    An Attempt to Improve Stance Mechanics of Trans-Tibial Amputee Gait by the Design of a Modular Ankle Joint Prosthetic

    Background: A-priori research shows that trans-tibial (TT) amputees display poor gait parameters when walking with low-cost ankle-foot prosthetics (here referred to as baseline AFP’s). This has drastic implications for the amputee populations in the developing world specifically, as they have limited access to advanced prosthetic technologies. Low-cost AFP’s are unable to adequately replicate natural stance mechanics, and reliance on these devices results in increased energy expenditure, osteoarthritis and lower-limb joint deterioration. Methodology: This project details the design of a novel ankle joint prosthetic (AJP) that serves as an attachment to baseline AFP’s, with the aim of facilitating better stance mechanics via the restoration of ankle joint mechanisms. The work is presented in three core sections: Part 1 explains the rationale as to why adequately replicating natural stance mechanics is an appropriate need; Part 2 presents the design of the modular low-cost AJP that utilises only simple mechanical elements; and Part 3 presents the experimental quantification of the impact the AJP has on stance mechanics of a baseline AFP (Otto Bock 1D10) in a simulation of the TT amputee walking gait cycle, via the use of three able-bodied participants and a pseudo-prosthesis. Results: The results indicate that the AJP significantly improves the stance mechanics of the baseline AFP. During forefoot rollover a stable joint moment and an increase in joint range of motion (RoM) was observed, yielding a decrease in ankle stiffness. During initial weight acceptance of early stance, an increase in joint RoM displays the restoration of controlled plantarflexion, which indicates an improved transition from heelstrike to footflat. This is a critical mechanism that facilitates stability control during weight acceptance, and the results suggest that the designed AJP is performing better in this regard than its closest functional competitor. However, equipment errors limited the ability to accurately report on ankle stiffness of this phase. Conclusions: Overall the final conclusions are that the designed AJP improves rollover shapes of the baseline AFP, eases phase transitions, and facilitates stability control and forward tibial progression. In combination with the low cost price (±50 USD), its ease of assembly and modular design, the AJP is thus a preferable option for low-income amputees in developing countries. Finally, there is significant evidence of functional and mechanical reliability, and therefore testing of the device can progress to a clinical study involving amputee participants

    Increased intestinal carbonate precipitate abundance in the sea bream (Sparus aurata L.) in response to ocean acidification

    Marine fish contribute to the carbon cycle by producing mineralized intestinal precipitates generated as by-products of their osmoregulation. Here we aimed at characterizing the control of epithelial bicarbonate secretion and intestinal precipitate presence in the gilthead sea bream in response to predicted near future increases of environmental CO2. Our results demonstrate that hypercapnia (950 and 1800 μatm CO2) elicits higher intestine epithelial HCO3- secretion ex vivo and a subsequent parallel increase of intestinal precipitate presence in vivo when compared to present values (440 μatm CO2). Intestinal gene expression analysis in response to environmental hypercapnia revealed the up-regulation of transporters involved in the intestinal bicarbonate secretion cascade such as the basolateral sodium bicarbonate co-transporter slc4a4, and the apical anion transporters slc26a3 and slc26a6 of sea bream. In addition, other genes involved in intestinal ion uptake linked to water absorption such as the apical nkcc2 and aquaporin 1b expression, indicating that hypercapnia influences different levels of intestinal physiology.     Regulation of bicarbonate secretion in marine fish intestine by the calcium-sensing receptor

    In marine fish, high epithelial intestinal HCO3- secretion generates luminal carbonate precipitates of divalent cations that play a key role in water and ion homeostasis. The present study was designed to expose the putative role for calcium and the calcium-sensing receptor (CaSR) in the regulation of HCO3- secretion in the intestine of the sea bream (Sparus aurata L.). Effects on the expression of the CaSR in the intestine were evaluated by qPCR and an increase was observed in the anterior intestine in fed fish compared with unfed fish and with different regions of intestine. CaSR expression reflected intestinal fluid calcium concentration. In addition, anterior intestine tissue was mounted in Ussing chambers to test the putative regulation of HCO3- secretion in vitro using the anterior intestine. HCO3- secretion was sensitive to varying calcium levels in luminal saline and to calcimimetic compounds known to activate/block the CaSR i.e., R 568 and NPS-2143. Subsequent experiments were performed in intestinal sacs to measure water absorption and the sensitivity of water absorption to varying luminal levels of calcium and calcimimetics were exposed as well.     Objectives: Herbal medicine is known as a valid alternative treatment. Salvia Reuterana, which has been used in the Iranian traditional medicine, is mostly distributed in the central highlands of Iran. Salvia Reuterana is a medicinal herb with various therapeutic usages. The aim of the present review is to take account of pharmacological properties of Salvia Reuterana. Materials and Methods: The present review summarizes the literature with respect to various pharmacological properties of Salvia Reuterana. Results: Salvia Reuterana possesses neurological, antimicrobial, antioxidant, chemotherapeutic, and antidiabetic properties. Conclusions: Salvia Reuterana can be used as an alternative for treatment of several disorders

