rate. Log-rank test was used to compare the differences in survival rate between the two groups. The factors influencing prognosis were analyzed using the Cox proportional hazard model.</p> <p>Results</p> <p>A total of 120 pathological sections were used in the analysis of this study-22 from basaloid squamous cell carcinoma cases and 98 from PDSC cases. Compared to the PDSC group, the basaloid squamous cell carcinoma group had a larger proportion of female patients (p = 0.001); however it had higher proportion of male smokers (p = 0.003). There were no statistically significant differences in survival rate between the two groups (χ<sup>2 </sup>= 1.200, p = 0.273). Additionally, prognosis of basaloid squamous cell carcinoma is significantly influenced by treatment mode and clinical stages of the tumor. The post-operation mortality hazard of patients treated with a combination chemotherapy and radiotherapy was 1.296 times higher than other treatment modes (<it>p </it>= 0.025). Increases in post-operation mortality hazard ratio were also associated with more advanced clinical stage of tumors (χ<sup>2 </sup>trend = 11.907, <it>p </it>= 0.000).</p> <p>Conclusions</p> <p>This study demonstrated that basaloid squamous cell carcinoma and PDSC have very similar clinical features, and there are no significant differences in survival rates between the two groups. Hence, we conclude that in the short term, the same clinical treatments and therapeutic modes can be administered to patients with basaloid squamous cell carcinoma and PDSC of the lung.</p
