    La teorĂ­a de la libertad de Hartmann en el contexto de su filosofĂ­a y en relaciĂłn con la kantiana

    Este trabajo pretende exponer la estructura ontolĂłgica fundamental del sistema filosĂłfico de Nicolai Hartmann desde un punto de vista tanto doxogrĂĄfico como ontolĂłgico, intentando, por tanto, encontrar temas filosĂłficos entre las exposiciones histĂłricas. De acuerdo con el volumen y la complejidad de la materia, este trabajo se centra en el problema de la libre voluntad, que es el fundamento del sistema Ă©tico expuesto por Hartmann en su monumental Ética y, quizĂĄ, tambiĂ©n el problema en el que mĂĄs tesis ontolĂłgicas encontramos implicadas. El armazĂłn ontolĂłgico de la obra de Hartmann y su teorĂ­a sobre la libertad son tratadas de conectar aquĂ­ no solo con su contexto histĂłrico-filosĂłfico, sino tambiĂ©n con todo el sistema de filosofĂ­a de Hartmann, ya que pretendiĂł la construcciĂłn de un sistema abarcante, en constante consideraciĂłn de la historia, las ciencias naturales y sociales y la filosofĂ­a contemporĂĄnea a Ă©l. De este modo, el sistema filosĂłfico de Hartmann se pone en relaciĂłn con sus principales raĂ­ces histĂłrico-filosĂłficas, tales como Kant, el neokantismo, la fenomenologĂ­a y el aristotelismo y lo mismo sucede con su teorĂ­a de la libertad, la cual es comparada con la de Kant y referida a la nociĂłn de valor, la principal en la Ă©tica fenomenolĂłgica.This work aims to expound the primary ontological framework of Nicolai Hartmann’s philosophical system both a doxographical and ontological point of view, so it tries to find philosophical issues in between the historical explanations. Attending to the length and the intricacy of the matter this work focuses on the free will problem, which is the ground of the ethical system presented by Hartmann in his huge work Ethics and maybe also it is the problem in which we can find more ontological thesis involved. Both the general ontological framework of Hartmann’s work and his theory about liberty are tried to be connected not only with their historical and philosophical context but also with the whole Hartmann’s system of philosophy because he aimed to develop a comprehensive one in constant viewing of history, natural and social sciences, and his contemporary philosophy. So the philosophical system of Hartmann is connected with its main historical philosophical roots such as Kant and neo-Kantianism, phenomenology and Aristotelianism and the same holds for his theory of freedom, which is compared with Kant’s one and referred to the notion of value, the main one in the phenomenological ethics.Universidad de Sevilla. EstadĂ­sticamente los terceros molares son los elementos dentarios que con mayor frecuencia sufren el fracaso eruptivo, por causas embriolĂłgicas, mecĂĄnicas o sistĂ©micas. Es comĂșn que los jĂłvenes no valoren su salud bucodental, y por ende, esta problemĂĄtica, ya que sus demandas sociales se encaminan hacia otras prioridades. Objetivos: Fomentar la concientizaciĂłn social acerca de la retenciĂłn de terceros molares y desarrollar programas masivos de prevenciĂłn, con participaciĂłn comunitaria integral con fuerte componente preventivo, que garantice impacto en la salud estudiantil, generando espacios integradores entre la Facultad de OdontologĂ­a y otros niveles educativos. MetodologĂ­a: La metodologĂ­a es participativa, con actividades tipo taller y otras modalidades creativas, promoviendo el compromiso de alumnos participantes, estimulados para interactuar entre sus pares y entorno cercano, promoviendo el conocimiento de la retenciĂłn de los terceros molares y sus connotaciones patolĂłgicas.     Integration of longitudinal deep-radiomics and clinical data improves the prediction of durable benefits to anti-PD-1/PD-L1 immunotherapy in advanced NSCLC patients

    Background Identifying predictive non-invasive biomarkers of immunotherapy response is crucial to avoid premature treatment interruptions or ineffective prolongation. Our aim was to develop a non-invasive biomarker for predicting immunotherapy clinical durable benefit, based on the integration of radiomics and clinical data monitored through early anti-PD-1/PD-L1 monoclonal antibodies treatment in patients with advanced non-small cell lung cancer (NSCLC).MethodsIn this study, 264 patients with pathologically confirmed stage IV NSCLC treated with immunotherapy were retrospectively collected from two institutions. The cohort was randomly divided into a training (n = 221) and an independent test set (n = 43), ensuring the balanced availability of baseline and follow-up data for each patient. Clinical data corresponding to the start of treatment was retrieved from electronic patient records, and blood test variables after the first and third cycles of immunotherapy were also collected. Additionally, traditional radiomics and deep-radiomics features were extracted from the primary tumors of the computed tomography (CT) scans before treatment and during patient follow-up. Random Forest was used to implementing baseline and longitudinal models using clinical and radiomics data separately, and then an ensemble model was built integrating both sources of information.ResultsThe integration of longitudinal clinical and deep-radiomics data significantly improved clinical durable benefit prediction at 6 and 9 months after treatment in the independent test set, achieving an area under the receiver operating characteristic curve of 0.824 (95% CI: [0.658,0.953]) and 0.753 (95% CI: [0.549,0.931]). The Kaplan-Meier survival analysis showed that, for both endpoints, the signatures significantly stratified high- and low-risk patients (p-value< 0.05) and were significantly correlated with progression-free survival (PFS6 model: C-index 0.723, p-value = 0.004; PFS9 model: C-index 0.685, p-value = 0.030) and overall survival (PFS6 models: C-index 0.768, p-value = 0.002; PFS9 model: C-index 0.736, p-value = 0.023).ConclusionsIntegrating multidimensional and longitudinal data improved clinical durable benefit prediction to immunotherapy treatment of advanced non-small cell lung cancer patients. The selection of effective treatment and the appropriate evaluation of clinical benefit are important for better managing cancer patients with prolonged survival and preserving quality of life

    The impact of surgical delay on resectability of colorectal cancer: An international prospective cohort study

    AimThe SARS-CoV-2 pandemic has provided a unique opportunity to explore the impact of surgical delays on cancer resectability. This study aimed to compare resectability for colorectal cancer patients undergoing delayed versus non-delayed surgery.MethodsThis was an international prospective cohort study of consecutive colorectal cancer patients with a decision for curative surgery (January-April 2020). Surgical delay was defined as an operation taking place more than 4 weeks after treatment decision, in a patient who did not receive neoadjuvant therapy. A subgroup analysis explored the effects of delay in elective patients only. The impact of longer delays was explored in a sensitivity analysis. The primary outcome was complete resection, defined as curative resection with an R0 margin.ResultsOverall, 5453 patients from 304 hospitals in 47 countries were included, of whom 6.6% (358/5453) did not receive their planned operation. Of the 4304 operated patients without neoadjuvant therapy, 40.5% (1744/4304) were delayed beyond 4 weeks. Delayed patients were more likely to be older, men, more comorbid, have higher body mass index and have rectal cancer and early stage disease. Delayed patients had higher unadjusted rates of complete resection (93.7% vs. 91.9%, P = 0.032) and lower rates of emergency surgery (4.5% vs. 22.5%, P ConclusionOne in 15 colorectal cancer patients did not receive their planned operation during the first wave of COVID-19. Surgical delay did not appear to compromise resectability, raising the hypothesis that any reduction in long-term survival attributable to delays is likely to be due to micro-metastatic disease