    Behavioral responses around conspecific corpses in adult eastern gorillas (gorilla beringei spp.)

    Humans were once considered unique in having a concept of death but a growing number of observations of animal responses to dying and dead conspecifics suggests otherwise. Complex arrays of behaviors have been described ranging from corpse removal and burial among social insects to quiet attendance and caregiving among elephants and primates. Less frequently described, however, are behavioral responses of individuals from different age/sex classes or social position toward the death of conspecifics. We describe behavioral responses of mountain gorillas (Gorilla beringei beringei) to the deaths of a dominant silverback and a dominant adult female from the same social group in Volcanoes National Park in Rwanda and the responses of Grauer's gorillas (Gorilla b. graueri) to the corpse of an extra-group silverback in Kahuzi-Biega National Park, Democratic Republic of Congo. In gorillas, interactions between groups or with a lone silverback often result in avoidance or aggression. We predicted that: (i) more individuals should interact with the corpses of same-group members than with the corpse of the extra-group silverback; (ii) adult females with infants should avoid the corpse of the extra-group silverback; and (iii) in the mountain gorilla cases, individuals that shared close social relationships with the dead individual should spend more time with the corpse than other individuals in the group. We used a combination of detailed qualitative reports, photos, and videos to describe all occurrences of affiliative/investigative and agonistic behaviors observed at the corpses. We observed similar responses toward the corpses of group and extra-group individuals. Animals in all three cases showed a variety of affiliative/investigative and agonistic behaviors directed to the corpses. Animals of all age/sex classes interacted with the corpses in affiliative/investigative ways but there was a notable absence of all adult females at the corpse of the extra-group silverback. In all three cases, we observed only silverbacks and blackbacks being agonistic around and/or toward the corpses. In the mountain gorilla cases, the individuals who spent the most time with the corpses were animals who shared close social relationships with the deceased. We emphasize the similarity in the behavioral responses around the corpses of group and extra-group individuals, and suggest that the behavioral responses were influenced in part by close social relationships between the deceased and certain group members and by a general curiosity about death. We further discuss the implications close interactions with corpses have for disease transmission within and between gorilla social groups

    The Complex Trauma Questionnaire (ComplexTQ). Development and preliminary psychometric properties of an instrument for measuring early relational trauma

    Research on the etiology of adult psychopathology and its relationship with childhood trauma has focused primarily on specific forms of maltreatment. This study developed an instrument for the assessment of childhood and adolescence trauma that would aid in identifying the role of co-occurring childhood stressors and chronic adverse conditions. The Complex Trauma Questionnaire (ComplexTQ), in both clinician and self-report versions, is a measure for the assessment of multi-type maltreatment: physical, psychological, and sexual abuse; physical and emotional neglect as well as other traumatic experiences, such rejection, role reversal, witnessing domestic violence, separations, and losses. The four-point Likert scale allows to specifically indicate with which caregiver the traumatic experience has occurred. A total of 229 participants, a sample of 79 nonclinical and that of 150 high-risk and clinical participants, were assessed with the ComplexTQ clinician version applied to Adult Attachment Interview (AAI) transcripts.Initial analyses indicate acceptable inter-rater reliability. A good fit to a 6-factor model regarding the experience with the mother and to a 5-factor model with the experience with the father was obtained; the internal consistency of factors derived was good. Convergent validity was provided with the AAI scales. ComplexTQ factors discriminated normative from high-risk and clinical samples. The findings suggest a promising, reliable, and valid measurement of early relational trauma that is reported; furthermore, it is easy to complete and is useful for both research and clinical practice
